Eligibility: Children of Hawaiian or part-Hawaiian ancestry Target Population: Preference is given to pregnant women, families with orexpecting their first or second child, and children up to age 6 months Fee: FreePublic Health NursingThese services are provided to individuals and families with barriers to healthcare; infants, toddlers and school-age children with chronic and/or complexmedical conditions; high-risk pregnant women; and the frail dependent elderly. The program is designed to include families in the decisionmakingprocess and as partners in their child’s education. Eligibility: Families within federal income guidelines with a preschool-agechild; a selection criterion is available for review prior to completing theapplication; children with certified special needs have priority for enrollment. Fee: FreeHealthy StartHealthy Start connects parents of newborns with helpful services andinformation. The service is free, voluntary and confidential. Healthy Startprovides home visiting services to qualified families. Home visitors partnerwith parents to address individual needs, set and support goals, and build ahome environment around the new baby that supports learning, safety, anddevelopmental stimulation. Eligibility: Families who would like more support in raising their children.Families may enroll during pregnancy or until an infant is 90 days old Home-based services may continue until the child is age 5 Target Population: Pregnant women, or families with newborns less than 3months old, who are identified as having stresses and burdens that make themmore likely than the general public to engage in child abuse and neglect andwho live in specific census tracts Fee: Free43
