Q6. Do you have anything available that would help us integrate the OASIS items<br />

into our own assessment?<br />

A6. The most current version <strong>of</strong> OASIS will be found on the CMS OASIS website.<br />

HHAs are required to incorporate the OASIS <strong>data</strong> items exactly as written into the<br />

agency's comprehensive assessment. For agencies using s<strong>of</strong>tware that does not<br />

accommodate bolding or underlining for emphasis <strong>of</strong> words in the same manner as the<br />

current OASIS <strong>data</strong> <strong>set</strong>, capitalizing those words is acceptable. We also recommend<br />

including the M item numbers when integrating to alert clinicians that the M items MUST<br />

be assessed and completed. Ultimately this will minimize delays in encoding due to<br />

uncompleted OASIS <strong>data</strong> items. Please refer to Chapter 4 <strong>of</strong> the OASIS-C Guidance<br />

cases where skilled nursing is not involved?<br />

A7. CMS does not have sample rehab assessment examples, though such<br />

assessments have been developed by commercial vendors. If an agency chooses to<br />

develop its own rehab-specific assessment <strong>forms</strong>, the principles for documenting OASIS<br />

items into an agency's clinical documentation are outlined in Appendix A <strong>of</strong> the OASIS-C<br />
