The PHA — offeredas part of WellFocused — is a great resource to get a quick and reliable snapshot of yourcurrent health status. This health and lifestyle questionnaire covers topics such asnutrition, stress, physical activity, and health history, and will instantly provide personalizedresults and a plan you can use to take action right away. The PHA takes about 15 minutesto complete and is completely confidential.What’s more, when you complete the PHA, UPHS will reduce your FY12 health carepremiums by $130!The PHA is available to you and your spouse/domestic partner at any time. However, inorder to receive the $130 savings on your health premium you must complete the PHAduring the specified time at Open Enrollment, or if you are a new hire, during your benefitelection period.
