Sani<strong>to</strong>rium, 3 Leprosy Hospitals, 1 Himachal Hospital ofMental <strong>Health</strong> and Rehabilitation, 1 Kamla Nehru Hospital for Mo<strong>the</strong>r and Child, One ESIHospital, 2 Medical Colleges and 1 one Govt. Dental college in <strong>the</strong> State as on 31.03.2007.In Himachal Pradesh presently One <strong>Health</strong> Sub-Centre is serving 2838 people against<strong>the</strong> National Average of 5401. Similarly, a PHC is providing health care facilities <strong>to</strong> about13,367 people against <strong>the</strong> National Average of 32,469 whereas a CHC is serving 88,911 peopleagainst <strong>the</strong> National Average of 2,43,427.The birth rate of <strong>the</strong> State is 20.0 per 1000 against 23.8 of <strong>the</strong> country; crude death rateis 6.9 against 7.6 at national level and infant mortality rate is 49 against <strong>the</strong> national figure of58. Total fertility rate is 2.1 as compare <strong>to</strong> 2.9 at <strong>the</strong> National level.H.P. HEALTH INDICATORSINDICATOR H.P. In <strong>the</strong> field of Family Planning our achievements are high and encouraging. Coverageof eligible couples with family planning methods in <strong>the</strong> State is around 68%, which is alsoquite high. The State has achieved near elimination of Leprosy with reduction in LeprosyPrevalence Rate <strong>to</strong> less than 0.32 cases per ten thousand population. Tuberculosis has also beenbrought under control with more than 88% cure rate. The achievements under <strong>the</strong> UniversalImmunization Programme have also been commendable. The State is 1 st in <strong>the</strong> country in <strong>the</strong>implementation of Registration of Births and Deaths <strong>Act</strong>, 1969 with 100% Registration ofBirths and Deaths.The <strong>Department</strong> functions under <strong>the</strong> control of <strong>the</strong> <strong>Health</strong> Minister who is assisted by<strong>the</strong> Principal Secretary <strong>Health</strong> <strong>to</strong> <strong>the</strong> Govt. of <strong>HP</strong> with its office in <strong>the</strong> <strong>HP</strong> Secretariat atShimla- 2, The Principal secretary is assisted by <strong>the</strong> Special/Additional/Joint/ Deputy/ UnderSecretaries as branch officers for <strong>the</strong> two branches ( <strong>Health</strong> –A ) and (<strong>Health</strong> –B) .Thesebranches function under <strong>the</strong> Section Officers. <strong>Health</strong> –A section looks after <strong>the</strong> programmesand Gazetted establishment of <strong>the</strong> Primary and Secondary <strong>Health</strong> care while <strong>Health</strong> –B sectionlooks after <strong>the</strong> programmes and establishment of <strong>the</strong> Medical Education and Tertiary healthcare institutions besides <strong>the</strong> budget and works of <strong>the</strong> entire health <strong>Department</strong>.The Direc<strong>to</strong>rate of <strong>Health</strong> and Family Welfare functions under <strong>the</strong> AdministrativeControl of Secretary <strong>Health</strong> GO<strong>HP</strong> and is located at Block No. 6- SDA Complex KasumptiShimla- 9.The Direc<strong>to</strong>rate is headed by The Direc<strong>to</strong>r of <strong>Health</strong> Services. There is also a Mission Direc<strong>to</strong>rspecifically for <strong>the</strong> NRHM. Direc<strong>to</strong>r <strong>Health</strong> Services is assisted by one Additional Direc<strong>to</strong>r , 4Joint Direc<strong>to</strong>rs , 6 Deputy Direc<strong>to</strong>rs /Programme Officers ,one Deputy Direc<strong>to</strong>r CRS, AssistantDirec<strong>to</strong>r Nursing , Communication Officer and o<strong>the</strong>r supervisory staff including oneAdministrative Officer, one Deputy Controller (F& A),One Assistant Controller (F&A ) 6Superintendents and One Assistant drug Controller . The Direc<strong>to</strong>rate is divided in <strong>to</strong> six mainbranches viz.Medical- 1 (Establishment of Medical Officers, Pharmacists, Staff Nurses, LabTechnicians etc.), Medical- 2 ( Establishment of Male & Female <strong>Health</strong> workers, Drivers,Class iv Staff etc.) Medical- 3 ( Procurement, Cash, Audit and Accounts ),Medical- 4(Planning , Budget, expenditure and major civil works), Medical -5 (NRHM and o<strong>the</strong>r NationalProgrammes ),Medical -6 General and Miscellaneous branch .There is a legal cell also <strong>to</strong>3


assist in <strong>the</strong> defence of court cases. A separate State level Aids control society under <strong>the</strong> StateProgramme officer implements <strong>the</strong> AIDs control programme exclusively. There are two<strong>Health</strong> and Family welfare Training institutes under Principals at Parimahal (SHFWTC)Shimla and RHFWTC Cheeb Kangra. There are also 6 GNM Training Schools and MPWtraining schools under <strong>the</strong> <strong>Department</strong>. A Composite Testing Lab. at Kandaghat also functionsunder <strong>the</strong> control of DHS .At <strong>the</strong> District level <strong>the</strong> <strong>Health</strong> <strong>Department</strong> functions through <strong>the</strong> office of ChiefMedical Officers who are assisted by Medical Officer (<strong>Health</strong>) and o<strong>the</strong>r District healthprogramme officers like DTO, DLO, DAPO, DSO etc.At <strong>the</strong> block level <strong>the</strong>re are <strong>the</strong> offices of <strong>the</strong> Block Medical officer ( BMO ‘s) wholook after <strong>the</strong> primary <strong>Health</strong> care establishments with in <strong>the</strong> blocks. The Civil and RegionalHospitals function under <strong>the</strong> control of <strong>the</strong> Senior Medical Officers while <strong>the</strong> Zonal and o<strong>the</strong>rSecondary care Hospitals function under <strong>the</strong> control of Medical Supdts.Functions, Duties and PowersIt is <strong>the</strong> nodal department for <strong>the</strong> implementation of <strong>the</strong> National <strong>Health</strong> Policy 2002and various National health Programmes like NRHM , RCH , RNTCP , AIDS Control , IDSP,National Blindness Control programme , National Vec<strong>to</strong>r Borne Disease control programme,NLEP , ICMR Cancer control project, National Mental <strong>Health</strong> programme and <strong>Health</strong> carecomponent of <strong>the</strong> ESIC .Procurement of Equipments and DrugsThe procurement of medical equipments and drugs has been brought out of <strong>the</strong>jurisdiction of Controller S<strong>to</strong>res, H.P. and <strong>the</strong> <strong>Health</strong> <strong>Department</strong> has been makingprocurements of <strong>the</strong> medical equipments through open tenders on competitive rates. APermanent <strong>Department</strong>al Purchase Committee has been constituted under <strong>the</strong> chairmanship ofSecretary (<strong>Health</strong>) for <strong>the</strong> procurement of medical equipments. Specialists of <strong>the</strong> particularfield are also invited <strong>to</strong> <strong>the</strong> meetings of P.D.P.C. Besides this, <strong>the</strong> procurement of medicalequipments is also made through GOI / DGS&D approved sources and H.P.S.E.D.C. whichalso involves <strong>the</strong> Specialists from <strong>the</strong> department in <strong>the</strong> procurement process.As per <strong>the</strong> Drug Procurement Policy of <strong>the</strong> State <strong>the</strong> budget for <strong>the</strong> drugs is divided in<strong>to</strong>two parts. 70% of <strong>the</strong> budget is utilized for <strong>the</strong> procurement of drugs through H.P. State CivilSupplies Corporation Ltd. which finalizes <strong>the</strong> Rate Contract involving <strong>the</strong> <strong>Health</strong> <strong>Department</strong>through <strong>the</strong> State Level Drug Purchase Committee. The remaining 30% of <strong>the</strong> budget is placedat <strong>the</strong> disposal of CMOs for <strong>the</strong> procurement of Life Saving Drugs and Emergencyrequirements after following <strong>the</strong> prescribed procedure. They also have <strong>the</strong> option <strong>to</strong> placeorders with <strong>the</strong> sources approved in <strong>the</strong> H.P. State Civil Supplies Corporation Ltd. Rate4


Contract. The medicines under <strong>the</strong> NRHM funds have also been procured through <strong>the</strong> H.P.State Civil Supplies Corporation Ltd..The orders for supply of medicines <strong>to</strong> E.S.I. Dispensaries out of <strong>the</strong> funds madeavailable through <strong>the</strong> revolving fund are placed <strong>to</strong> <strong>the</strong> sources approved in <strong>the</strong> E.S.I.C. RateContract.During 2006-07, <strong>the</strong> State Govt. has supplied medicines worth Rs. 11,36,20,000/- <strong>to</strong> <strong>the</strong>health institutions working under <strong>the</strong> control of Direc<strong>to</strong>r <strong>Health</strong> Services. Besides this, medicalkits worth Rs. 1,17,26,000/- have been made available <strong>to</strong> <strong>the</strong> <strong>Health</strong> Sub-Centres. Medicinesworth Rs. 47,55,719/- have been made available <strong>to</strong> <strong>the</strong> ESI Institutions.NRHMHimachal Pradesh has been included among 18 state of <strong>the</strong> country for <strong>the</strong>implementation of National Rural <strong>Health</strong> Mission. NRHM is a landmark for providingaccessible and affordable health care <strong>to</strong> all citizens living in rural areas particularly <strong>to</strong> <strong>the</strong>poorer and weaker sections. It lays stress on reducing maternal and infant mortality, universalaccess <strong>to</strong> public health services, prevention and control of communicable and noncommunicabledisease, ensuring population stabilization, maintaining gender balance,revitalization of local health traditions and promotion of healthy life styles. Under <strong>the</strong> overallumbrella of NRHM a number of programmes especially <strong>the</strong> Reproductive and Child <strong>Health</strong>Programme (RCH-II), Immunization Programme, Janani Surksha Yojna (JSY) and DiseaseControl Programme have been included. The Mission focuses on decentralized implementationof <strong>the</strong> activities and funneling of funds, it sets <strong>the</strong> stage for District Management of <strong>Health</strong> andactive community participation in <strong>the</strong> implementation of health programmes. The HimachalPradesh government has already constituted <strong>the</strong> State and District <strong>Health</strong> Mission and grassroot activities have been started with zeal and zest. All <strong>the</strong> CHCs would be converted in<strong>to</strong> FirstReferral Units under <strong>the</strong> programme in a phased manner. Similarly, 50% of <strong>the</strong> PHCs in <strong>the</strong>State would be providing 24 Hours Services by <strong>the</strong> end of <strong>the</strong> programme. The programmealso focuses on convergance with IPH, Rural Development and Panchayati Raj, Ayurveda andSocial Justice and Empowerment <strong>Department</strong>s.In <strong>the</strong> year <strong>2005</strong>-06, a sum of Rs. 53.36 crores was received from Govt. of India underNRHM and Rs. 34.54 crores has been spent out of it. In 2006-07, Rs. 76.61 crores has beenreleased by Govt. of India till 31.12.2006 out of which Rs. 39.95 crores has been spent as on31.12.2006.The Direc<strong>to</strong>rate of <strong>Health</strong> mainly performs <strong>the</strong> following functions viz.Developmental and welfare functions5
