Medicare<br />

The term Medicare means the program of medical care<br />

benefits provided under Title XVIII of the Social Security Act<br />

of 1965 as amended.<br />

Necessary Services and Supplies<br />

The term Necessary Services and Supplies includes any<br />

charges, except charges for Bed and Board, made by a<br />

Hospital on its own behalf for medical services and supplies<br />

actually used during Hospital Confinement, any charges, by<br />

whomever made, for licensed ambulance service to or from<br />

the nearest Hospital where the needed medical care and<br />

treatment can be provided; and any charges, by whomever<br />

made, for the administration of anesthetics during Hospital<br />

Confinement.<br />

The term Necessary Services and Supplies will not include<br />

any charges for special nursing fees, dental fees or medical<br />

fees.<br />

individual other than a Physician who is licensed or otherwise<br />

are not limited to physical therapists, registered nurses and<br />

licensed practical nurses. Other <strong>Health</strong> Professionals do not<br />

include providers such as Certified First Assistants, Certified<br />

Operating Room Technicians, Certified Surgical<br />

Assistants/Technicians, Licensed Certified Surgical<br />

Assistants/Technicians, Licensed Surgical Assistants,<br />

Orthopedic Physician Assistants and Surgical First Assistants.<br />

Participating Pharmacy<br />

The term Participating Pharmacy means a retail Pharmacy<br />

with which <strong>CIGNA</strong> has contracted to provide prescription<br />

services to insureds, or a designated home delivery Pharmacy<br />

with which <strong>CIGNA</strong> has contracted to provide home delivery<br />

prescription services to insureds. A home delivery Pharmacy<br />

is a Pharmacy that provides Prescription Drugs through mail<br />

order.<br />

Participating Provider<br />

The term Participating Provider means a hospital, a Physician<br />

or any other health care practitioner or entity that has a direct<br />

or indirect contractual arrangement with <strong>CIGNA</strong> to provide<br />

covered services with regard to a particular plan under which<br />

the participant is covered.<br />

Nurse<br />

The term Nurse means a Registered Graduate Nurse, a<br />

Licensed Practical Nurse or a Licensed Vocational Nurse who<br />

has the right to use the abbreviation "R.N.," "L.P.N." or<br />

Other <strong>Health</strong> Care Facility/Other <strong>Health</strong> Professional<br />

The term Other <strong>Health</strong> Care Facility means a facility other<br />

than a Hospital or hospice facility. Examples of Other <strong>Health</strong><br />

Care Facilities include, but are not limited to, licensed skilled<br />

Patient Protection and Affordable Care Act of 2010 means the<br />

Patient Protection and Affordable Care Act of 2010 (Public<br />

Law 111-148) as amended by the <strong>Health</strong> Care and Education<br />

Reconciliation Act of 2010 (Public Law 111-152).<br />
