CPD Continuing Professional Development 

Dental technicians are part of the team 

There is a regrettable tendency in some quarters to forget dental technicians when 

considering the dental team. They are part of it whether they work on the practice premises 

(once common in the past) or in a UK laboratory or even overseas. It is also four years since it 

became obligatory for all dental professionals working in the UK to register with the General 

Dental Council (GDC). This article looks at the obligations that members of the team have 

towards each other and what is required of them to remain as registrants. 

Although most dental technicians (DT) and clinical 

dental technicians (CDT) do not work in the same 

place as other team members, this does not mean 

they should be excluded from the team, according 

to General Dental Council (GDC) guidance1 . Dental 

teams can take many different forms, depending on 

the needs of the patient. They are not just limited to 

those working together in the same practice. 

A dentist may have his or her own practice, but still 

works as part of a wider dental team. An example 

would be when the dentist refers a patient to 

another dental professional, whether a specialist, a 

hygienist, a therapist or a technician working in a 

laboratory. As a dental care professional you have an 

obligation to work with other members of the team. 

Communication 

Any failure of healthcare professionals to 

communicate effectively has consequences ranging 

from the inconvenient to the tragic, as in the 

case where a drug prescription is misread or an 

operation carried out on the healthy side. 

In dentistry Communication and the dental team 

is the title of a useful book2 by Kay and Tinsley, 

which describes itself as an ‘essential guide’. A 

reviewer wrote about it that the importance of 

communication ‘is a topic which is often forgotten 

and this book reminds readers that its signifi cance 

to a successful dental practice should not be 

underestimated’. 

Last year the British Dental Journal published 

an audit of dental prescriptions between clinics 

and dental laboratories3 . It found that there was 

a high level of non-compliant prescriptions being 

submitted by undergraduates and qualifi ed dentists

in a University Hospital teaching environment. The 

author concluded that the level of communication 

between dentists and dental technicians, even 

within a close working environment, can at times 

be inadequate. 

Prescriptions for laboratory work 

The audit found that one common piece of missed 

information was the clinician’s signature. This 

information is required for compliance with 

legislation proposed by the Medicines and 

Healthcare products Regulatory Agency (MHRA). 

It also found that the type of material required to 

fabricate the prosthesis was poorly specifi ed. This 

could be due to inadequate technical knowledge, 

not being aware of what materials were available 

and the assumption that the laboratory will use 

standard materials. Legally, however, the material 

has to be specifi ed on the prescription. 

In the audit, just over half of the written instructions 

were considered clear and provided suffi cient 

information to construct the appliance. The 

authors comment that it is important that dentists 

recognise their ethical and legal responsibilities. It 

is their responsibility to design the fi nal prosthesis 

without seeking assistance from the technician. A 

dental technician would not have access to clinical 

information regarding the patient. The responsibility 

of the technician is to fabricate the prosthesis 

specifi cally prescribed by the prescription. 

The authors continue: ‘If these responsibilities are 

not adhered to the outcome may be an unacceptable 

prosthesis. Clear and effective communication 

between dentist and technician is essential for 

successful fabrication of the fi nal prosthesis.

Continuing Professional Development CPD 

Inappropriate use of the prescription will leave much 

of the decision making to the technician.’ 

The Medical Devices Directive 

It is over two years since changes were made 

to the Medical Devices Directive 93/42/EC 

affecting the provision and manufacture of dental 

appliances. The GDC has published ‘Standards 

on Commissioning and Manufacturing Dental 

Appliances’ which is available on its website4 . 

One of the key issues arising from the amendment 

was the Statement of Manufacture. Patients must 

be made aware that they can request a statement 

of manufacture and registrants have a responsibility 

to ensure that it is made available if it is requested. 

Not doing so is punishable as a criminal offence. 

Any registrant providing a patient with a dental 

appliance is responsible for ensuring that they 

are offered a copy of the statement and provided 

with such a copy if it is requested. A record should 

be kept of whether or not the patient chooses 

to accept a copy of the statement. If the patient 

doesn’t accept a copy, the registrant will need to 

keep the statement for the lifetime of the appliance. 

If a dentist or other dental care professional 

decides to sub-contract the manufacture of a 

dental appliance outside of the UK, they will be 

held professionally accountable for the safety and 

quality of the appliance. This will include ensuring 

that the statement of manufacture - or if outside of 

the EU, the name and address of the manufacturer 

- are disclosed to the registrant who prescribed the 

appliance as well as to the patient. 

Scope of practice 

The General Dental Council registers all members 

of the dental team and places obligations on its 

registrants, such as paying an Annual Registration 

Fee (ARF) and carrying out both verifi able and 

non-verifi able Continuing Professional Development 

(CPD). It also publishes the scope of practice for 

each class of registrant. 

This Scope of Practice document5 is being revised 

in 2012 and once published will be essential 

reading for all members of the dental team. Despite 

the review the current document still stands and 

should be used until a revised version is produced. 

It defi nes the roles of technicians and clinical dental 

technicians within the dental team. 

Dental technicians 

Dental technicians are registered dental 

professionals who make dental devices including 

dentures, crowns and bridges to a prescription from 

a dentist or clinical dental technician. They also 

repair dentures direct for members of the public. 

The Scope of Practice document outlines the 

working relationship between dentist and 

technician. In the course of their work dental 

technicians: 

review cases coming into the laboratory to decide 

how they should be progressed 

work with the dentist or clinical dental technician 

on treatment planning and outline design 

design, plan and make a range of custom-made 

dental devices according to a prescription 

repair and modify dental devices 

carry out shade taking 

carry out infection control procedures to 

prevent physical, chemical and microbiological 

contamination in the laboratory 

keep full and accurate laboratory records 

verify and take responsibility for the quality and 

safety of devices leaving a laboratory 

make appropriate referrals to other healthcare 

professionals. 

The Scope of Practice document also lists additional 

skills which dental technicians could develop during 

their careers. These could include, for instance, 

working with a dentist in the clinic and assisting 

with treatment by taking impressions, recording 

facebows, and recording occlusal registrations. It 

also lays down what dental technicians do not do. 

This includes working independently in the clinic, 

performing clinical procedures related to providing 

removable dental appliances and carrying out 

independent clinical examinations.

Clinical dental technicians 

Clinical dental technicians (CDTs) are registered 

dental professionals who provide complete 

dentures direct to patients and other dental devices 

on prescription from a dentist. They are also 

qualifi ed dental technicians. Patients with natural 

teeth or implants must see a dentist before the 

CDT can begin treatment. CDTs refer patients to a 

dentist if they need a treatment plan or if the CDT 

is concerned about the patient’s oral health. 

CDTs also: 

take detailed dental history and relevant medical 

history 

perform technical and clinical procedures related to 

providing removable dental appliances 

carry out clinical examinations 

take and process radiographs and other images 

related to providing removable dental appliances 

distinguish between normal and abnormal 

consequences of ageing 

recognise abnormal oral mucosa and related 

underlying structures 

refer patients to other healthcare professionals if 

necessary 

fi t removable appliances 

give appropriate advice to patients. 

Additional skills which CDTs could develop during 

their career include oral health education, providing 

sports mouth guards and providing anti-snoring 

devices on prescription of a dentist.

Continuing Professional 

Development (CPD) 

The GDC has launched what it describes as a ‘big 

conversation’ on CPD in dentistry. This is part of 

an on-going CPD review by the GDC. While this 

is going on, the requirements for Dental Care 

Professionals to carry out CPD remain. These are 

laid out in a recent document6 . 

By law, if you are a dental care professional, you 

must do and keep records of 150 hours of CPD 

over every fi ve-year cycle. At least 50 of the 150 

hours must be verifi able CPD; the remaining hours 

can be non-verifi able. You can split your CPD 

between each of the fi ve years in a way which 

works for you, but the GDC advises that you should 

spread it out across the fi ve-year cycle as evenly 

as possible. 

There are areas of CPD that dental care 

professionals should do as part of the overall 50 

hour minimum verifi able CPD requirement. For 

dental technicians these are: 

medical emergencies (at least 10 hours in every 

CPD cycle) 

disinfection and decontamination (at least fi ve 

hours) 

materials and equipment (at least fi ve hours).

Delivering better oral health: for all the dental team 

Prevention will play a signifi cant part in the new NHS contract and it is also central to those 

working in private practice. The Department of Health and the British Association for the Study 

of Community Dentistry published evidence-based guidance fi rst in September 2007. There 

was a second edition in April 2009 1 and a third one is due in 2012. 

This is a topic for the whole dental team. As the 

Chief Dental Offi cer for England, Barry Cockcroft, 

wrote in the introduction to the second edition: ‘we 

hope that dentists will share it with their teams in 

order to improve delivery of prevention within the 

dental practice.’ A recent review of dental skill mix2 found that oral health education and topical fl uoride 

application were being carried out by Dental Care 

Professionals (DCPs) including dental nurses. 

In his foreword to the Government’s proposals3 for 

pilots of a new NHS dental contract, Health Minister, 

Lord Howe said: ‘Until now, the NHS dentistry 

contract has remained focused on treatment; 

there has been little or no incentive for dentists to 

practise the sort of preventative dentistry that most 

people today want and need. It is time for this to 

change.’ 

The Minister added that the new contract 

would ‘focus on achieving good health … 

with an additional focus on the oral health of 

schoolchildren.’ He commented that people wanted 

a dental service that helped them to prevent oral 

health problems and maintain good oral health. 

‘Treatment should increasingly be to help people to 

achieve and then maintain improved oral health’, 

he said. 

Delivering Better Oral Health 

Adherence to the guidance, Delivering Better Oral 

Health: An evidence- based toolkit for prevention, 

is becoming a contractual obligation for those 

practices working in the NHS. Recent thinking 

suggests that all patients should be given the benefi t 

of advice regarding their general and dental health, 

not just those thought to be ‘at risk’. The guidance 

lists the advice and actions that should be provided 

for all patients to maintain good oral health.

The guidance covers the principles of tooth 

brushing for oral health and increasing fl uoride 

availability. Improving periodontal health and the 

prevention of erosion have sections. Finally healthy 

eating advice and sugar-free medicines, stop 

smoking guidance and alcohol misuse are covered. 

Principles of tooth brushing for oral health 

The major dental conditions of caries and 

periodontal disease can both be reduced by regular 

tooth brushing with fl uoride toothpaste. The fl uoride 

in toothpaste serves to prevent, control and arrest 

caries. The physical removal of plaque reduces 

the infl ammatory response of the gingivae and its 

sequelae. Some toothpastes contain ingredients 

which also reduce the initiation and progression of 

periodontal breakdown. 

There is evidence to suggest that the preventive 

action of tooth brushing can be maximised if the 

following principles are followed: 

Brushing should occur twice daily – clean teeth 

last thing at night before bed and at least one other 

time each day. 

Rinsing with lots of water after brushing should 

be discouraged – spitting out excess toothpaste is 

preferable. 

The patient’s existing method of brushing may 

need to be modifi ed, emphasising the need 

to systematically clean all tooth surfaces. No 

particular technique has been shown to be better 

than another. 

Disclosing tablets can help to indicate areas that 

are being missed. 

Brushing is more effective with a small-headed 

toothbrush with soft, round-ended fi laments, a 

compact, angled arrangement of long and short

fi laments and a handle which is comfortable. 


Powered brushes with an oscillating/ rotating action 

remove plaque more effectively and studies show 

they reduce gingivitis when used for over 3 months. 

No other powered designs were as consistently 

superior to manual brushes. 

For children: 

Brushing should start as soon as the fi rst deciduous 

tooth erupts. 

Children under 3 years should use a toothpaste 

containing no less than 1,000 ppm fl uoride. They 

should use no more than a smear of toothpaste (a 

thin fi lm of paste covering less than three-quarters 

of the brush) and must not be permitted to eat or 

lick toothpaste from the tube. 

Children between 3 and 6 years should use no 

more than a pea-sized amount of toothpaste. 

Children need to be helped or supervised by an 

adult when brushing until at least 7 years of age 

and must not be permitted to eat or lick toothpaste 

from the tube. 

Family fl uoride toothpaste (1,350–1,500 ppm 

fl uoride) is indicated for maximum caries control for 

all children except those who cannot be prevented 

from eating toothpaste. Advice must be given about 

adult supervision and the small amounts to be used. 

Increasing fl uoride availability 

The Guidance gives excellent advice for 

all the dental team on ways of increasing 

fl uoride availability. There are tables of fl uoride 

concentration levels of commonly bought 

toothpastes and also of fl uoride varnishes. Advice is 

also given on how to prescribe high concentration 

fl uoride toothpaste and an advised prescribing 

regime for fl uoride tablets. This section however 

begins with looking at water fl uoridation. 

Currently approximately 10% of England’s 

population, or about 6 million people, benefi t from 

a water supply where the fl uoride content either 

naturally or artifi cially is at the optimum level for 

dental health. In terms of population coverage, the 

West Midlands is the most extensively fl uoridated 

area, followed by parts of the North East of England. 

In 2003 the law was changed enabling strategic 

health authorities to require water companies to 

fl uoridate water supplies providing there is support 

from the local population following consultation. 

The risk of dental fl uorosis is higher from the use 

of topical fl uorides in fl uoridated areas. This is why 

in the earlier section, particular emphasis is put on 

the need to help and supervise tooth brushing for 

children up to at least age 7 and use a very small 

volume of toothpaste for children under 3 years. 

Many people in non-fl uoridated areas incorrectly 

think that their water supply is fl uoridated and it is 

important that they are aware of the true fl uoride 

status of their supplies. If in doubt, information can 

be obtained from their water supplier by quoting 

the residential postcode. 

There follows information on how fl uoride 

availability can be increased to improve oral health, 

starting with a table of types of over-the-counter 

toothpastes by fl uoride concentration level. The list 

was last updated in March 2009. 

Fluoride varnish is one of the best options for 

the application of topical fl uoride to teeth in the 

absence of water fl uoridation. High quality evidence 

of the caries-preventive effectiveness of fl uoride 

varnish in both permanent and primary dentitions is 

available. A number of systematic reviews conclude 

that twice-yearly applications produce a mean 

caries increment reduction of 33% in the primary 

dentition and 46% in the permanent. 

The evidence also supports the view that varnish 

application can arrest existing lesions on the smooth 

surfaces of primary teeth and roots of permanent 

teeth. Much of the evidence of effectiveness is 

derived from studies which have used sodium 

fl uoride 22,600 ppm varnish for application. 

Fluoride rinses can be prescribed for patients 

aged 8 years and above, for daily or weekly use, 

in addition to twice daily brushing with toothpaste 

containing at least 1,350 ppm fl uoride. Rinses 

require patient compliance and should be used 

at a different time to tooth brushing to maximise 

the topical effect, which relates to frequency of 

availability.

Improving periodontal health 

Mechanical plaque control, principally correct tooth 

brushing, remains the main method for improving 

periodontal health. In addition evidence suggests 

that toothpastes containing triclosan in combination 

with a copolymer or with zinc citrate are more 

effective than a fl uoride toothpaste in improving 

plaque control and gingival health. 

Chlorhexidine mouthrinses, either 10 ml of 0.2% 

or 15 ml of 0.12%, are very effective in improving 

plaque control and gingival health when used as an 

adjunct to tooth brushing. They are useful for short 

periods when an individual is unable to clean due 

to acute problems or incapacity. 

Conditions that predispose to periodontal disease 

include diabetes, some genetic disorders, Down’s 

syndrome, blood dyscrasias and haematological 

disorders. Other factors are pregnancy, long-term 

smoking, and certain medications, for example 

Epanutin.

Prevention of erosion 

Erosion is an increasing problem especially in the 

young. A number of epidemiological studies show 

that soft drinks are associated with erosion and the 

World Health Organization recommends that the 

amount and frequency of intake of soft drinks and 

juices should be limited. 

The guidance advises dental professionals to give 

tailored, specifi c advice for each individual patient. 

This may include using toothpaste containing 1,450 

ppm fl uoride twice daily, avoiding frequent intake of 

acidic foods or drinks and not brushing after eating 

or drinking acidic food or drinks. There are also 

useful facts about food and drink associated with 

erosion. 

The role of the dental team 

The Department’s Skill Mix review group referred 

to earlier quotes from the Coalition’s public health 

strategy, Healthy Lives, Healthy People4 . This 

articulates the need for a clear focus on prevention 

rather than solely on treatment, as a means of 

delivering the highest standard of oral and general 

health for the nation. 

The key to the success of this strategy will be the 

engagement of professionals across the service, 

and Skill Mix review group believes that the 

signifi cant range of preventive procedures that 

are within the scope of DCPs’ practice, makes 

this group fundamental to the achievement of the 

Government’s public health goals.

Ethical marketing for the dental team 

Marketing and advertising are relatively new concepts in dental practice. Earlier this year the 

General Dental Council (GDC) issued guidance on ethical advertising 1 , to which all registrants 

must now adhere. It covers advertising your services, content of websites, specialist titles 

and honorary degrees. 

Development of advertising in dentistry 

Until 25 years ago advertising was almost an 

absolute taboo for dentists. The GDC would take 

action against any dentist who did much more 

than display a professional plate outside the 

practice and even that had to be of an approved 

size and content. Those who spoke to the media 

had to ensure that what they said could not be 

construed as promoting their practice and some 

were identifi ed only as ‘a dentist from Leeds’ for 

example. There were also rules about what you 

could or could not call your practice. For instance 

you could not call it after the town where you 

practised, even if you were the only dentist 

working there. 

In 1987 the GDC changed the rules and allowed 

dentists to advertise under strict conditions. 

Advertisements had to be tasteful, not disparage 

other practices and not claim that any special 

talents or treatments were available. This was in 

the days before the Internet and practice websites 

were a distant dream. Since then new ways of 

marketing have evolved and earlier this year the 

GDC issued new guidance on ethical advertising 

which became mandatory on March 1, 2012. 

GDC guidance on ethical advertising 

The key messages in the guidance in relation to 

advertising are: 

All information or publicity material regarding 

dental services should be legal, decent, honest and 

truthful. 

Advertising by dental professionals can be a source 

of information to help patients make informed 

choices about their dental care.

Advertising that is false, misleading or has the 

potential to mislead patients is unprofessional 

and may lead to referral to fi tness to practise 

proceedings and can be a criminal offence. 

Patients may be confused and uncertain about 

dental treatment so you should take special care 

when explaining your services to them. This 

includes providing balanced, factual information 

enabling them to make an informed choice about 

their treatment. 

Do not exploit the trust, vulnerability or relative lack 

of knowledge of your patients. 

Misleading claims can make it more diffi cult for 

patients to choose a dental professional or dental 

services. 

The guidance warns that misleading statements 

and claims can lead to expectations which cannot 

be fulfi lled and, in more serious cases, can put 

patients at risk of harm from an inappropriate 

choice. The GDC reminds dental professionals 

that patients can check with them that they are 

registered and whether they are on a specialist 

list. But the guidance points out patients are more 

likely to rely on information that dentists provide 

such as practice leafl ets or certifi cates on the 

practice wall. 

The onus is on the dental professional to be honest 

in the presentation of their skills and qualifi cations. 

If they make misleading claims, they may have to 

justify their decisions to the GDC through its fi tness 

to practise procedures. 

On advertising your services, the guidance 

states that whenever you, your practice, or any 

place where you work as a registrant, produce 

any information containing your name, you are 

responsible for checking that it is correct.

You must: 

ensure information is current and accurate; 

make sure that your GDC registration number is 

included; 

use clear language that patients are likely to 

understand; 

back up claims with facts; 

avoid ambiguous statements; and 

avoid statements or claims intended or likely to 

create an unjustifi ed expectation about the results 

you can achieve. 

Advertisements and other practice publicity must 

make clear whether the practice is NHS, mixed or 

wholly private. Dental professionals should only 

recommend products if they are the best way to 

meet a patient’s needs. 

The guidance warns you, as a dental professional, 

that if you wish to offer services which your 

training does not qualify you to provide, you must 

make sure you undertake appropriate additional 

training to attain the necessary competence. It 

clearly states: ‘Do not mislead patients into 

believing that you are trained and competent to 

provide other services purely by virtue of your 

primary qualifi cation as a healthcare professional, 

but make clear that you have undertaken extra 

training to achieve competence.’ 


Websites 

Registrants must abide by the Code of Ethics for 

Dentists in the EU for Electronic Commerce2 . These 

rules extend to websites and much of this part of 

the GDC guidance comes from the Code. 

For all dental professionals providing dental care, 

who are mentioned on the site the following 

information must be displayed: 

their professional qualifi cation and the country 

from which that qualifi cation is derived; and 

their GDC registration number. 

Dental practice websites must display the following 

information: 

the name and geographic address at which the 

dental service is established; 

contact details of the dental service, including 

e-mail address and telephone number; 

the GDC’s address and other contact details, or a 

link to the GDC website; 

details of the practice’s complaints procedure and 

information of who patients may contact if they 

are not satisfi ed with the response. These are 

the relevant NHS body for NHS treatment and the 

Dental Complaints Service for private treatment; 

the date the website was last updated must also be 

included. 

The GDC guidance reminds you to update the 

information showing on your website regularly 

so that it accurately refl ects the personnel at 

the practice and the services offered. A dental 

practice website must not display information 

comparing the skills or qualifi cations of any dental 

professional providing any service with the skills 

and qualifi cations of other dental professionals. 

Specialist and other titles 

There has been a long-standing dispute both 

within and outside the profession about whether 

dentists could adopt the courtesy title of ‘Doctor’. 

In 1995 the GDC gave a ruling3 that it would not 

take action against a dentist who did this, provided 

that the words dentist or dental practitioner 

were added. However the Advertising Standards 

Authority has since made an adjudication4 that 

“the title ‘Dr’ before a practitioners name should 

not be used in ads unless the practitioner held 

a general medical qualifi cation, a relevant PhD 

or doctorate (of suffi cient length and intensity) or 

unless the similarities and differences between 

the practitioner’s qualifi cations and medical 

qualifi cations were explained in detail in the ad.” 

The matter is being reconsidered by the GDC and 

no mention was made of this in their guidance. 

The GDC’s guidance does say that only dentists 

who are on a GDC specialist list may use the 

title ‘Specialist’ or describe themselves as a 

‘specialist in….’ Dentists who are not on a GDC 

specialist list should not use titles which may imply

specialist status such as Orthodontist, Periodontist, 

Endodontist etc. 

There are no specialist lists for dental care 

professionals. Dental care professionals should 

ensure that they do not mislead patients by using 

titles which could imply specialist status, such as 

‘Smile specialist’ or ‘Denture specialist’. 

Registrants who are not on a specialist list should 

not describe themselves as ‘specialising in…’ a 

particular form of treatment but may use the terms 

‘special interest in..’, ‘experienced in..’ or ‘practice 

limited to..’. 

On the point of honorary degrees and memberships, 

the guidance states that patients may reasonably 

believe that if you put a qualifi cation after your 

name, it has been ‘earned’, that is, it represents 

a particular level of academic achievement. This 

may not be the case where a degree is honorary. 

Listing memberships or fellowships of professional 

associations or societies can also mislead. The 

letters may imply to the public that a registrant has 

attained a certain level of skill which in fact may 

not be the case. 

Advertising Standards Authority 

In addition dentists must abide by rules laid down 

by the Advertising Standards Authority5 . This body 

can make adjudications on advertisements which 

are binding, one of which has been mentioned. 

Although not mandatory, guidance on advertising 

generally is also available from the Chartered 

Institute of Marketing6 . 

Social media marketing 

With the growing use of websites and other social 

media marketing tools, new concepts are coming 

to dentistry, mainly from the USA. Traditionally 

practices confi ned themselves to an entry in Yellow 

Pages and the like. Recently the BDA has warned 

however against companies, often based overseas, 

who offer an entry in ‘a prestigious international 

directory’ for a large fee. Many of these cold calls 

are a scam.

Some practices are looking at advertising in other 

media such as local radio and some consider the 

benefi ts of direct marketing, mail shots and the 

like. Some use social media such as Twitter and 

Facebook. The website is undoubtedly a signifi cant 

way to put across your practice’s message, give 

news or make offers. However the GDC recently 

warned about one aspect of such marketing 

‘offered by companies like Groupon and Living 

Social.’ 

It said that although such deals were growing in 

popularity, with many sites now offering deals on 

dental procedures, including tooth whitening and 

veneers, it could take action to restrict or stop a 

dental professional practising if its standards were 

not met. The GDC says: ‘If you are considering 

offering a treatment deal, you must assess the 

patient, obtain valid consent, obtain a medical 

history and explain all the options before carrying 

out any work. You must put patients’ interests 

before your own or those of any colleague, 

organisation or business.’ 

Final food for thought 

It is worth, however, looking at the basic rules of 

advertising and marketing. Recent research by the 

British Dental Trade Association7 found that trust 

in the dentist by the patient is the overwhelming 

factor in deciding to go to and remain with a 

practice. Being local and available are key aspects 

in their decision. Despite all your efforts a good 

recommendation from friends or family will count 

for more than the fl ashiest of websites.

Developing NHS Dentistry 

The new NHS contract pilots were announced in December 2010 1 and volunteer practices 

were sought to take part in them. The Department of Health announced last year that 68 

pilots were working under the scheme from September 2011. They introduce a number of 

new concepts and new acronyms to go with them. The objective of this article is to introduce 

these to members of the dental team who may not be familiar with them. 

In the Coalition Agreement2 , the Government 

made a specifi c commitment to introduce a 

new NHS dentistry contract that would ‘focus on 

achieving good health and increasing access to 

NHS dentistry, with an additional focus on the oral 

health of schoolchildren’. Launching the pilots, the 

Government said it needed ‘to change to a system 

whereby dentists are fairly rewarded through 

weighted capitation funding for the patients they 

take on, and motivated to provide the best clinical 

care through incentives to improve quality and 

clinical outcomes’. 

Many of the concepts in the proposed new contract 

fl ow from the Steele Review. 3 This introduced the 

concept of a ‘pathway through care’, which would 

include an oral health assessment. Alongside this 

the new contracts will include a Dental Quality 

and Outcomes Framework (DQOF). 4 The document 

outlining these proposals explains that the DQOF 

will measure the quality of dentists’ work, and the 

clinical outcomes they achieve. 

The changes coming from the pilots are 

likely to lead to a greater role for dental care 

professionals (DCPs) according to a recent report5 . 

It recommended: ‘As part of the new contract pilots, 

the Department of Health should learn how DCPs 

can be used more effectively to deliver the coalition 

government’s objectives for primary care NHS 

dentistry.’ 

The Steele Report 

It is three years since the Department of Health 

published Professor Steele’s independent review 

of NHS dental services in England. It is essential 

reading if you are to understand the new way of 

working, which will form the basis of new NHS

contractual arrangements to be introduced in 2014, 

unless delayed. 

Two key aspects of the report are shown 

diagrammatically as Figure 3: Priorities for public 

investment in oral health (page 43) and Figure 4: 

Proposed patient pathway (page 45). Professor 

Steele proposed that people choosing to use NHS 

dental services should receive ‘a lifetime-focused 

oral health service, based on evidence where 

possible, which will: 

help them to prevent oral disease and the damage 

it causes 

provide effective and prompt urgent care when 

required 

minimise the impact of dental diseases when they 

occur by providing proper assessments, treatment 

to manage disease and the opportunity for regular 

maintenance and review for those who want it 

provide treatments to maintain and restore 

quality of life subject to a stable oral environment 

being achieved, and subject to pre-set criteria 

for appropriate NHS care, built around long-term 

health gain.’ 

To achieve this, the report recommended that 

NHS primary care dentistry ‘should be staged 

around a pathway through care’. The report also 

recommended that the pathway should be based 

around: 

making urgent care available 

assessing risk and preventing disease 

routine management of disease 

monitoring during continuing care 

provision of advanced services intended to restore 

and maintain quality of life.


The essential features of the proposed pathway 

are: 

The opportunity for urgent care for everyone when 

required. 

For those without a regular dentist, the opportunity 

to enter continuing care. 

For new patients there should be a formal oral 

health assessment to evaluate the risks of all 

major dental disease (decay, gum disease and oral 

cancer) and the need for treatment. Personalised 

prevention should be started. 

For existing patients who are in a continuing 

care arrangement (who are returning within an 

appropriate period, depending on recall interval), 

there is an assumption that the dentist is aware of 

and managing their preventive needs. 

Advanced aspects of restorative care are provided 

only when there is a stable oral environment, where 

disease risks are managed and when the patient is 

established in a continuing care relationship. 

Entry to advanced care is dependent on 

professionally derived clinical guidance, based on 

evidence or – where there is insuffi cient evidence 

– best practice, with clear evaluation and entry 

criteria. 

A key element of this is that quality measures 

should fi t the pathway. Progression through the 

pathway and a visible reduction in risk will be key 

performance indicators for NHS dental providers. 

Oral Health Assessment 

The care pathway approach, as put forward by 

Steele, forms the basis of the new way of working 

being piloted. It is intended to facilitate the move 

from a restorative to a preventive and long term 

approach to patient care. It is risk based and meets 

the specifi c needs of individual patients. It also 

aims to encourage the involvement of patients in 

managing their own oral health. 

The patient’s journey begins with an oral health 

assessment at the point of entry into NHS primary 

care dentistry. Its overall purpose is to capture the 

oral health status and individual risk factors of the 

patient. These are used to formulate a care plan 

incorporating advice, actions and treatment that is 

appropriate to their need. 

Through assessing levels of existing disease and 

identifying modifying factors which contribute to 

current or future problems, dentists can identify 

the patient’s future risk from disease. This helps 

identify the patient’s preventive and treatment 

needs. Risk screening should be performed at the 

initial patient assessment stage as well as at each 

oral health review. 

Much of the information collected in the oral health 

assessment is standard, medical history, charting 

of restorations, caries and periodontal condition as 

well as soft tissue examination. Some of it such as 

lifestyle and behaviour history, patient perceptions 

and expectations, and their dental attendance 

pattern may not be routinely collected but are part 

of assessing the risk of future disease as well as 

plotting a care pathway. Software is being tested 

which will record all the necessary data. 

Although much of the oral health assessment is 

already established practice, risk screening will 

be a new concept to many. The fi ne detail of it 

is being piloted, but the basic principles behind 

it are established. Central to it is the red/amber/ 

green (RAG) status of patients, representing high 

(red), medium (amber) and low (green) risk of 

future disease. The risk is assessed in four areas 

(domains): caries, periodontal health/disease, 

non-carious tooth surface loss (erosion, attrition & 

abrasion) and soft tissue health/disease. 

The aim is that patients in red or amber categories 

should move to green. This may be achieved 

by treatment from the dentist or another one of 

the dental team. But it will also require action to 

be taken by the patient eg a better oral hygiene 

routine. Communicating their RAG status to them 

helps patients understand their oral health status 

and encourages them to take ownership and 

responsibility for their own oral health. 

The screening process also provides an informed 

basis for discussions about future care and 

helps the practitioner describe what the possible 

outcomes might be. It is one of the principles of 

the Steele review that complex work should only

be offered ‘when there is a stable oral environment 

and where disease risks are managed.’ The onus of 

achieving is not just the dentists’ but requires the 

involvement of patients. 

Dental Quality and Outcomes Framework 

The Dental Quality and Outcomes Framework 

(DQOF) is intended to measure the quality of 

dentists’ work, and the clinical outcomes they 

achieve. The Department of Health believes this is a 

better way of holding them to account, than simply 

measuring the number of units of dental activity 

they carry out. The aim of DQOF is to measure the 

maintenance or improvement of oral health with 

respect to caries and periodontal health. 

The risk screening process (outlined above) 

incorporates both clinical and patient modifying 

factors. For the purposes of the outcome measures, 

only the clinical factors are measured and 

evaluated. Clinical factors are objective and can 

be measured, recorded and improved by good 

care. Patient modifying factors can be subjective 

and some cannot be improved or changed by the 

dental team, such as sibling experience for caries 

risk assessment in children or diabetic control for 

periodontal risk screening. 

It is intended that ten per cent of the practice’s 

contract value will depend on satisfactory 

achievement of the DQOF. Most of this payment 

relates to clinical outcomes, a smaller amount to 

patient experience and a tenth to ‘safety’, namely 

taking a medical history for each patient. 

Most of the clinical effectiveness outcomes consist 

of the reduction or stability in the number of 

carious teeth found at three different ages. The 

rest consists of improvement or maintenance of 

Basic Periodontal Examination (BPE) scores for 

periodontal health. The other outcomes measured 

concern ‘patient experience indicators’, in effect 

asking patient what they think of the care they 

have received. They will be asked questions such 

as whether they are satisfi ed with the cleanliness 

of the practice, whether the staff were helpful and 

would they recommend it to a friend.

Skill mix and the dental team 

Last year’s review of dental skill mix by the Dental 

Programme Board of Medical Education England, 

already referred to, recommended that: ‘As part 

of the new contract pilots, the Department of 

Health should learn how DCPs can be used more 

effectively to deliver the coalition government’s 

objectives for primary care NHS dentistry.’ 

The new contract and the current pilots present 

‘a signifi cant opportunity to implement guidance 

on how DCPs can best be employed in general 

practice to the benefi t of both patients and the 

business.’ For example the review identifi ed greater 

use of suitably trained dental nurses in topical 

fl uoride application and oral health education. 

Conclusion 

Over the coming years different ways of working, 

including the whole dental team, will be introduced. 

Although these are currently restricted to those 

practices working in the pilots, they will soon be 

extended throughout the whole range of NHS dental 

services.
