Things to Know… Informatics Corner The redesign of the admission and shift assessments chapters, ADL, Pain, Neuro check and CV check forms is in full swing. If you have not already seen the drafts of the revised forms, please contact your representatives to the Staff Informatics Council (aka Superusers) as soon as possible. There will be major changes in documentation! The goal is concise and effective documentation with no duplication or unnecessary fields and more time for our patients! So far we have removed about 300 questions from the current forms, changed many questions to a check box format, and streamlined answer choices. The actual build of these new forms is slated to begin this month and continue for about 8 weeks. We need everyone’s feedback to make these the best they can be! If you don’t know who your representative is, please send Kathy Zopf-Herling an email at kzopfherling@cchosp.com. A big welcome to the Labor and Delivery staff, who are about to launch nursing documentation in Soarian! L&D staff will be using the Labor and Delivery Admission Assessment, Labor Record, Epidural Flow Sheet, Intake and Output form, Vital Signs and Clinical Notes. Classes for all staff are starting November 12 th and will continue to early December. Classes are being taught by Annette Pappas, Amber Evans, Liz Waterhouse, Rebecca Wagner-Campbell, and Kathy Zopf-Herling. Anticipated “Go Live” date is December 11 th. and a formalized, evidence-based approach to the care of patients with specific disease processes. The Chester County Hospital has decided to target stroke, heart failure, acute myocardial infarction, diabetes, orthopedics, and wound care as programs for certification. Specialized teams have been meeting to look at our processes and outcomes for each of these disease specific categories. The teams are charged with defining and refining processes for clinical care, patient self-management, clinical information management and compliance with national patient safety goals. Another huge piece of the certification puzzle involves the development of CPGs – Clinical Practice Guidelines – which are specific to the disease process and address nationally recognized standards of care. DSC is a major undertaking. The teams need you to help make this endeavor successful. Scrub the HUB for any IV/CVC access with pressure and friction for 15 seconds with alcohol and allow to dry. 2. Scrub entire hub tip, including luer lock and grooves EVERYTIME you start a piggyback, give an IV push, anytime you need to access any CVC or PIV.
