Life and Accidental Death and Dismemberment Insurance<br />

To help you create a secure financial future for you and your dependents, <strong>Jones</strong> <strong>Lang</strong> <strong>LaSalle</strong> provides basic life and AD&D insurance at no cost<br />

to you at 2X your annual salary up to a max of $250,000(commission eligible employees receive flat rate. Refer to the Benefits Summary Guide<br />

on Connect for more information. Open Enrollment is your chance to elect or increase supplemental coverage, subject to approved evidence of<br />

insurability (EOI). The chart below highlights the coverage and eligibility requirements.<br />

Note: National, Regional and International Directors have different life insurance options and a separate <strong>enrollment</strong> process administered by Lenox<br />

Advisors and MetLife.<br />

Employee supplemental Life, Supplemental AD&D and Dependent Life Insurance options<br />

Employee Supplemental $50,000 - $700,000 You pay 100% (see chart below)<br />

Spouse/Domestic partner $10,000 - $150,000 up to 100% of your total insurance You pay 100% (see chart below)<br />

Child(ren) $5,000 - $25,000 You pay 100%; premium is the same regardless of the<br />

number of children covered.<br />
