<strong>Process</strong> <strong>Flow</strong> – <strong>Rx</strong> <strong>Order</strong>U.S. <strong>Safety</strong> / <strong>VWR</strong> <strong>International</strong><strong>Panel</strong> <strong>Optician</strong>:1. Customer sends employee to U.S. <strong>Safety</strong> <strong>Panel</strong> <strong>Optician</strong> assigned to Customer’sprogram.2. <strong>Optician</strong> assists employee with frame and lens selection, based on frame samplesavailable at optician office, and takes measurements for the employee’s order.*3. <strong>Optician</strong> calculates order total and advises employee of any advance paymentrequired, based on purchase limits designated by Customer.4. If advance payment required, employee provides credit card information or check(made out to U.S. <strong>Safety</strong>) to optician.5. <strong>Optician</strong> completes order form provided by U.S. <strong>Safety</strong> and faxes or mails orderto U.S. <strong>Safety</strong> (fax if advance payment by CC, mail if advance payment bycheck).6. U.S. <strong>Safety</strong> receives order and enters order according to <strong>Rx</strong> from optician.7. U.S. <strong>Safety</strong> processes any advance payment from employee and applies theamount as a credit to the order.8. U.S. <strong>Safety</strong> manufactures the prescription glasses and ships the glasses to theaddress indicated on the order form.9. U.S. <strong>Safety</strong> invoices <strong>VWR</strong> <strong>International</strong>, less any advance payment collectedfrom the employee, and pays dispensing fee to optician that placed the order.10. <strong>Optician</strong> receives completed order and notifies employee directly that theirglasses are ready for fitting.11. Employee returns to optician to have completed glasses fitted.12. <strong>VWR</strong> <strong>International</strong> invoices Customer for product(s) ordered and dispensing fee,less any advance payment collected from the employee.*** U.S. <strong>Safety</strong> will provide <strong>Panel</strong> <strong>Optician</strong>s with a reasonable number of framesamples to support the Customer’s program at no charge to the Customer.** Applicable taxes and shipping charges also apply.

Employee selects safety frame from sample kit at Customer site and obtains U.S.<strong>Safety</strong> order form and program brochure from Customer safety/administrativecontact.*2. Employee takes the frame sample, program brochure and order form to theirchoice of eye care professional.3. <strong>Optician</strong> assists employee with frame and lens selection, and takes measurementsfor the employee’s order.4. <strong>Optician</strong> calculates order total and advises employee of any advance paymentrequired, based on purchase limits designated by Customer.5. If advance payment required, employee provides check (made out to U.S. <strong>Safety</strong>)or credit card information to optician.13. <strong>Optician</strong> completes U.S. <strong>Safety</strong> order form and faxes or mails order to U.S. <strong>Safety</strong>(fax if advance payment by CC, mail if advance payment by check).6. If required, employee pays fee for dispensing services to optician at time ofservice (at the discretion of their eye care professional).7. Employee returns the frame sample and program brochure to the Customersafety/administrative contact.8. U.S. <strong>Safety</strong> receives order and enters order according to <strong>Rx</strong> from optician.9. U.S. <strong>Safety</strong> processes any advance payment from employee and applies theamount as a credit to the order.10. U.S. <strong>Safety</strong> manufactures the prescription glasses and ships the glasses to theaddress indicated on the order form.11. U.S. <strong>Safety</strong> invoices <strong>VWR</strong> <strong>International</strong>, less any advance payment from theemployee.12. <strong>Optician</strong> receives completed order and notifies employee directly that theirglasses are ready for fitting.13. Employee returns to optician to have completed glasses fitted.14. <strong>VWR</strong> <strong>International</strong> invoices Customer for product(s) ordered, less any advancepayment collected from the employee.*** Sample frame kits can be purchased by the Customer at a discounted sample price.** Applicable taxes and shipping charges also apply.


<strong>Process</strong> <strong>Flow</strong> – <strong>Rx</strong> <strong>Order</strong>U.S. <strong>Safety</strong> / <strong>VWR</strong> <strong>International</strong>On-Site <strong>Optician</strong> (In-Plant Dispensing):1. The U.S. <strong>Safety</strong> On-Site <strong>Optician</strong> makes regular visits to the Customer facility,according to the agreed-upon schedule.2. <strong>Optician</strong> assists the employee with frame and lens selection, based on the framesamples the optician brings in, and takes measurements for the employee’sorder.*3. <strong>Optician</strong> calculates the order total and advises the employee of any advancepayment required, based on purchase limits designated by the Customer.4. If advance payment is required, the employee provides credit card information ora check (made out to U.S. <strong>Safety</strong>) to optician.5. <strong>Optician</strong> completes the order form provided by U.S. <strong>Safety</strong> and faxes or mails theorder to U.S. <strong>Safety</strong> (fax if CC adv pay, mail if check adv pay).6. U.S. <strong>Safety</strong> receives the order and enters it into the U.S. <strong>Safety</strong> order system.7. U.S. <strong>Safety</strong> processes any advance payment from the employee and applies theamount as a credit to the order.8. U.S. <strong>Safety</strong> manufactures the prescription glasses and ships the glasses back to theU.S. <strong>Safety</strong> optician.9. U.S. <strong>Safety</strong> invoices <strong>VWR</strong> <strong>International</strong>, less any advance payment collectedfrom the employee.10. <strong>Optician</strong> receives the completed order and notifies the employee directly that theirglasses are ready for fitting.11. Employee returns to have the completed glasses fitted during the scheduled visitfrom the U.S. <strong>Safety</strong> On-Site <strong>Optician</strong>.12. <strong>VWR</strong> <strong>International</strong> invoices the Customer for the product(s) ordered, less anyadvance payment collected from the employee.*** U.S. <strong>Safety</strong> will provide On-Site <strong>Optician</strong>s with a reasonable number of framesamples to support the Customer’s program at no charge to the Customer.** Applicable taxes and shipping charges also apply.Hourly Dispensing Fees:The U.S. <strong>Safety</strong> On-Site <strong>Optician</strong> will submit time sheets directly to U.S. <strong>Safety</strong> for theservice hours performed at the Customer facility. On a bi-weekly basis, U.S. <strong>Safety</strong>will issue payment directly to the <strong>Optician</strong> for the service hours submitted. U.S. <strong>Safety</strong>will then invoice <strong>VWR</strong> <strong>International</strong>.
