Dental Hygiene Associate of Applied Science 2014 REQUIRED DOCUMENTS—STEP 1 THIS IS A CHANGE FROM LAST YEAR. PLEASE READ CAREFULLY. 1. Fill Out a General Admissions Form Online—Applicants who are new to MHCC or have not attended within the last 4 terms MUST fill out a general admissions form (https://my.mhcc.edu/ics/Admissions), select General Studies as your major . Please submit the general admissions form prior to submitting your dental hygiene application. Dental hygiene applications without a recent admissions form on file will be incomplete and not allowed to move forward. REQUIRED DOCUMENTS—STEP 2 2. Application Packet Checklist (page 3 of application) 3. Allied Health Division Application (page 4 of application) 4. Prerequisite Course Planning Sheet (page 5 of application) 5. Program Experience/Observation Form and Related Dental Experience. A minimum of 20 hours of observation in a dental setting is required, 10 of which need to be with a hygienist (page 6 of application). In addition to the items above, applicants must also submit the following: 6. $25.00 Application Fee. Non refundable. Make check payable to MHCC. You may also pay in person at Student Services (AC2253) with a bank card or cash. 7. Personal Resume ‐ All applicants must submit a personal resume with their application materials. Your resume should include both related and unrelated work experience along with any volunteer information. College coursework completed at a college without regional accreditation will not be considered for admissions or graduation requirements; however, it may be used toward proof of related education/experience. For assistance with your resume, contact the Career Planning and Counseling Center at 503‐491‐7432. 8. College Coursework • All college transcripts must indicate a minimum cumulative GPA of 2.50 or higher. • College courses taken as pass/fail, satisfactory/unsatisfactory or audit do not fulfill admission criteria for the Dental Hygiene program. CLEP and AP courses will be accepted as a “C” grade. Applicants will earn 1 point for each AP and CLEP course that is a direct admission or program requirement in science, math, or writing. • All science courses that are required for admissions or for which points are assessed must be completed with a grade “C” or higher, and not taken prior to Winter 2007. 9. Academic Completion ‐ Students must show proof of academic completion. Provide one of the following: • Official high school transcripts showing date of graduation or official GED record indicating an overall pass score. • Home‐schooled applicants must meet this requirement by completion of the GED test. • Official college transcript documenting degree completion (a certificate is not proof of academic completion, it must be an associate degree or higher). • Students must submit their official high school transcripts to earn high school discretionary points. • No photocopies are allowed. If a current official GED/high school transcript is on file, you do not need to submit another copy. 10. Transcript Requirements • Transcripts from every college attended, regardless of program relevancy, program length, or age, must be included for an application to be complete. Transcripts must be official (unopened) and issued directly by the accredited institution (official electronic (EDI) transcripts are fine). Do NOT include MHCC transcripts. • Secondary transferred coursework is not accepted. Original transcripts must accompany all earned, attempted, or audited college credits. • Nationally accredited (for‐profit institutions) transcripts are not required and will not fulfill program or degree requirements, but may be submitted. • If an up‐to‐date transcript is on file (showing most recently completed coursework with nothing in progress) applicants do not need to submit another copy. • Transcripts from foreign institutions must be evaluated before the application deadline by an outside credential services. For more information, visit (www.aacrao.org) or (www.naces.org). A foreign bachelor’s degree does not fulfill the skills proficiencies requirement. Applicants must demonstrate proficiencies in reading, writing, and math by one of the ways listed below: Subject Option 1 ‐ CPT placement into: Option 2 ‐ Completion of: Option 3: Reading* RD117 RD115 or higher, earning a “B” grade or better, not taken prior to Winter term 2011 Bachelor’s Degree Writing WR121 WR115 or higher, earning a “B” grade or better, not taken prior to Winter term 2011 Bachelor’s Degree Math MTH095 MTH065 or higher with a “B” grade or better and not taken prior to Winter term 2011 N/A * Regardless of college coursework completed, students MUST meet the reading proficiency by one of the three ways listed above. MAKE SURE YOU MEET THIS REQUIREMENT! • The Accuplacer College Placement Test (CPT) is a non‐timed skills assessment test given on a walk‐in basis. There is no fee to take the CPT. Your scores will be available upon completion of the test. • CPT scores taken on or prior to 12/23/2011 do not count as the CPT results are only valid for two years. If the CPT date expires on the application deadline date or before, the CPT will need to be taken again. • Applicants who live more than two hours away may take the test by proctor. Please call the MHCC Testing Services for more information at 503‐491‐7678. • If you have COMPASS scores from another school, you will not need to take MHCC’s Accuplacer CPT. Please submit your COMPASS score report with your application packet, and we will convert your test results for use at MHCC. • If you are only taking portions of the CPT, you will need to obtain a “waiver” form by bringing appropriate transcripts to the Academic Advising and Transfer Center, Room AC 2253. • Test scores are not on official college transcripts. You will need to contact your college’s testing center to obtain your reading, writing, and math scores for use at MHCC. • Students who choose to retest in reading, writing or math will need to complete a Retest Self‐Referral Form, which is available in the Testing Center or Academic Advising. WORK EXPERIENCE AND INTERVIEW WITH PROGRAM FACULTY The top 50 applicants will be invited to attend a group interview session with program faculty. This is a professional interview. Applicants must be prepared to respond to questions verbally and in writing. Written responses will be evaluated for neatness, legibility, grammar, spelling, and content. For those applicants who interview, program faculty will be awarding points at that time for work and volunteer experience.
