5.3.1.2. If the laboratory staff member was the main <strong>PulseNet</strong> contact, contact information for the<br />

person who will serve as the replacement/interim contact person should be provided.<br />

5.3.1.3. If the participant was analysis-certified, access to the national database(s) will be removed and<br />

the SecurID key fob must be returned to CDC.<br />

5.3.1.4. Access to the <strong>PulseNet</strong> Listserv will be removed<br />

5.3.1.5. Information will be removed from the PFGE contact list<br />

5.3.1.6. Farewell and/or change in duties announcement will be printed in <strong>PulseNet</strong> News and contact<br />

information will be removed from the newsletter distribution list<br />

5.3.1.7. If the laboratory staff member was promoted or reassigned to a different position within their<br />

current laboratory and will no longer be performing routine <strong>PulseNet</strong> duties, but will serve as a<br />

<strong>PulseNet</strong> backup, access to the <strong>PulseNet</strong> Listserv and the national database(s) (if participant<br />

was analysis-certified) may be retained; therefore the staff member should keep their assigned<br />

SecurID key fob. They will not be removed from the PFGE contact list, but their information<br />

should be updated and a note must be written in the information box to indicate this<br />

participant’s new role.<br />
