To help determine the needs of the individual students when planning for instruction,classroom teachers are required to maintain a portfolio on each student. The portfoliowill contain, but not be limited to the following:• Work samples, including items such as photos or tapes of child createdproducts (e.g., block creations, sculptures, dramatizations, childinterviews), actual (or copies of) developmental writing samples, or otheritems to document child development;• Anecdotal records, including informal notes on the child's problem solving<strong>and</strong> critical thinking behaviors related to classroom, social, <strong>and</strong> academicinteractions;• Checklist <strong>and</strong> inventories for recording observations of child behaviors <strong>and</strong>skills such as the Creative Curriculum Developmental Checklist,High/Scope Child Observation Record (COR), Work Sampling Systems(WSS);• Parent conferences, including information on the child provided by theparent <strong>and</strong> conference notes;• Health screening reports, <strong>and</strong>• Referral records for support services.Portfolios must be kept up-to-date in the classroom <strong>and</strong> available for review by thechild’s parent/guardian <strong>and</strong> appropriate LEA <strong>and</strong> State Department personnel.SECTION 5: BEFORE- AND AFTER-SCHOOLENRICHMENT ACTIVITIESThe LEA must provide written notice of the availability of before- <strong>and</strong> after-schoolenrichment activities to all parents of children enrolled in prekindergartenprograms in the system or partial system. The precise start <strong>and</strong> end times of the fullday program shall be determined by the LEA <strong>and</strong> may vary at different school sites;however, those times shall be based on a needs assessment of the community served.The needs assessment must be kept on file at the school site.• The LEA must make before- <strong>and</strong> after-school enrichment activities servicesavailable for all children enrolled in the school system’s or partial system’sprekindergarten classes. These services shall be provided at no cost toparents/guardians of children attending prekindergarten through <strong>LA</strong> 4Prekindergarten Program funds <strong>and</strong> may be provided for other children (living inthe school district <strong>and</strong> enrolled in the <strong>LA</strong> 4 Prekindergarten Program) at no costor may be provided for a fee that is not more than the average per pupil costexcluding the cost of providing space <strong>and</strong> utilities.16<strong>LA</strong> 4 <strong>Guidelines</strong> <strong>and</strong> <strong>Implementation</strong> H<strong>and</strong>book
