Cover image: iStock To say that we are currently in the midst of an obesity crisis leaves me wondering what we will call it in 10 to 15 years times, when it is predicted that almost three-quarters of men and two-thirds of women in the UK will be overweight or obese. And these figures show no signs of slowing down with experts predicting that obesity will soon overtake smoking as the principal cause of cancer. The economic burden is crippling. Obesity costs the country nearly £47bn a year and at current rates the cost to the NHS could increase from between £6bn and £8bn in 2015 to between £10bn and £12bn in 2030. Yet bewilderingly the UK spends less than £638m a year on obesity prevention programs –that’s around 1% of what it is currently costing us. Things do not look good. The environment in which we live is designed to make us fat; encouraging us to overeat the wrong foods and discouraging us from physical activity by the many energy saving devices we have developed. We can blame an evolutionary mismatch between the way our bodies have evolved, and the ways the environments in which we now live and work have evolved. Without bringing about urgent changes to this environment the crisis is only going to get worse. So we must act. Getting people to lose weight once they have become obese is hard; making sure people do not get obese in the first place should be easier, but will require significant intervention including government legislation on education, advertising, food production and healthcare, and hard work for all of us. Our environment doesn’t change solely through chance but also through intention, and the source of this change can come from anyone. It will take a mixture of science, education, and intelligent collective action. I hope this supplement inspires us all to get started. Dr Christian Jessen 3
