Exposure (limited dorsal approach)<br />

• Make incision over 4th dorsal compartment tendons.<br />

• Develop interval between 3rd and 4th, or 4th and 5th<br />

compartments.<br />

• Transpose EPL if needed.<br />

• Expose the dorsal cortex of the distal radius.<br />

Direct Fixation of Dorsal Fragments<br />

• Place Buttress Pin upside down on dorsal cortex to determine<br />

insertion location and angle for legs.<br />

• Insert a 1.1mm (.045”) K-wire to prepare hole for each leg and<br />

confirm position with X-ray.<br />

• Using Pin Clamp, snap Buttress Pin into Wire Plate.<br />

• Cut legs of Buttress Pin to desired length, leaving one leg slightly<br />

longer than the other.<br />

Insertion of Buttress Pin<br />

• Starting with longer leg, withdraw corresponding K-wire and<br />

immediately insert leg of Buttress Pin.<br />

• Repeat with shorter leg.<br />

• Complete seating with Pin Impactor.

Fracture Reduction and Final Fixation<br />

• Reduce fracture and fix proximally with 2.3mm bone screw.<br />

• If necessary, place a second screw through Wire Plate.<br />

TIPS<br />

• Buttress Pin can also be used to directly<br />

buttress a free articular fragment.<br />

• For lowest profile, a standard washer can be<br />

used instead of Wire Plate. Seat screw until<br />

washer flexes to ensure fixation of Wire Form.<br />

• The Wire Form can be modified with Wire<br />

Bender or Pin Clamp.
