The <strong>Board</strong> will forward to the Ministry of Finance a copy of the selected proponent's signed Tax<br />
Compliance Declaration Form for verification. By signing this form, the proponent is consenting<br />
to the release of such information from the <strong>Board</strong> to the Ministry of Finance and from the<br />
Ministry of Finance to the <strong>Board</strong> for this purpose.<br />
Proponents may direct enquiries regarding the Tax Compliance Declaration Form to the <strong>Board</strong>’s<br />
Contact.<br />
3.2.4 Reference Form (Appendix F)<br />
Each proposal must include a Reference Form completed by the proponent according to the<br />
instructions contained in that form. The references should be from persons for whom the<br />
proponent has successfully provided similar goods and services to those described under<br />
Deliverables within the past three (3) years. The name and telephone number of a contact<br />
person for each reference and a brief outline of the nature of the goods and services provided<br />
should be included.<br />
The <strong>Board</strong>, in its sole discretion, may confirm the proponent’s experience and/or ability to<br />
provide the Deliverables required and described in its proposal by checking the proponent’s<br />
references.<br />
3.3 Stage II – Evaluation of Rated Criteria<br />
3.3.1 Rated Criteria<br />
The following is an overview of the categories and weighting for the Rated Criteria of the <strong>RFP</strong>:<br />

3.4 Stage IV - Evaluation of Pricing<br />
3.4.1 Pricing<br />

Pricing will be scored based on a relative pricing formula.<br />

Each proponent will receive a percentage of the total possible points allocated to price for the<br />
particular category it has bid on by dividing that proponent’s price for that category into the<br />
lowest bid price in that category. For example, if the lowest bid price for a particular category is<br />
$120.00, that proponent receives 100% of the possible points for that category (120/120 =<br />
100%), a proponent who bids $150.00 receives 80% of the possible points for that category<br />
