previously visited the patient. The information can be obtained over the telephone by any<br />
RN, PT, OT or SLP familiar with OASIS <strong>data</strong> collection practices. This guidance applies<br />
only to the Transfer and Death time points, as a visit is required to complete the<br />
comprehensive assessments and OASIS <strong>data</strong> collection at the Start <strong>of</strong> Care,<br />
Resumption <strong>of</strong> Care, Recertification, Other Follow-up and Discharge.<br />

assessment. What date do we list on OASIS for M0090 - Date Assessment<br />
Completed when information is gathered on day 1, 3 and 5?<br />
A14. Generally, you would enter the last day that assessment information was obtained<br />
on the patient in his/her home, if all clinical <strong>data</strong> items were completed. However, if the<br />
clinician needs to follow-up, <strong>of</strong>f site, with the patient's family or physician in order to<br />
complete an OASIS or non-OASIS portion <strong>of</strong> the comprehensive assessment, M0090<br />
should reflect the date that last bit <strong>of</strong> information is collected.<br />

Q15. M0090. We had a patient admitted to the hospital on April 15 and found out<br />
about it on April 19. When we enter the transfer (patient discharged) assessment<br />
(M0100 reason for assessment 7) into HAVEN, we get a warning message that the<br />
record was not completed within correct timing guidelines. (M0090) date should<br />
be no earlier than (M0906) date AND no more than 2 days after M0906 date.<br />
A15. That message is intended to be a reminder that you should complete a transfer<br />
assessment within 48 hours <strong>of</strong> learning <strong>of</strong> it. The regulation states that the assessment<br />
must be completed within 48 hours <strong>of</strong> learning <strong>of</strong> a transfer to an inpatient facility, so in<br />
this case, the assessment has been completed in compliance. The warning does not<br />
prevent the assessment from being transmitted. If you find that this warning occurs<br />
consistently, you may want to examine whether your staff are appropriately tracking the<br />
status <strong>of</strong> patients under their care.<br />

Q16. M0090. Is the date that an assessment is completed, in M0090, required to<br />
coincide with the date <strong>of</strong> a home visit? When must the date in M0090 coincide<br />
with the date <strong>of</strong> a home visit?<br />
A16. M0090, date assessment completed, records the date the assessment is<br />
completed. The start <strong>of</strong> care (SOC), resumption <strong>of</strong> care (ROC), follow-up, and discharge<br />
assessments (reason for assessments [RFA] 1, 3, 4, 5, and 9 for M0100) must be<br />
completed through an in-person contact with the patient; therefore these assessments<br />
will most <strong>of</strong>ten coincide with a home visit. The transfer or death at home assessments<br />
(RFAs 6, 7, or 8 for M0100) will report in M0090 the date the agency completes the<br />
assessment after learning <strong>of</strong> the event.<br />
In the situation where the clinician needs to follow up, <strong>of</strong>f site, with the patient’s family or<br />
physician in order to complete a specific clinical <strong>data</strong> item that the patient is unable to<br />
answer, M0090 should reflect that date.<br />
