Q17. Unless otherwise indicated, scoring <strong>of</strong> OASIS items is based on the patient's<br />

status on the “day <strong>of</strong> the assessment.” Does the “day <strong>of</strong> the assessment” refer to<br />

the calendar day or the most recent 24- hour period?<br />

A17. Since home care visits can occur at any time <strong>of</strong> the day, and to standardize the<br />

time frame for assessment <strong>data</strong>, the “day <strong>of</strong> the assessment” refers to the 24-hour<br />

period directly preceding the assessment visit, plus the time the clinician is in the home<br />

conducting the assessment. This standard definition ensures that fluctuations in patient<br />

status that may occur at particular times during the day can be considered in<br />

determining the patient’s ability and status, regardless <strong>of</strong> the time <strong>of</strong> day <strong>of</strong> the visit.<br />

Q19. Must the OASIS-C items (on the screen and when printed) match the <strong>data</strong> <strong>set</strong><br />

language and format exactly?<br />

A19. The OASIS hard copy information for the chart printed out by a point <strong>of</strong> care<br />

system must match the OASIS-C <strong>data</strong> <strong>set</strong> exactly, including formatting and wording for<br />

the items. If the printout <strong>of</strong> the assessment (i.e., the "hard copy" to be retained in the<br />

patient's clinical record) does not match the assessment <strong>data</strong> entered and submitted to<br />

the state, that may be problematic for the following reasons: 1) State surveyors will likely<br />

review records and compare the record on site in the agency with the <strong>data</strong> submitted to<br />

the state; 2) If a patient record was requested by the Fiscal Intermediary for medical<br />

review, it would be imperative that the printed record match the <strong>data</strong> collected and<br />

submitted to the state (since the same <strong>data</strong> were used to document the plan <strong>of</strong> care and<br />

calculate the billing codes); and 3) One way for an agency to monitor quality is to review<br />

responses to OASIS items in clinical records and compare those responses with <strong>data</strong><br />

collected at prior and subsequent visits to the same patient. If any <strong>of</strong> these processes<br />

would be complicated by the printouts received from your system, it could create<br />

problems for the agency.<br />

Due to the size and complexity <strong>of</strong> some <strong>of</strong> the items (e.g.<br />

M1020/1022/1024/1308/2100/22502400) the formatting may be modified to fit the<br />

computer screen as long as the hard copy print out matches the <strong>data</strong> <strong>set</strong> and the<br />

modification in no way impacts the accuracy <strong>of</strong> the item scoring.<br />

Q20. Our agency has been using a typical OASIS form that integrated the<br />

comprehensive assessment information with OASIS (as required by the<br />

Conditions <strong>of</strong> Participation) within one single form. We recently decided to use<br />

two separate <strong>forms</strong>. One form is the Comprehensive Assessment as stated above<br />

and the second is CMS OASIS-C. Someone told us that this was unacceptable<br />

and a single, physically integrated form is required. Is this true?<br />

A20. In order to be compliant with the Medicare Condition <strong>of</strong> Participation, 484.55,<br />

Comprehensive Assessment <strong>of</strong> Patients, the OASIS Assessment Items must be<br />
