REPORTING ALL WORK-RELATED INJURIES OR ILL <strong>HEALTH</strong> Employers must record and evaluate all accidents or harm, including “near misses” where injury or harm could have occurred. They are then required to investigate whether the incident was caused by a significant hazard (section 7(2)), and, if so, control the hazard. WAIKATO TIMES FOOD, WINE AND JAZZ FESTIVAL ACCIDENT PROCESS AN ACCIDENT / INCIDENT OR NEAR MISS OCCURS ... Call 111 if necessary and provide First Aid. Fill out the Accident Form / Serious Harm Form – kept in Waikato Times Marketing. This is then entered into the Accident register. A nominated Staff Member will have the responsibility of maintaining the First Aid Kit and its contents. Was there Serious Harm YES NO Don’t alter the scene — other than what is necessary to prevent further harm. Report event to OSH immediately (within 7 days). If the circumstances could have caused Serious Harm notify staff, take remedial measures and update the Hazard Register accordingly. Investigate the accident to find causes.
