Get Ready for the 2012 Medicare Part D Drug<br />
and Medicare Advantage Plans New Fall Open<br />
Enrollment dates which are Oc<strong>to</strong>ber 15 <strong>to</strong> December<br />

During this Fall Open Enrollment, people with<br />
Medicare have the right <strong>to</strong> change their Medicare<br />
health and drug coverage options without restrictions.<br />
<strong>The</strong>ir changes will take effect on January 1,<br />
2012.<br />
You should review all <strong>you</strong>r coverage options<br />
even if <strong>you</strong> are happy with <strong>you</strong>r current coverage,<br />
because plans change their costs and<br />
benefits every year.<br />
Medicare Supplements (often called Medigap) only change premiums not coverage.<br />
You should receive the changes from <strong>you</strong>r Drug plans and Medicare Advantage Plans by Oc<strong>to</strong>ber 1.<br />

public policy are important <strong>to</strong> <strong>you</strong>, we invite <strong>you</strong> <strong>to</strong> consider a bequest through <strong>you</strong>r will or living trust <strong>to</strong> <strong>The</strong> <strong>ALS</strong> <strong>Association</strong><br />
Blueprint Chapter. Below is language <strong>you</strong> can take <strong>to</strong> <strong>you</strong>r financial advisor for including our Chapter in <strong>you</strong>r will<br />
or living trust:<br />
General bequest.<br />
This is an outright bequest and is usually a gift of a stated sum of money.<br />
Sample language: I give $25,000 <strong>to</strong> <strong>The</strong> <strong>ALS</strong> <strong>Association</strong> , <strong>Greater</strong> Sacramen<strong>to</strong> Chapter, Sacramen<strong>to</strong>, CA for its general<br />
purposes.<br />
Percentage of remainder.<br />
This bequest gives a percentage of what is left of <strong>you</strong>r estate after taking care of loved ones.<br />
Sample language: I give <strong>The</strong> <strong>ALS</strong> <strong>Association</strong>, <strong>Greater</strong> Sacramen<strong>to</strong> Chapter, Sacramen<strong>to</strong>, CA __ percent of the rest,<br />
residue and remainder of my estate, for its general purposes.<br />
Contingent bequest.<br />
You can arrange <strong>you</strong>r bequest so that it is made on the condition that a certain event must occur before distribution <strong>to</strong><br />
our Chapter.<br />
Sample language: I give $20,000 <strong>to</strong> my daughter Shelly, provided she survives me; otherwise, I give it <strong>to</strong> <strong>The</strong> <strong>ALS</strong> <strong>Association</strong>,<br />
<strong>Greater</strong> Sacramen<strong>to</strong> Chapter, Sacramen<strong>to</strong>, CA for its general purposes.<br />
