REQUIRED DOCUMENTS—STEP 1<br />

THIS IS A CHANGE FROM LAST YEAR. PLEASE READ CAREFULLY.<br />

1. Fill Out a General Admissions Form Online—Applicants who are new to MHCC or have not attended within the last 4 terms<br />

MUST fill out a general admissions form (https://my.mhcc.edu/ics/Admissions), select General Studies as your major . Please<br />

submit the general admissions form prior to submitting your dental hygiene application. <strong>Dental</strong> hygiene applications without a<br />

recent admissions form on file will be incomplete and not allowed to move forward.<br />

3. Allied Health Division Application (page 4 <strong>of</strong> application)<br />

4. Prerequisite Course Planning Sheet (page 5 <strong>of</strong> application)<br />

5. Program Experience/Observation Form and Related <strong>Dental</strong> Experience. A minimum <strong>of</strong> 20 hours <strong>of</strong> observation in a dental<br />

setting is required, 10 <strong>of</strong> which need to be with a hygienist (page 6 <strong>of</strong> application).<br />

In addition to the items above, applicants must also submit the following:<br />

6. $25.00 Application Fee. Non refundable. Make check payable to MHCC. You may also pay in person at Student Services<br />

(AC2253) with a bank card or cash.<br />

7. Personal Resume ‐ All applicants must submit a personal resume with their application materials. Your resume should<br />

include both related and unrelated work experience along with any volunteer information. College coursework completed at<br />

a college without regional accreditation will not be considered for admissions or graduation requirements; however, it may<br />

be used toward pro<strong>of</strong> <strong>of</strong> related education/experience. For assistance with your resume, contact the Career Planning and<br />

Counseling Center at 503‐491‐7432.<br />

8. College Coursework<br />

• All college transcripts must indicate a minimum cumulative GPA <strong>of</strong> 2.50 or higher.<br />

• College courses taken as pass/fail, satisfactory/unsatisfactory or audit do not fulfill admission criteria for the <strong>Dental</strong> <strong>Hygiene</strong><br />

program. CLEP and AP courses will be accepted as a “C” grade. Applicants will earn 1 point for each AP and CLEP course that is<br />

a direct admission or program requirement in science, math, or writing.<br />

• All science courses that are required for admissions or for which points are assessed must be completed with a grade “C” or<br />

higher, and not taken prior to Winter 2007.<br />

9. Academic Completion ‐ Students must show pro<strong>of</strong> <strong>of</strong> academic completion. Provide one <strong>of</strong> the following:<br />

• Official high school transcripts showing date <strong>of</strong> graduation or <strong>of</strong>ficial GED record indicating an overall pass score.<br />

• Home‐schooled applicants must meet this requirement by completion <strong>of</strong> the GED test.<br />

• Official college transcript documenting degree completion (a certificate is not pro<strong>of</strong> <strong>of</strong> academic completion, it must be an<br />

associate degree or higher).<br />

• Students must submit their <strong>of</strong>ficial high school transcripts to earn high school discretionary points.<br />

• No photocopies are allowed. If a current <strong>of</strong>ficial GED/high school transcript is on file, you do not need to submit another copy.<br />

10. Transcript Requirements<br />

• Transcripts from every college attended, regardless <strong>of</strong> program relevancy, program length, or age, must be included for an<br />

application to be complete. Transcripts must be <strong>of</strong>ficial (unopened) and issued directly by the accredited institution (<strong>of</strong>ficial<br />

electronic (EDI) transcripts are fine). Do NOT include MHCC transcripts.<br />

• Secondary transferred coursework is not accepted. Original transcripts must accompany all earned, attempted, or audited<br />

college credits.<br />

• Nationally accredited (for‐pr<strong>of</strong>it institutions) transcripts are not required and will not fulfill program or degree requirements,<br />

but may be submitted.<br />

• If an up‐to‐date transcript is on file (showing most recently completed coursework with nothing in progress) applicants do not<br />

need to submit another copy.<br />

• Transcripts from foreign institutions must be evaluated before the application deadline by an outside credential services. For<br />

more information, visit (www.aacrao.org) or (www.naces.org). A foreign bachelor’s degree does not fulfill the skills pr<strong>of</strong>iciencies<br />

requirement.<br />
