Subject Synopsis/Indicative Syllabus1. IntroductionLearning from critical incidents. Review of government requirements onaccident reporting and prevention. Classification of loss incidents.2. Theories of Accident CausationReview of accidents and incidents in the principal employment sectors.Accident Causation Models (Human Factors Model, Domino Model,Epidemiological Theory, Energy Exchange Model, System Model,Behavioural Theory of Accident Causation) and their roles in accidentprevention.3. Monitoring TechniquesHazard identification and analysis techniques: Preliminary HazardAnalysis (PHA), Job Hazard Analysis (JHA), Hazards and OperabilityAnalysis (HAZOP), Failure Modes and Effects Analysis (FMEA). Safetyinspection techniques. Comparison of active and reactive monitoringtechniques. Development of active and reactive monitoring policies andprocedures to meet statutory and organisational needs.4. Accident InvestigationPurposes and principles of accident investigation. Accident investigationtechniques: handling of accident, collecting evidences, interviewingwitnesses, identifying causes and follow-up actions. Accident analysistechniques: Fault Tree Analysis (FTA), Management Oversight Risk Tree(MORT), Event and Causal Factor Analysis (ECFA). Accident reportwriting.5. Accident Recording and AnalysisDocumentation requirements related to the recording and reporting ofaccidents / incidents / ill-health. Definitions such as accident rate, severityrate, lost-time injury, and codes. Different levels of loss incidence analysis.State and enterprise levels of data collection. Statistical analysis. Workrelatedaccident costs. Presentation and publishing of loss data.LearningMethodologyBasic understanding of accident prevention and application of various hazardand accident analysis techniques will be covered in the lectures. Students willbe required to search for related literature or articles to support what they havelearnt. A site visit will be arranged for the student to conduct a safetyinspection.
