Applicants are requested to provide details of any previous business experience, where applicable. The table that follows provides descriptions/guidelines/instructions for the completion of each field contained in section J of the supplier database registration form: Field No. 1 2 Instructions and/or <strong>Guidelines</strong> Has the Business Conducted Business with a Public Sector Institution Before ü Applicants must complete this field by marking the appropriate box with an X. ü This field is a mandatory field. Previous Business Name ü Applicants who indicated “Yes” in 1 above must complete this field. ü Applicants are requested to provide the details of atleast one (1) but not more than three (3) references. ü For each reference, the Institution Name who awarded the applicant work, the order number, the order date, the value of the contract and a short description of the work completed is required. 3 Previous Core Business Experience ü Applicants who indicated “No” in 1 above must complete this field. ü Applicants are requested to provide adequate details of any previous experience of the business related to its core operations. 3.12. Section L: Declaration of Interest All applicants must complete a declaration of interest which has been included in section L of the supplier database registration form. This information is mandatory and required by the Department in terms of the Supply Chain Management regulations issued by National Treasury. 3.13. Section M: Certification of Correctness of Information All applicants must complete the certificate of correctness of information section in the supplier database registration form and ensure that the form is commissioned by a commissioner of oaths.
