The projectassures that Hawaiian, Filipino, Pacific Islander, Hispanic, and teen women andtheir families receive the care they need to reduce infant morbidity andmortality. The services provided by the project encompass outreach, casemanagement, and health education; these include screening for various socialand health issues by nurses, social workers, and social service aides. Eligibility: Pregnant teens and women Target population (but not limited to): Hawaiian, Filipino, Pacific Islander,and Hispanic, high-risk groups Fee: FreePublic Health NursingThese services are provided to individuals and families with barriers to healthcare, infants, toddlers and school-age children with chronic and/or complexmedical conditions, high-risk pregnant women, and the frail dependent elderly.
