Research and Continuing Pr<strong>of</strong>essional<br />

Development at AECC<br />

2001 – 2003<br />

Since the reorganisation <strong>of</strong> research at AECC in 1999, further developments took place in the autumn <strong>of</strong><br />

2001. This reflected the pivotal role <strong>of</strong> research activity both in the pr<strong>of</strong>essional development <strong>of</strong> Faculty at<br />

AECC and in the delivery <strong>of</strong> postgraduate education and continuing pr<strong>of</strong>essional development (CPD). As<br />

a result, a single department within the AECC was established to provide postgraduate education, training<br />

and research opportunities to maintain and enhance the pr<strong>of</strong>essional skills and competencies <strong>of</strong> chiropractors,<br />

and to provide research and other scholarly activities for the pr<strong>of</strong>essional development <strong>of</strong> AECC staff. The<br />

Department <strong>of</strong> Research and Pr<strong>of</strong>essional Development was established in October 2001, and for the first time<br />

the AECC Research Report is expanded to include Postgraduate Studies.<br />

The period covered in this Report (2001-2003) has been productive and exciting. Research activity has continued<br />

and in some areas expanded, maintaining the reputation <strong>of</strong> the AECC as a research centre <strong>of</strong> excellence<br />

among chiropractic educational institutions worldwide. Research at the AECC is evidenced in the main by<br />

the Faculty who have either completed or are currently enrolled on PhD and Masters programmes, by<br />

publications in the peer-reviewed literature and by platform and poster presentations at national and<br />

international research conferences. A record <strong>of</strong> these is given in the first half <strong>of</strong> the Report. The AECC is<br />

committed to research as a means <strong>of</strong> providing a stimulating and intellectual environment in which<br />

undergraduate students, postgraduate students and Faculty alike all learn. Research ensures that teaching<br />

staff are current and up-to-date in their respective disciplines, and that as a result, the education provided at<br />

AECC is <strong>of</strong> the highest standards and quality.<br />

Since the establishment <strong>of</strong> the Department in 2001, there has been a striking expansion <strong>of</strong> the postgraduate<br />

education and training portfolio. The AECC has developed, in collaboration with the University <strong>of</strong> Portsmouth,<br />

two new postgraduate Masters programmes and a new Pr<strong>of</strong>essional Doctorate programme. The latter is the<br />

first <strong>of</strong> its kind in chiropractic education worldwide, meeting the demand for more pr<strong>of</strong>essionally relevant<br />

programmes in research. Further details <strong>of</strong> these innovative developments in postgraduate education can be<br />

found in the second half <strong>of</strong> the Report. In addition to these award-bearing programmes, the Department has<br />

implemented an impressive CPD portfolio. These short courses and workshops in a wide range <strong>of</strong> subject and<br />

skills-based areas for qualified chiropractors are delivered in the main by AECC Faculty, again reflecting the<br />

expertise, energy and commitment <strong>of</strong> our teaching staff. Again, details <strong>of</strong> these CPD events are recorded in<br />

the second half <strong>of</strong> the Report.<br />

This Report reflects the considerable expertise, talent, ability and effort in research and postgraduate education<br />

expended by our staff at AECC during their working and pr<strong>of</strong>essional lives. I am very proud as Director <strong>of</strong> the<br />

Department to be able to present this Report and take this opportunity to sincerely thank everyone who has<br />

contributed to research and to postgraduate studies over the last two years. I would also like to thank the<br />

Department’s Research Associate, Mrs. Denise Stys-Norman, for her expert skills in compiling the Report, and<br />

the Departmental Administrator, Mrs. Georgina Enright, for her invaluable support in managing the postgraduate<br />

education and training programmes.<br />

Texas Visit<br />

In 2001, Dr. Jeff Bagust took a month’s study leave to<br />

visit the laboratories <strong>of</strong> Pr<strong>of</strong>essor W G Willis in<br />

Galveston, Texas. Pr<strong>of</strong> Willis is one <strong>of</strong> the world’s<br />

top experimental spinal physiologists, and his<br />

laboratory specialises in pain physiology, with<br />

particular emphasis on inflammatory pain. A major<br />

area <strong>of</strong> current interest is the role that nervous activity<br />

travelling backwards along sensory nerves from the<br />

spinal cord to the periphery – the Dorsal Root Reflex –<br />

may have in the generation <strong>of</strong> peripheral<br />

inflammation.<br />

Jeff had worked on the dorsal root reflex for many years<br />

before joining the AECC, and was invited by Pr<strong>of</strong> Willis<br />

to visit Galveston to investigate the pharmacology <strong>of</strong><br />

the dorsal root reflex using an isolated spinal cord<br />

preparation that he has developed. Whilst there, Jeff<br />

gave a seminar on “The Isolated Spinal Cord and the<br />

Dorsal Root Reflex”.<br />

The visit was very successful. It was shown that the<br />

sensory nerves leaving the spinal cord exhibit two<br />

different patterns <strong>of</strong> antidromic (backwards)<br />

discharges, which can be distinguished using a<br />

convulsant agent, muscimol. In the absence <strong>of</strong><br />

muscimol, the dorsal root reflex consists <strong>of</strong> bursts <strong>of</strong><br />

large amplitude action potentials, but this activity is<br />

abolished by low concentrations <strong>of</strong> muscimol (2µM)<br />

and replaced by activity in nerve fibres producing very<br />

small action potentials, typical <strong>of</strong> those carrying pain<br />

signals. It seems probable that it is this small amplitude<br />

activity that is responsible for the spread <strong>of</strong><br />

inflammatory reaction and pain to sites distant from<br />

an area <strong>of</strong> damaged tissue.<br />

Cutaneous Sensitivity<br />

Palpation is one <strong>of</strong> the major investigative techniques<br />

used by chiropractors, and a considerable amount<br />

<strong>of</strong> time is spent by students at the AECC learning<br />

palpatory techniques. Palpatory skills are essentially<br />

subjective and difficult to quantitate, but a series <strong>of</strong><br />

three BSc projects (PS Chandhok, IE Foster, MC de<br />

Jager) have attempted to measure aspects <strong>of</strong> sensory<br />

acuity that might contribute to palpatory ability and<br />

be expected to change with training.<br />

Two parameters have been measured; 2 point<br />

discrimination threshold (2-PD)- a measure <strong>of</strong> the<br />

minimum distance between a pair <strong>of</strong> points touched<br />

on the tip <strong>of</strong> the index finger that can be identified as<br />

two, and not one; and the thickness <strong>of</strong> paper through<br />

which a thin nylon mon<strong>of</strong>ilament can be accurately<br />

located (the Palpation Threshold).<br />

Comparison <strong>of</strong> 1 st year students who had no training<br />

in palpation, with 5 th year students who were using<br />

palpation techniques in the clinic on a daily basis,<br />

showed that the experienced group were able to<br />

identify two points touched on the skin more easily<br />

than the inexperienced students, and they were also<br />

able to locate the filament under a greater thickness<br />

<strong>of</strong> paper.<br />

This indicates that the fingers <strong>of</strong> the 5 th year students<br />

were more sensitive than those <strong>of</strong> the 1 st year students,<br />

giving support to the suggestion that it is possible to<br />

improve palpatory ability by appropriate training.<br />

Rod and Frame<br />

The Rod and Frame test is a measure <strong>of</strong> an individual’s perception <strong>of</strong> verticality. Subjects are required to<br />

rotate a rod to a vertical position within a square frame. Errors in orientation are produced when the outer<br />

frame is tilted 10° - 20°. In a recent publication, researchers at CMCC have shown that patients with neck<br />

pain produce greater errors on the Rod and Frame test than asymptomatic controls. They suggest that this test<br />

might be a useful tool in assessing neck problems and the effects <strong>of</strong> therapy.<br />

All studies using the Rod and Frame test so far have used cumbersome mechanical systems, requiring specialized<br />

facilities. In collaboration with Dr Kim Humphreys (Dean <strong>of</strong> Research at CMCC), Jeff has developed a computer-<br />

based version <strong>of</strong> the Rod and Frame test, which has the advantages <strong>of</strong> ease <strong>of</strong> use and portability. The test<br />

can either be performed in a darkened room using a conventional VDU screen, or using a pair <strong>of</strong> video<br />

glasses.<br />

Results <strong>of</strong> preliminary trials on asymptomatic subjects show results very similar to those <strong>report</strong>ed in the literature<br />

using mechanical systems. Investigations involving subjects with neck problems are planned in the near<br />

future.<br />

All healthcare pr<strong>of</strong>essionals now recognise the need for CPD to ensure that they maintain the necessary<br />

skills and competencies throughout pr<strong>of</strong>essional life. The chiropractic pr<strong>of</strong>ession is now on the verge <strong>of</strong><br />

introducing mandatory CPD in order for practitioners to retain their registration with the national regulatory<br />

bodies. It is generally viewed within the pr<strong>of</strong>ession that on one hand, time away from practice, travel and<br />

cost are negative aspects <strong>of</strong> engaging in venue-based CPD events but on the other hand, social and<br />

pr<strong>of</strong>essional interaction on a<br />

personal level is a very positive<br />

aspect. In addition, previous studies<br />

have identified a potential<br />

requirement for alternative modes<br />

<strong>of</strong> delivery <strong>of</strong> CPD learning activities<br />

such as distance learning and use<br />

<strong>of</strong> the Internet (Bolton 2002).<br />

As part <strong>of</strong> the staff development<br />

initiative at AECC, Dr. Ian Brown<br />

undertook a Masters degree in<br />

education specialising in teaching<br />

and learning on the Internet. This<br />

brought knowledge and expertise<br />

into the college that would allow<br />

future development <strong>of</strong> on-line<br />

courses for both distance learning<br />

for CPD and in-house<br />

undergraduate computer-assisted<br />

learning.<br />

The Master’s course was undertaken<br />

at the Department <strong>of</strong> Education,<br />

University <strong>of</strong> Sheffield, a leading<br />

school in the implementation <strong>of</strong><br />

technology in education. The course<br />

itself was a distance learning<br />

exercise with all modules carried out<br />

on-line including the project viva-<br />

voce and graduation! The four<br />

modules making up the course<br />

consisted mainly <strong>of</strong> practical action<br />

research relevant to the students’<br />

individual pr<strong>of</strong>essional practice, and<br />

developing the relevant skills to both<br />

develop on-line courses and to<br />

implement their delivery through<br />

virtual learning environments (Brown<br />

2003). The final module consisted <strong>of</strong><br />

a short research project (shorter in<br />

scope and duration than a typical<br />

science or medical Master’s<br />

project).<br />

Ian Brown’s dissertation was entitled ‘A survey <strong>of</strong> chiropractors’ and final year chiropractic<br />

students’ attitudes to on-line learning before and after participating in an on-line learning<br />

event, either as independent learners or collaborative learners’.<br />

The study’s aims and objectives were to investigate and compare chiropractors’ attitudes to learning on-line<br />

before and after taking part in a short on-line course relevant to their clinical practice, having worked either<br />

as independent learners or as part <strong>of</strong> a collaborative group <strong>of</strong> learners. In particular, the study set out to<br />

identify the perceived positive or negative aspects <strong>of</strong> chiropractors’ respective experiences and whether or<br />

not these were relevant to views held about the delivery <strong>of</strong> CPD in general.<br />

Two cohorts <strong>of</strong> volunteers were recruited for the study. Thirty senior intern undergraduate students from the<br />

<strong>Anglo</strong>-<strong>European</strong> <strong>College</strong> <strong>of</strong> <strong>Chiropractic</strong>, who were about to enter practice and 30 qualified practitioners<br />

currently attending voluntary weekend CPD events at the same institution. All participants were sent an initial<br />

questionnaire to provide data about their previous experiences <strong>of</strong> and attitudes to, using the Internet or other<br />

on-line resources as part <strong>of</strong> their learning and practice. A second questionnaire was sent to all participants<br />

after they had all completed a short on-line course within a web-based virtual learning environment specifically<br />

authored for the study. Fifteen <strong>of</strong> the undergraduates and fifteen qualified practitioners undertook the course<br />

as independent learners and the remainder engaged with identical course material but with the freedom to<br />

collaborate with their peers within that group through a text based chat room and discussion forum.<br />

The data from the first questionnaire showed that undergraduate students used the Internet more as a part <strong>of</strong><br />

their learning compared to the qualified chiropractors. Although the frequency <strong>of</strong> use was greater in the<br />

undergraduate cohort, the type <strong>of</strong> resources used was similar for both groups. Both groups identified that<br />

convenience, lower cost and flexibility <strong>of</strong> learning were advantages to learning on-line and that isolation and<br />

lack <strong>of</strong> contact with colleagues, peers and tutors, lack <strong>of</strong> technical support and feedback were a<br />

disadvantage. Overall, undergraduate interns expressed less negative concerns than did the qualified<br />

practitioners.<br />

Data from the second questionnaire showed that most respondents generally thought that the learning event<br />

had been a positive or very positive experience, with no significant difference between the individual or<br />

collaborative learning sets. A greater number <strong>of</strong> qualified chiropractors remained neutral, saying it had been<br />

neither a positive nor negative experience. No respondents claimed it had been a negative experience. All<br />

groups indicated that they thought the level <strong>of</strong> support received during the learning event was satisfactory.<br />

There were however, significant differences as to the level <strong>of</strong> support that different groups thought that they<br />

had received. The highest level <strong>of</strong> satisfaction was shown by the undergraduate and practitioner collaborative<br />

learning groups. The lowest was shown by the undergraduate and practitioner independent learning groups.<br />

This difference in perception within the collaborative learning groups may be due to peer interactions within<br />

the learning sets.<br />

In conclusion, the results <strong>of</strong> this study show an overall positive attitude by chiropractors to learning on-line.<br />

Collaboration and cooperation with peers and tutors when learning on-line may partially satisfy some <strong>of</strong> the<br />

concerns shown about the perceived fear <strong>of</strong> isolation and lack <strong>of</strong> support associated with distance learning.<br />

Attention should be paid to the less computer-competent learners so as not to exclude or disadvantage them<br />

in any future on-line CPD distance learning events.<br />

The aim <strong>of</strong> this project is to understand the effects<br />

<strong>of</strong> muscle fatigue and whiplash on cervical<br />

function and to identify the effects <strong>of</strong> muscle<br />

fatigue and cervical whiplash on the vestibular<br />

(balance) and cervical somatosensory (neck)<br />

components <strong>of</strong> neck function. This should eventually<br />

provide pr<strong>of</strong>essionals involved in treating patients that<br />

have suffered a whiplash injury with additional<br />

information necessary to optimise the rehabilitative<br />

process.<br />

The first part <strong>of</strong> the past year has been spent on<br />

investigating the validity and reliability <strong>of</strong> the<br />

equipment. This was followed by the first<br />

experimentation, which has been written up and<br />

submitted for publication. (Gosselin, Rassoulian and<br />

Brown, 2003)<br />

The second and third experiments are presently being<br />

concluded. These are: “Effects <strong>of</strong> neck extensor<br />

muscles fatigue on head repositioning” and<br />

“Electromyography <strong>of</strong> the neck extensor muscle during<br />

an isometric contraction”. During the next months, a<br />

specially developed linear oscillator used in the<br />

production <strong>of</strong> vestibular fatigue will be validated.<br />

The Intra-Articular Synovial Folds <strong>of</strong> the<br />

Cervical Spine<br />

Autopsy studies <strong>of</strong> the victims <strong>of</strong> motor vehicle<br />

trauma have demonstrated damage to the<br />

synovial joints <strong>of</strong> the cervical spine, including<br />

haemarthrosis, contusions <strong>of</strong> the intra-articular synovial<br />

folds, rupture <strong>of</strong> the joint capsule and occult fractures<br />

<strong>of</strong> the articular processes. Thus these structures are<br />

considered to be possible sources <strong>of</strong> neck pain and<br />

headache following trauma to the cervical spine.<br />

Previous studies have examined the intra-articular<br />

synovial folds <strong>of</strong> the cervical spine in vitro. However,<br />

different methods <strong>of</strong> morphological investigation have<br />

been used and the descriptions and classifications <strong>of</strong><br />

the synovial folds are conflicting. Hence,<br />

themorphology <strong>of</strong> these structures, their function and<br />

their clinical relevance remains a controversial issue.<br />

During the last 12 months work has progessed in the<br />

Department <strong>of</strong> Anatomical Sciences (University <strong>of</strong><br />

Southampton) developing a technique for assessing<br />

the morphology <strong>of</strong> the intra-articular synovial folds in<br />

the cervical spine. The data that have been collected<br />

are currently being analysed in conjunction with<br />

histological material. Preliminary Magnetic Resonance<br />

Imaging (MRI) experiments have been conducted at<br />

Southampton General Hospital and will continue over<br />

the course <strong>of</strong> 2003.<br />

The Role <strong>of</strong> Eye:Neck Coordination in the<br />

Rehabilitation <strong>of</strong> Cervical Spine Pain<br />

Neil Osborne, BSc(Chiro), DC, FCC(Orth), FRSH<br />

(PhD Candidature, Brighton University)<br />

Over the last few decades, researchers have<br />

noted that individuals <strong>report</strong>ing chronic neck<br />

pain (especially that <strong>of</strong> traumatic onset) seem<br />

to have a disruption in the reflexive relationship<br />

between eye and cervical spine movement.<br />

Under normal circumstances, fast gaze (saccades) <strong>of</strong><br />

the eyes in the direction <strong>of</strong> a neck movement<br />

precedes the actual neck movement itself. This<br />

cervico-ocular reflex causes the eyes to fix on an<br />

object and, throughout the full range <strong>of</strong> the neck<br />

movement, necessarily leads to a slow contralateral<br />

gaze <strong>of</strong> the eyes. It is this reflex association that some<br />

have noted becomes disrupted in cervical spine pain<br />

patients. Indeed, it has been <strong>report</strong>ed that saccadic<br />

movement may become ‘interrupted’ following a few<br />

days <strong>of</strong> immobilisation in a cervical collar.<br />

Others have devised exercises to reinforce this<br />

eye:neck relationship and have demonstrated an<br />

improvement in both neck pain and reflex control<br />

following a period <strong>of</strong> training. Indeed, with practise, it<br />

seems that a blindfolded patient may become better<br />

at the task <strong>of</strong> accurate head repositioning and be able<br />

to reduce their neck pain in the process.<br />

Whilst more and more work is being developed in this<br />

field, there is a considerable lack <strong>of</strong> tangible evidence<br />

to clearly underpin the actual relationship between<br />

eye and neck movement. Indeed, it is yet to be firmly<br />

established whether eye movement is truly impaired<br />

in the manner that it is claimed, in neck pain patients.<br />

The aim <strong>of</strong> this study is therefore to investigate the<br />

relationship between eye and neck movements in<br />

both normal individuals and cervical spine pain<br />

patients.<br />

Study to Document Outcomes Following Cervical Spinal Manipulation<br />

Treatments by Chiropractors<br />

To date there is controversy regarding the risks associated with manipulation to the cervical spine. The evidence<br />

that does exist originates mainly from reviews <strong>of</strong> individual case studies and medico-legal insurance claims<br />

data. (Haldeman, Carey, Townsend and Papadopoulos, 2001). This study aims to document outcomes<br />

prospectively from a large number <strong>of</strong> consecutive cervical spine manipulation treatments administered by<br />

chiropractors in field practices. The study is a large-scale observational design, similar to that conducted on<br />

the safety <strong>of</strong> acupuncture in UK clinics <strong>report</strong>ed in the British Medical Journal in 2001. (MacPherson, Thomas,<br />

Walters and Fitter, 2001; White, Hayhoe, Hart and Ernst, 2001).<br />

The study aims to recruit 500 chiropractors, each <strong>of</strong> whom will <strong>report</strong> on either 100 consecutive cervical<br />

manipulation treatments or consecutive cervical manipulation treatments over a six-week data collection<br />

period, whichever is sooner. Outcomes will be monitored immediately following the treatment using a<br />

specifically designed data capture form. This will include patient demographics, type <strong>of</strong> manipulation and<br />

area <strong>of</strong> the cervical spine manipulated, change in presenting symptoms and onset <strong>of</strong> new symptoms. Any<br />

serious adverse event will necessitate the completion <strong>of</strong> an adverse event <strong>report</strong>ing form. On the subsequent<br />

visit, data will be captured on treatment-related outcomes that may have occurred in the intervening period.<br />

Again, any serious adverse event will be recorded. The study will be implemented in four phases: i) compilation<br />

<strong>of</strong> documentation and ethics approval; ii) peer-review; iii) feasibility study; and iv) main study. Data analysis in<br />

the main will be descriptive, but will also include sub-group and time series analyses.<br />

The documentation, which includes Data Collection Forms and an Operations Manual, has undergone extensive<br />

revisions and was finalised in November 2002. Ethics approval for the study was obtained from the University <strong>of</strong><br />

Portsmouth and ethical opinion from the Eastern Multi-centre Research Ethics Committee was received in<br />

November 2002. For the second phase, a multidisciplinary peer review panel was constituted, including field<br />

chiropractors, chiropractic and biomedical researchers and statisticians. Following revisions based on these<br />

responses from the panel, the feasibility study will start in summer 2003. This will involve a small number <strong>of</strong><br />

randomly selected field practitioners to determine the feasibility <strong>of</strong> implementing the research design in the<br />

routine practice setting. The main study will begin in Autumn 2003.<br />

The Paediatrics Programme at AECC has a new look with the addition <strong>of</strong> a<br />

Master’s degree. Accredited by Portsmouth University and coordinated by<br />

Joyce Miller as well as featuring numerous acclaimed guest lecturers (see<br />

panel), this three-year part-time programme leads Master’s students to gain<br />

expertise in diagnostic and therapeutic skills to create comfort and satisfaction<br />

in treating the enigmatic problems <strong>of</strong> the paediatric patient. Upon successful<br />

completion, the Master’s students will lead the pr<strong>of</strong>ession in qualifications and<br />

expertise to provide optimal care to the smallest members <strong>of</strong> the family.<br />

Chiropractors pride themselves on the ability to fully<br />

understand the anatomy and biomechanics <strong>of</strong> the<br />

spine and extremities <strong>of</strong> the human body in order to<br />

successfully diagnose and manipulate for a healing<br />

outcome. Most undergraduate programmes are<br />

limited in the amount <strong>of</strong> tuition devoted to the special<br />

health issues <strong>of</strong> the paediatric population. It is well<br />

documented that children are not merely small adults<br />

and that specialized study is required to create<br />

expertise in this unique arena.<br />

More and more, parents are turning to conservative<br />

therapies for their children, and the pr<strong>of</strong>ession must<br />

adequately prepare its practitioners to meet the<br />

needs <strong>of</strong> this patient population. Consistent with<br />

parental demand, there is also a growing demand<br />

from health care workers. According to the AECC electronic data collection system, health care workers refer<br />

44% <strong>of</strong> paediatric patients less than two months <strong>of</strong> age. Infants less than six months <strong>of</strong> age represent the<br />

largest group <strong>of</strong> paediatric patients presenting to AECC (See Figure 1).<br />

Paediatrics at AECC is taught in a<br />

graduated step-wise fashion. At the base<br />

is a 26 hour required undergraduate<br />

course just before the students enter their<br />

clinical training. The course is designed<br />

to teach the student the necessary skills<br />

to enable him/her to conduct a<br />

meaningful history and examination <strong>of</strong><br />

various age groups (between 1 day and<br />

16 years <strong>of</strong> age) and to assess their<br />

growth and development. It is also<br />

expected that upon completion <strong>of</strong> the<br />

course, the student will be familiar with<br />

common congenital and acquired<br />

disorders <strong>of</strong> the paediatric patient (with<br />

emphasis on musculoskeletal and<br />

nervous system disorders) as well as the<br />

social, behavioural and emotional<br />

problems, which are common to<br />

children.


Elective Paediatrics at AECC<br />

While the students are in their clinical year, they are<br />

able to diagnose and treat a wide variety <strong>of</strong> clinical<br />

disorders in the paediatric<br />

population simply because<br />

there is a large paediatric<br />

patient base that presents<br />

to the clinic. At least 12.4%<br />

(numbering approximately<br />

6200) <strong>of</strong> the clinic’s patients<br />

are under 16 years <strong>of</strong> age.<br />

To support the interns, an<br />

elective course is <strong>of</strong>fered<br />

two hours per week in the<br />

clinical year that deals with<br />

all aspects <strong>of</strong> both routine<br />

and unusual cases that are<br />

seen in the chiropractic<br />

environment. A paediatrician has agreed to consult<br />

on difficult clinical cases that are seen in the AECC<br />

teaching clinic.<br />

Elective and Post-Graduate Paediatric Seminars<br />

at AECC<br />

New in 2002 is an undergraduate elective and<br />

postgraduate seminar series involving all aspects <strong>of</strong><br />

chiropractic clinical care <strong>of</strong> the paediatric patient.<br />

This seminar series is designed for the student or<br />

practising chiropractor that wishes to enhance or<br />

update his or her paediatric education. Issues such as<br />

growth and development, paediatric orthopaedics,<br />

radiology, immunization, foetal development, birth<br />

process and birth trauma as well as treatment for the<br />

pregnant woman are covered in depth in weekend<br />

seminars. Certificate or Diploma level awards are given<br />

depending on attendance and fulfilling educational<br />

requirements.<br />

Master’s Degree<br />

The Master’s degree in <strong>Chiropractic</strong> Paediatrics serves<br />

as the pinnacle <strong>of</strong> paediatric education <strong>of</strong>fered at<br />

AECC (see figure below). The Master’s Programme is<br />

itself in its infancy; six students courageously began<br />

this course <strong>of</strong> study in the autumn <strong>of</strong> 2002. Weekend<br />

seminar course work imbedded in self-study and<br />

research, the hallmarks <strong>of</strong> Master’s level study, will<br />

prepare these students to set a precedent for state-<br />

<strong>of</strong>-the-art clinical care for the paediatric patient.<br />

<strong>Chiropractic</strong> History:<br />

A New Focus for Research at the AECC<br />

Research at the AECC has traditionally focused mainly on clinical and basic science topics, most frequently<br />

employing quantitative methodologies. Recently however, new fields <strong>of</strong> research and alternative<br />

methodologies have been given serious consideration. The inclusion <strong>of</strong> chiropractic history as an accepted<br />

field for academic study at the <strong>College</strong> is a good example <strong>of</strong> this.<br />

Within the <strong>European</strong> context, concern was raised in the<br />

1990s that chiropractic history had been widely<br />

neglected by researchers. <strong>European</strong> chiropractic lacked<br />

detailed written histories. At a General Council Meeting<br />

<strong>of</strong> the <strong>European</strong> Chiropractors’ Union (ECU) in May 1997,<br />

it was unanimously agreed that it was essential to begin<br />

working towards just such a history. Dr Kyrre Myhrvold<br />

(1998) wrote: “A pr<strong>of</strong>ession without a written history is a<br />

spineless pr<strong>of</strong>ession. Pride and identity in one’s pr<strong>of</strong>ession<br />

is based on knowledge <strong>of</strong> the past and the goals for<br />

the future.” 1<br />

The AECC is now taking a leading role in this field. Francis<br />

Wilson, a member <strong>of</strong> AECC Faculty, is undertaking an<br />

MPhil/PhD in which he is examining the history <strong>of</strong><br />

chiropractic’s pr<strong>of</strong>essionalization in Britain, a project that<br />

is receiving ECU funding. Francis has also been<br />

instrumental in establishing a History Library at the<br />

<strong>College</strong>, made possible by financial support from the<br />

Treatment a Month (TAM) Club. Dr Michael Copland-Griffiths, Chairman <strong>of</strong> the General <strong>Chiropractic</strong> Council,<br />

<strong>of</strong>ficially opened the Library on 7th March 2003. By<br />

coincidence, the most convenient date for the opening<br />

was DD Palmer’s birthday, suggesting that the library might<br />

have the blessing <strong>of</strong> chiropractic’s Founder!<br />

chiropractic historical data from across Europe. It is<br />

intended not only for the use <strong>of</strong> students and staff at the<br />

Gonstead Master Classes at the AECC<br />

Over this last year, the AECC joined forces with the Gonstead Clinical Study Society<br />

(GCSS) in a unique and innovative programme to present a series <strong>of</strong> seminars on<br />

the Gonstead technique system. The first discussions for the project took place early<br />

in 2001 when Dr Greg Plaugher, the Director <strong>of</strong> Research for the GCSS, visited the UK and<br />

took time out <strong>of</strong> his schedule to visit the AECC and present an afternoon technique workshop.<br />

A brainstorming session took place after the workshop about the best method <strong>of</strong> organising<br />

a presentation <strong>of</strong> the Gonstead Technique system in the UK. It was decided that a series <strong>of</strong><br />

ten seminars over a period <strong>of</strong> one year presenting the system in a regional approach could<br />

be a viable proposition. That was the beginning <strong>of</strong> the hard work <strong>of</strong> organising, co-ordinating,<br />

promoting and presenting the series. Within two months, dates were chosen and ten<br />

chiropractors from six different states in the US were selected as presenters.<br />

By November the series was up and running. The first two weekends were an overall introduction to Gonstead,<br />

presented by, Dr. Greg Plaugher, the editor <strong>of</strong> “ The Textbook <strong>of</strong> Clinical <strong>Chiropractic</strong>, A Specific Biomechanical<br />

Approach and Dr. Mark Lopes who co-edited the book. In the early spring, the regional approach <strong>of</strong> the<br />

technique series was started by looking at the pelvis, and then working our way up through the body including<br />

weekends on both the upper extremity and lower extremity. Each weekend included assessing, adjusting,<br />

and managing specific conditions for the region discussed. The final weekend <strong>of</strong> the series addressed the<br />

problems <strong>of</strong> ‘special populations’ that included geriatrics and paediatrics. To conclude, the final weekend<br />

was a written assessment covering the whole course. To be eligible for the assessment, participants had to<br />

attend at least eight <strong>of</strong> the ten weekends.<br />

Participants ranged from very experienced field practitioners to student<br />

interns and even a few students in the earlier years at the <strong>College</strong> who<br />

qualified to attend by their extensive participation in the Gonstead Study<br />

Club at the AECC. All participants seemed to share the same feelings <strong>of</strong><br />

accomplishment <strong>of</strong> the refining <strong>of</strong> adjustive technique skills. Not only<br />

the students, but practitioners also voiced their appreciation <strong>of</strong> the<br />

hands-on opportunity to practise and have the opportunity to have<br />

feedback on the refinement <strong>of</strong> their technique.<br />

We hope that in the future the AECC can collaborate further with the<br />

GCSS to <strong>of</strong>fer more Gonstead weekends. If there is support from the<br />

chiropractic pr<strong>of</strong>ession during the next several workshops, further<br />

programmes will be scheduled. Hopefully we can continue what we<br />

have started and continue to promote the feeling <strong>of</strong> enthusiasm for<br />

chiropractic that occurs during each <strong>of</strong> the weekends.<br />

Presentations at Research Conferences<br />

The AECC has always been well represented at international research conferences. Over the last two<br />

years, Faculty at AECC has made important and significant contributions to the ECU Research Conferences<br />

in Oslo (May 2002) and Heidelberg (May 2003), and to the WFC Conference in Orlando (May 2003).<br />

ECU Conferences-Oslo 2002 and Heidelberg 2003<br />

The 2002 conference was held at the impressive Holmenkollen Park Hotel overlooking the city <strong>of</strong> Oslo. Five<br />

members <strong>of</strong> AECC Faculty were invited speakers at the conference, providing approximately one-third <strong>of</strong> the<br />

conference’s plenary programme, and half <strong>of</strong> the workshop programme. The 2003 conference was held in the<br />

beautiful town <strong>of</strong> Heidelberg in Southern Germany. On this occasion, six <strong>of</strong> the AECC Faculty were invited to<br />

give presentations and workshops, once again making the major contribution to the conference programme.<br />

Both these events reflect the expertise <strong>of</strong> the Faculty at the AECC and the support and contribution that<br />

organisers <strong>of</strong> the ECU conferences have come to expect from us. The annual ECU Conference is the major<br />

research platform for chiropractic research in Europe. It not only affords members <strong>of</strong> Faculty the opportunity<br />

to present their work and network with other researchers in the chiropractic pr<strong>of</strong>ession, but also to demonstrate<br />

our support and commitment to chiropractic research in Europe.<br />

WFC Conference-Orlando 2003<br />

The WFC’s 7 th Biennial Congress was held in early May in Orlando, Florida.<br />

Pr<strong>of</strong>essor Jenni Bolton and Dr Haymo Thiel represented the AECC and<br />

Pr<strong>of</strong>essor Alan Breen represented the Institute for Musculoskeletal Research<br />

and Clinical implementation (IMRCI). In addition to making presentations,<br />

we received two research awards. Alan received the prestigious WFC<br />

Research Honour Award for his considerable contribution to chiropractic<br />

research over the last twenty years. Hugh Hurst and Jenni Bolton won<br />

third prize in the original research paper competition for their work on<br />

the “Clinical Significance <strong>of</strong> Outcome Measures”. Again, our presence<br />

at the Conference and the recognition <strong>of</strong> our work through these awards<br />

helps maintain our reputation as a chiropractic research centre <strong>of</strong><br />

excellence not only in Europe, but throughout the world.<br />

LEARNING FOR CHIROPRACTORS<br />

WORLDWIDE<br />

The World Wide Web <strong>of</strong>fers a new dimension for distance learning, providing<br />

easy access for self–paced study to chiropractors undertaking CPD<br />

wherever in the world they are. The AECC is making use <strong>of</strong> internet learning<br />

in CPD, and is now launching its first two innovative distance–based CPD<br />

courses in Radiology.<br />

To undertake the course, chiropractors must register with AECC.<br />

This will give access to a set number <strong>of</strong> test cases with questions.<br />

Answers are sent by email to the tutor who marks and feedbacks<br />

to the individual. Successful completion <strong>of</strong> each package is<br />

awarded a Certificate and CPD credits (UK).<br />

Postgraduate Studies<br />

Postgraduate studies at the AECC can be categorised into award-bearing programmes on one hand, and<br />

short courses/workshops on the other. Award-bearing programmes lead to academic qualification such as a<br />

Masters or a Doctorate, and are normally undertaken part-time over a period from three to six years. In contrast,<br />

short courses/workshops are <strong>of</strong>ten held over one or two days and are awarded CPD points based on attendance<br />

rather than assessment.<br />

Masters and Doctorate programmes<br />

In 2001, two new postgraduate programmes were developed in collaboration with colleagues at the University<br />

<strong>of</strong> Portsmouth. The Pr<strong>of</strong>essional Doctorate (PD) in <strong>Chiropractic</strong> was based on the unique PD programme<br />

pioneered at the University in Portsmouth to address the need for research at Doctorate level in the wider<br />

context <strong>of</strong> pr<strong>of</strong>essional practice, rather than the narrow confines <strong>of</strong> a monograph thesis typical <strong>of</strong> the traditional<br />

PhD. The PD allows research students to develop advanced skills in research methods, publication and<br />

dissemination, and CPD, before going on to the second part <strong>of</strong> the programme in which practice-based<br />

research is carried out and a Doctoral thesis produced. An advantage <strong>of</strong> this programme is that the first part<br />

<strong>of</strong> the programme is delivered in a multidisciplinary format in which chiropractors can develop their research<br />

skills alongside other healthcare pr<strong>of</strong>essionals.<br />

The other new programme, again developed in collaboration with colleagues at the University <strong>of</strong> Portsmouth,<br />

is the MSc Advanced Pr<strong>of</strong>essional Development (APD) in <strong>Chiropractic</strong>. This course enables chiropractors to<br />

develop advanced skills in CPD at the same time as advanced clinical skills within a pr<strong>of</strong>essional development<br />

framework. Like the PD, the core skills are taught in a multidisciplinary environment, enabling chiropractors to<br />

interact with a variety <strong>of</strong> healthcare pr<strong>of</strong>essionals. The first chiropractors were enrolled on the Masters and PD<br />

programmes in September 2001.<br />

In 2002, another new Masters programme was developed at the AECC, this time specifically to meet the<br />

demands for advanced skills in the area <strong>of</strong> chiropractic paediatrics. Using the framework <strong>of</strong> the original MSc<br />

APD in <strong>Chiropractic</strong>, the MSc APD in <strong>Chiropractic</strong> Paediatrics incorporates core modules in chiropractic<br />

paediatrics. The Course Leader for this programme is Dr Joyce Miller, whose expertise and energy in this subject<br />

area has made the programme a great success. The first cohort <strong>of</strong> chiropractors was enrolled in September<br />

2002.<br />

Short Courses/Workshops<br />

The AECC is a major provider <strong>of</strong> CPD for the chiropractic pr<strong>of</strong>ession in the UK, and over the last two years both<br />

the scope <strong>of</strong> subject areas and the number <strong>of</strong> events have risen sharply. CPD events are typically held over<br />

one or two days, normally at weekends, and mainly delivered by Faculty at AECC. Subject areas vary from the<br />

academic such as Evidence-based Practice to skills-based such as Dry Needling. In addition, a number <strong>of</strong><br />

longer courses have been held such as the Certificate/Diploma in <strong>Chiropractic</strong> Paediatrics and the Gonstead<br />

Technique Series. A full list <strong>of</strong> CPD events held in the period September 2001 to August 2003. As shown on page<br />

43 and 44.<br />

An exciting innovation in CPD has been the recent development <strong>of</strong> distance-based learning packages by Dr<br />

Ian Brown at the AECC. These can be accessed by chiropractors throughout the world on the World Wide<br />

Web, and CPD points awarded for successful completion <strong>of</strong> the programmes. So far, we have developed two<br />

radiology packages in which chiropractors can work through practice cases before completing test cases<br />

that are then assessed by an expert. We hope that this development marks the beginning <strong>of</strong> new ways for<br />

chiropractors to learn from their own practices and homes, and so change the face <strong>of</strong> CPD for the future.<br />
