Referral for bleeding varices<br />

Shrunken nodular liver. Enlarged spleen (16 cm) with varices around the hilum.<br />

Reverse flow is present in the portal vein and there is increased intra hepatic arterial flow. Patent right and<br />

middle hepatic veins - left technically difficult to demonstrate.<br />

Patent paraumbilical vein is noted. Gross ascites is present.<br />

Conclusion: ultrasound appearances are compatible with advanced liver disease with portal<br />

hypertension.<br />

Referral for Contrast Enhanced <strong>Ultrasound</strong><br />

The sonographer should ensure that the report describes: -<br />

• the behaviour of the lesion(s) in the arterial, portal and late phases<br />

• if the behaviour is benign or malignant and if possible conclude with the nature of the lesion e.g.<br />

haemangioma<br />

• cases where characterisation of small lesions (
