Monitoring TechniquesHazard identification <strong>and</strong> analysis techniques: Preliminary Hazard<strong>Analysis</strong> (PHA), Job Hazard <strong>Analysis</strong> (JHA), Hazards <strong>and</strong> Operability<strong>Analysis</strong> (HAZOP), Failure Modes <strong>and</strong> Effects <strong>Analysis</strong> (FMEA). Safetyinspection techniques. Comparison of active <strong>and</strong> reactive monitoringtechniques. Development of active <strong>and</strong> reactive monitoring policies <strong>and</strong>procedures to meet statutory <strong>and</strong> organisational needs.4. <strong>Accident</strong> InvestigationPurposes <strong>and</strong> principles of accident investigation. <strong>Accident</strong> investigationtechniques: h<strong>and</strong>ling of accident, collecting evidences, interviewingwitnesses, identifying causes <strong>and</strong> follow-up actions. <strong>Accident</strong> analysistechniques: Fault Tree <strong>Analysis</strong> (FTA), Management Oversight Risk Tree(MORT), Event <strong>and</strong> Causal Factor <strong>Analysis</strong> (ECFA). <strong>Accident</strong> reportwriting.5. <strong>Accident</strong> Recording <strong>and</strong> <strong>Analysis</strong>Documentation requirements related to the recording <strong>and</strong> reporting ofaccidents / incidents / ill-health. Definitions such as accident rate, severityrate, lost-time injury, <strong>and</strong> codes. Different levels of loss incidence analysis.State <strong>and</strong> enterprise levels of data collection. Statistical analysis. Workrelatedaccident costs. Presentation <strong>and</strong> publishing of loss data.LearningMethodologyBasic underst<strong>and</strong>ing of accident prevention <strong>and</strong> application of various hazard<strong>and</strong> accident analysis techniques will be covered in the lectures. Students willbe required to search for related literature or articles to support what they havelearnt. A site visit will be arranged for the student to conduct a safetyinspection.
