Today you have received your enrolment package which includes vital information that supports<br />
and enables your child’s enrolment at <strong>Meridan</strong> <strong>State</strong> <strong>College</strong>.<br />
Parents/Carers are required to make an enrolment interview with one of our Principals/Deputy<br />
Principals before enrolment can commence.<br />
If your family resides outside of the school management boundary, your enrolment cannot be<br />
considered for processing unless an exemption has been applied for and approved by the Exemption<br />
Committee, chaired by myself as <strong>College</strong> Director.<br />
48 hours prior to the enrolment interview, parents are required to return completed enrolment<br />
paperwork and the necessary supporting documentation to the main Executive office. Enrolment<br />
interviews are held on Wednesday mornings by appointment. If necessary, alternative times can be<br />
arranged.<br />
Please note that if any part of the enrolment form is not completed or supporting documentation<br />
not provided, the enrolment interview cannot occur until the necessary paperwork is completed<br />
and received.<br />
As this is an official <strong>Education</strong> Queensland document, the Principal reserves the right to render a<br />
student’s enrolment invalid, if incorrect information is provided in the enrolment application.<br />

PRIVACY AGREEMENT<br />
Enrolment<br />
The Department of <strong>Education</strong> collects this information for the purpose of school enrolment and<br />
student management. Personal information collected by the Department is protected by the<br />
Queensland Government’s Information Standard 42 – Information Privacy. However, in accordance<br />
with Information Sharing Protocols and Memoranda of Understanding, some of this information may<br />
be passed on to government agencies. Some of these state government agencies include<br />
Queensland Health, Queensland Transport, Queensland Police Service and Department of Families.<br />
The commonwealth government, through Centrelink, may require information for matching<br />
purposes in relation to the payment of study assistance benefits to some students. Personal<br />
information on the enrolment form can be disclosed to other third parties without the individual’s<br />
consent where authorised or required by law.<br />
Attendance/Achievement/Behaviour<br />
While students are enrolled in and attend state schools, school staff will collect personal<br />
information about their academic performance, attendance and behaviour for the purpose of<br />
monitoring their educational progress and providing educational programs to suit the needs of the<br />
student. Year 11 and 12 student details and academic profiles will be passed on to the Queensland<br />
Studies Authority for the purpose of processing Queensland Certificate of <strong>Education</strong> (QCE) and<br />
Tertiary Entrance <strong>State</strong>ments. Year 3, 5 and 7 student names are passed on to the Queensland<br />
Studies Authority for the purpose of issuing individual reports in relation to the assessment of<br />
numeracy and literacy skills of students. This personal information can be disclosed to other third<br />
parties without the individual’s consent where authorised or required by law.<br />
Wellbeing, Protection and Safety<br />
During a student’s attendance, the Department of <strong>Education</strong> may also collect personal information<br />
that relates to the wellbeing, protection and safety of the student. This personal information may<br />
be passed on to agencies such as Queensland Health, Queensland Police Service and Department of<br />
Families in accordance with <strong>Education</strong> Queensland’s Student Protection Policy and other policies<br />
relating to student behaviour. This personal information can be disclosed to other third parties<br />
without the individual’s consent where authorised or required by law.<br />
Children and Young People in the Care of the <strong>State</strong> – Data/Matching<br />
The Department of <strong>Education</strong> and Department of Families conduct a data-matching program to<br />
improve school achievement outcomes for children and young people in care. The data-matching<br />
program involves a comparison, through a unique identifier, of certain personal information of<br />
children and young people in care and those children of similar age in the general school student<br />
population, including those who have specific needs. This personal information may include:<br />
achievement levels; retention rates; age; school year levels; school disciplinary absences; and<br />
student movement between schools. You may obtain further information about the Queensland<br />
Government’s privacy regime contained in Information Standard 42 – Information Privacy at<br />

What is <strong>Education</strong> Queensland’s role<br />
The Department of <strong>Education</strong>, known as <strong>Education</strong> Queensland, operates the state education<br />
system through a network of more than 1300 sites and administers grants and funding on behalf of<br />
the Government to the non-state school sector and universities. The department also provides<br />
advisory, monitoring and regulatory functions and strategic support to these areas.<br />
Why does your privacy need protecting<br />
Every day, a vast amount of personal information is given to <strong>State</strong> Government departments and<br />
agencies. The Queensland Government respects your privacy and has established rules to ensure<br />
that your personal information is protected.<br />
The Government’s privacy policy (Information Standard 42 – Information Privacy) stipulates that<br />
personal information be collected, stored, used and disclosed under very strict guidelines to<br />
prevent it from being misused or passed on without your permission.<br />
<strong>Education</strong> Queensland is committed to protecting your privacy.<br />
What is personal information<br />
Personal information is an opinion or information about a person, whether true or false, that<br />
identifies or could identify the person. This information may be recorded in a document, or form<br />
part of a database. Examples of personal information include (but are not limited to) your date<br />
and place of birth, race or ethnicity, religion, financial record, criminal record or medical record.<br />
Sometimes personal information about a person can reveal their identity even if their name is not<br />
mentioned.<br />
How does the privacy policy work<br />
In all your dealings with <strong>Education</strong> Queensland, staff will make sure that your privacy is protected.<br />
Any personal information collected must be:<br />
• directly related to, or necessary for, a lawful purpose or function of the agency;<br />
• relevant, up-to-date and complete;<br />
• collected fairly and lawfully; and<br />
• stored safely and securely.<br />
Generally, your personal information must be used only for the purpose for which it was collected<br />
or a directly related purpose, unless you have otherwise given permission.<br />
The privacy principles do not apply to personal information contained in documents concerning<br />
covert police activity, witness protection, disciplinary actions and misconduct, whistleblowers and<br />
Cabinet and Executive Council matters. This is because the law overrides privacy principles in<br />
particular cases.<br />
Under what other circumstances can <strong>Education</strong> Queensland disclose your personal information<br />
If you consent, or if disclosure is: permitted by law; essential for law enforcement; or believed to<br />
be necessary to prevent or lessen an imminent threat to health or life.<br />
<strong>Education</strong> Queensland may disclose limited information (for example, name, contact details,<br />
attendance, achievement etc) about you to other parties for the purpose of issuing achievement<br />
awards, processing certificates, statements or reports in relation to assessments.<br />
Listed below are agencies <strong>Education</strong> Queensland may give relevant personal information to where<br />
needed or authorised under law. The guidelines for release of personal information are strict.<br />
Personal information may be disclosed to:<br />
Queensland Health or its nominated Health Providers, Queensland Transport, Queensland Police<br />
Service, Aboriginal and Torres Strait Islander Policy, Department of Employment and Training,<br />
Department Families, Disability Services Queensland, Department of Housing, Department of<br />
Justice and Attorney-General, Sport and Recreation Queensland, Centrelink and research<br />
organisations under contract to <strong>Education</strong> Queensland.
Data Matching<br />
<strong>Education</strong> Queensland passes some personal information collected on to Centrelink for data<br />
matching purposes in relation to entitlements to Centrelink-administered payments; for example,<br />
study assistance benefits. The Data-matching Program (Assistance and Tax) Act 1990 and the<br />
Social Security Administration Act 1999 authorises Centrelink to compare the personal information<br />
they hold to ensure that payments are being paid correctly. <strong>Education</strong> Queensland and the<br />
Department of Families conduct a data-matching program to improve school achievement outcomes<br />
for children and young people in care.<br />
The data-matching program involves a comparison of certain personal information of children and<br />
young people in care and those children of similar age in the general school student population,<br />
including those who have specific needs. Personal information may include achievement levels,<br />

Can you have access to personal information held by the Government<br />
Yes. You can apply under the Freedom of Information Act 1992 to access, or to correct personal<br />

the FOI Coordinator, <strong>Education</strong> Queensland, PO Box 33, Brisbane Albert Street, Q 4002 or telephone<br />

Worried about privacy<br />
If you have a question about privacy, confidentiality or access to information, contact <strong>Education</strong><br />

To ensure our students are well equipped to function in, and contribute fully to our current society and<br />
information economy, <strong>Meridan</strong> <strong>State</strong> <strong>College</strong> and <strong>Education</strong> Queensland are responding to the<br />
innovation directions of the Smart <strong>State</strong> Strategy through Smart Classrooms. This strategy focuses<br />
innovative programs and resources used by the <strong>College</strong>, teachers and students. An essential tool for our<br />
<strong>College</strong> in the provision of innovative educational programs is the utilisation of intranet, internet and<br />
network services. This includes the growing number of Virtual Classrooms our students utilise through<br />
the Learning Place and the integration of these technologies in the investigation and collaboration<br />
phases of many curriculum programs. Therefore, access to these technologies is an increasingly<br />
essential part of the modern educational program provided in schools.<br />
In order to maintain a safe and supportive learning environment, students need to accept the<br />
responsibility that comes with being given access to these resources, and understand that this access will<br />
be removed if not used appropriately.<br />

network, including trespassing in another person’s files, home drive or e-mail.<br />
Divulge personal information (e.g. name, parent’s name, address), via the internet or e-<br />
mail, to unknown entities or for reasons other than to fulfil the educational program<br />
requirements of the school.<br />
Make deliberate attempts to access restricted files, disrupt the <strong>College</strong> network system,<br />
destroy data or gain unauthorised data.<br />
I also understand that it is not acceptable, and will NOT:<br />
Use images of students in the <strong>College</strong> uniform, or images taken at <strong>College</strong><br />
activities/lessons, without the permission of the <strong>College</strong> Executive Team (eg. Putting<br />
photos or video taken at school on My Space, YouTube, Facebook or other social<br />
networking sites).<br />
Use USB or other memory storage devices to bring non-educational files onto the <strong>College</strong><br />
network (eg games, music, images…).<br />
Deliberately waste printing and Internet resources.
Damage computers, printers or the network equipment.<br />
Use <strong>College</strong> resources to play games, access or play unauthorised music, download<br />
applications or other forms of software, or to procure without the express permission of<br />
the <strong>College</strong> Executive Team.<br />
Use personal devices to create wireless hotspots to circumvent the secure <strong>College</strong> network.<br />
I further understand that access to the <strong>Meridan</strong> <strong>State</strong> <strong>College</strong> network by student’s wholly privately<br />
owned ICT devices could compromise the integrity of the <strong>Education</strong> Queensland network, and is<br />
therefore not permitted.<br />
Students need to be prepared for the possibility of unanticipated access to harmful information,<br />
materials or approaches from unknown persons via the internet (e.g. SPAM or viruses and Trojans with<br />
other files from the net). It is therefore a condition of use that students must report accidental access<br />
to offensive, illegal, harmful information and approaches from unknown persons via the internet to the<br />
supervising teacher or school staff member.<br />
<strong>Meridan</strong> <strong>State</strong> <strong>College</strong> reserves the right to restrict/remove student access to the intranet, extranet,<br />
internet or network facilities if parents or students do not adhere to this network, facilities and<br />
equipment usage policy. This may also include other college behavioural consequences (eg. Suspension)<br />
as outlined in <strong>College</strong>’s Responsible Behaviour Plan for Students and as seen as appropriate by the<br />
<strong>College</strong> eLearning Head of Department, Community Leaders and members of the <strong>College</strong> Executive<br />
Team.<br />
ICT Network, Facilities and Equipment Agreement<br />
The <strong>College</strong> and the Department monitor access to and usage of the ICT network. For example, e-mail<br />
monitoring will occur to identify inappropriate use, protect system security, maintain system<br />
performance, determine compliance with <strong>State</strong> and departmental policy and determine compliance with<br />
<strong>State</strong> and Federal legislation and regulation.<br />
Student Agreement<br />
I understand that the school’s ICT network provides me with access to a range of essential learning tools,<br />
including the internet. I understand that the internet can connect me to useful information stored on<br />
computers around the world. While I have access to the school’s ICT network: I will use it only for<br />
educational purposes; I will not undertake or look for anything that is illegal, dangerous or offensive; and<br />
I will not reveal my password or allow anyone else to use my school account.<br />
Specifically in relation to internet usage, should any offensive pictures or information appear on my<br />
screen I will close the window and immediately quietly inform my teacher, or at home tell my<br />
parents/guardians.<br />
If I receive any inappropriate emails at school I will tell my teacher or if I receive any at home I will tell<br />
my parents/guardians.<br />
When using e-mail or the internet I will not:<br />
reveal names, home addresses or phone numbers – mine or that of any other person;<br />
use the school’s ICT network (including the internet) to annoy or offend anyone else.<br />
I understand that if the school decides I have broken the rules for using its ICT network, appropriate<br />

1 CONSENT GIVEN
On behalf of the individual identified in Section 6 of this Consent Form (the Individual), the person or<br />
persons signing this Consent Form (the Signatory)* grant consent to the Department of <strong>Education</strong>,<br />
Training and Employment (the Department) and to any other Department or Agency of the <strong>State</strong> of<br />
Queensland (the <strong>State</strong>) to use, record and disclose the Individual’s:<br />

copyright material, including their written, artistic or musical works or video or sound recordings<br />
(Individual work).<br />
*Note: If the Individual is under 18 years of age, the Signatory must be a parent or guardian of the<br />
Individual. The Individual must also sign if he or she is under 18 and able to give and understand the<br />
consent. If the Individual is 18 or older, the Signatory and the Individual will be the same person.<br />
2 PURPOSE<br />
This consent applies to any use, recording or disclosure of the Individual’s personal information or<br />
Individual work, in connection with the Department or the <strong>State</strong>, for the following purposes:<br />

events, including, for example, dramatic or musical performances, sporting activities and award<br />

any other activities identified in further schedules attached to this Consent Form.<br />
3 DURATION<br />
This consent will continue:<br />

for an Individual under 18 years of age, until the Individual turns 18; or<br />
until the Individual revokes consent by writing to the Principal of the school or the Institute Director of<br />
the TAFE Institute, or, in the case of employees, by writing to the District Office;<br />
and where more than one of these events may apply, until the first event occurs.<br />
Despite the above, if, at the time such an event occurs, the Department or the <strong>State</strong> is using the<br />
Individual’s personal information or Individual work, or the <strong>State</strong> has entered into contractual obligations in<br />
relation to that material, the consent will continue in relation to that material until the use by Department or<br />
the <strong>State</strong> is complete or until the contractual obligations come to an end.<br />
4 UNDERSTANDINGS<br />

‘Use’ includes:<br />
- to create, make copies of, reproduce or retain in any form, including by camera, video, digital<br />
recorder, webcam, closed circuit television, mobile phone or any other device; and<br />
- to distribute, publish or communicate in any form, including in newsletters and other print media,<br />
television and the internet,<br />
in whole or in part, and to permit other persons to do so.<br />
The Department or the <strong>State</strong> will not pay the Signatory or the Individual for giving this consent or for the<br />
use of the Individual’s personal information or Individual work.<br />
This Consent Form revokes and replaces all previous consent forms in relation to the Individual.<br />
<br />
Nothing in this Consent Form limits the rights that the Department or the <strong>State</strong> reserve in relation to the<br />
use of the Individual’s personal information, Individual work or other intellectual property under any<br />
other law.
The ‘Department’ and the ‘<strong>State</strong>’ include the officers and employees of the Department and the <strong>State</strong><br />
engaged in performing services for the Department and the <strong>State</strong>.<br />
This consent extends to the Department and the <strong>State</strong>:<br />
- disclosing the Individual’s personal information and Individual work to the Department’s and the<br />
<strong>State</strong>’s agents, contractors and volunteers for the purpose of performing services for the<br />
Department and the <strong>State</strong>; and<br />
- permitting those persons to use, record and disclose such material to the same extent as the<br />
Department and the <strong>State</strong> are entitled to deal with the Individual’s personal information and<br />
Individual work.<br />
5 LIMITATIONS ON CONSENT<br />

to do so, but takes no responsibility for circumstances in which it is misled as to the identity or authority or<br />

Principal of the school or the Institute Director at which the Individual is enrolled or works or send a request<br />
in writing to the District Office.<br />
8 INFORMATION<br />
What is this consent for<br />
This Consent Form authorises the Department and the <strong>State</strong> to use the Individual’s personal information<br />
and copyright material, together with information about the Individual’s participation in Departmental and<br />
<strong>State</strong> initiatives, for any use by the Department and the <strong>State</strong> associated with the purposes identified in<br />
Section 2 of this Consent Form. The consent covers the entire or partial use of the Individual’s personal<br />
information and copyright material in conjunction with other words and images.<br />
For example, the Individual’s personal information and copyright material may appear in school or TAFE<br />
newsletters, magazines, websites and other school, TAFE, departmental or <strong>State</strong> publications, as well as<br />
in television advertising, videos, brochures, forms, public relations displays, annual reports, press<br />
advertising, internal documents such as manuals, websites, certificates and strategic plans, and posters<br />
and other promotional material. There may also be occasions on which the Department may approve the<br />
media, such as local newspapers and television stations, using information and copyright material in<br />
relation to the Individual (for example, where the Individual is involved in dramatic or musical<br />
performances, sporting activities or award ceremonies).<br />
The Department has specific policies and guidelines in relation to publication of student images on the<br />
Internet (see, for example, Web Publishing for Schools and Publishing Student and Staff Information on<br />
School Web Sites). These policies and guidelines may be viewed at www.education.qld.gov.au.<br />
What is copyright material<br />
An Individual’s copyright material may include written work (e.g. stories and poems), paintings, pictures,<br />
drawings, designs, photographs, videos, films, music, performance, recordings, computer programs,<br />
websites, sculptures, fashion, metal- or wood-works made by them or to which they contributed. In the<br />
case of students, it includes, but is not limited to, work that they create in the course of their studies during<br />
the time they are enrolled at a <strong>State</strong> school. These materials may form part of their academic assessment<br />
or be part of their studies generally and may attract copyright.<br />
The Department understands that students and volunteers generally own the intellectual property rights in<br />
the material they create and that this Consent Form is not meant to transfer the Individual’s ownership of<br />
the intellectual property in their copyright material.<br />
This Consent Form does not provide for copyright consent in relation to copyright works an<br />
Individual creates in the course of employment (whether or not the material is created in normal work<br />
hours or using departmental facilities or equipment). Where copyright material is created by a <strong>State</strong><br />
employee while performing their duties under the terms of their employment, the copyright is owned by the<br />
<strong>State</strong> as the employer (section 35 Copyright Act 1968). There are limited exceptions to this including, for<br />
example, where the copyright material is created by an employee pursuant to a prior agreement with the<br />
employer.<br />
Generally, the deciding factor is whether the employee is performing their official duties. In addition, section<br />
176 of the Copyright Act 1968 applies where the work was created by or under the direction or control of<br />
the <strong>State</strong>. However, moral rights may still apply to copyright material created by an employee. The<br />
Queensland Public Sector Intellectual Property Guidelines provide further information on intellectual<br />
property. If as an employee you have any further queries about the ownership of the intellectual property in<br />
respect of the works you create you should contact the Legal and Administrative Law Branch.<br />
What is personal information<br />
Personal information includes information or opinions, whether true or not, about an individual whose
identity is apparent or can reasonably be ascertained from the information or opinion. This includes the<br />
Individual’s name, recording or image. It also includes the Individual’s educational information such as the<br />
Individual's assessment and results, and health information and court orders provided to the Department,<br />
where such information may enable the Individual to be identified.<br />
Why is a Schedule used<br />
A Schedule may be attached to this Consent Form describing a specific use of the Individual’s copyright<br />
material or personal information, either because the use falls outside the purposes listed in Section 2 or to<br />
confirm the consent in this form.<br />
What happens to the Consent Form once it is completed and signed<br />
The Consent Form is retained by the Department and it will be placed on the Individual’s file and or the<br />
project file. The Individual or Signatory may request a copy of the signed form by contacting the Principal of<br />
the school or the Institute Director of the TAFE Institute, or in the case of employees, by sending a request<br />
in writing through the District Office.<br />
What if I give my consent and later change my mind<br />
This consent will be in effect for the period described in Section 3 of the Consent Form. The consent can<br />
be modified or withdrawn at any time by writing to the Principal of the school or the Institute Director of the<br />
TAFE Institute, or, in the case of employees, by writing to the District Office. However, any changes will<br />
apply only from the date that the Department receives any consent withdrawal. Any existing material will<br />
not be withdrawn from use if the Department or the <strong>State</strong> is currently using the material or where the<br />
Department or the <strong>State</strong> has entered into contractual obligations in relation to this material. In such cases<br />
the withdrawal will be effective after the Department or the <strong>State</strong>’s use is complete or after the contractual<br />
obligations come to an end.<br />
Privacy<br />
Your consent to the recording, use and disclosure of the Individual’s personal information and Individual<br />
work is required in accordance with the Copyright Act 1968 (Cth), the <strong>Education</strong> (General Provisions) Act<br />
2006 and the Information Privacy Act 2009. Personal information will be stored securely. The Department<br />
will only disclose the Individual’s personal information in accordance with your consent, except where<br />
authorised or required by law. If you wish to access or correct any of the personal information on this form<br />
or discuss how it has been dealt with, or if you have a concern or complaint about the way the Individual’s<br />
personal information has been collected, used, stored or disclosed, please contact the relevant<br />
organisation, school or TAFE Institute.

Hello and welcome to <strong>Meridan</strong> <strong>State</strong> <strong>College</strong>. We hope your family will enjoy your time here with us and<br />
you will enjoy becoming a part of this wonderful school community.<br />
The P&C Association is an integral part of the <strong>College</strong> and we are very proud of the role the parents have<br />
played in shaping our school. We meet in the Rainforest Retreat on the second Tuesday of the month at<br />
7:00pm Our meetings are a terrific way of finding out what is happening in the school and what is being<br />
planned. All parents/carers and community members are welcome to attend.<br />
There are many ways in which you can become involved in the school and as a family new to the area<br />
this is also a great way to meet new people. You may like to help out in class room activities, in the<br />
Resource Centre, in the café, coaching teams or fundraising. Please let us know if there is any other<br />
area you may have some expertise and we will happily help you find some use for that. As our <strong>College</strong> is<br />
still relatively new there are many areas where your help would be appreciated.<br />
<strong>Meridan</strong> <strong>State</strong> <strong>College</strong> is a wonderful school to be a part of and we hope your children settle in well. The<br />
P&C pre-loved Uniform Shop is open Tuesday, Thursday Afternoons 2:00 – 3:30pm and Friday mornings<br />
from 8:15am -10:00am. Our Rainforest Café operates each day.<br />
