óóóIn the early stages of ectopic pregnancy, symptoms may be absent or mild,but eventually they will become severe. A combination of these signs orsymptoms should increase suspicion of ectopic pregnancy:− Unusual abdominal pain or tenderness− Abnormal vaginal bleeding or no monthly bleeding—especially if this isa change from her usual bleeding pattern− Light-headedness or dizziness− FaintingIf ectopic pregnancy or other serious health condition is suspected,refer at once for immediate diagnosis and care. (See Female Sterilization,Managing Ectopic Pregnancy, p. 179, for more on ectopic pregnancies.)Abdominal pain may be due to other problems, such as enlarged ovarianfollicles or cysts.− A woman can continue to use implants during evaluation.− There is no need to treat enlarged ovarian follicles or cysts unlessthey grow abnormally large, twist, or burst. Reassure the client thatthey usually disappear on their own. To be sure the problem isresolving, see the client again in 6 weeks, if possible.8ImplantsNew Problems That May Require Switching MethodsMay or may not be due to method.óóóóóUnexplained vaginal bleeding (that suggests a medical condition notrelated to the method)Refer or evaluate by history and pelvic examination. Diagnose and treatas appropriate.If no cause of bleeding can be found, consider stopping implants tomake diagnosis easier. Provide another method of her choice to useuntil the condition is evaluated and treated (not progestin-onlyinjectables, or a copper-bearing or hormonal IUD).If bleeding is caused by sexually transmitted infection or pelvicinflammatory disease, she can continue using implants during treatment.Starting treatment with anticonvulsants or rifampicinBarbiturates, carbamazepine, oxcarbazepine, phenytoin, primidone,topiramate, and rifampicin may make implants less effective. If using thesemedications long-term, she may want a different method, such as monthlyinjectables, progestin-only injectables, or a copper-bearing or hormonalIUD.If using these medications short-term, she can use a backup method alongwith implants.Helping Continuing Users of Implants127
