Pulmonary Hypertension: Beyond the Swan Ganz Catheter

Diuretics• Reduce edema, hepatic congestion and ascites that occurswith RHF• Hypervolemia distends already dilated RV• Mortality/morbidity data are not known– Spironolactone• Loop diuretics are first line– May need more than one, add different classes– Patients with hepatic failure and ascites respond favourably tospironolactone• Amiloride• May increase dyspnea in patients with RHF
