6 Winter 1998RegistrationRegistrationRegistration consists of paying fees and enrolling in classes.• Registration fees and other University charges are paid throughthe <strong>UCLA</strong> Billing Statement (BAR), mailed monthly to your <strong>UCLA</strong>mailing address by the Student Accounting Office• Enrollment in classes is completed through URSA Telephone,(310) 208-0425 (see “URSA” for full details)You must complete both processes by the established deadlines (see“Calendar”) to be officially registered and enrolled for the term.Payments can be mailed, deposited in the Main Cashier’s Drop Slot(1125 Murphy Hall), sent Western Union Quick Collect, or faxed ifpaying with a credit card (see “Billing” for details). Payments submittedafter the published deadline must be made in person at 1125Murphy Hall and are assessed an additional $50 late payment fee.Mandatory Medical InsuranceRequirement<strong>UCLA</strong> requires, as a condition of registration, that all internationalstudents (graduate and undergraduate) on nonimmigrant visas andall graduate students have adequate medical insurance coverageduring all periods of enrollment.The following “insurance” plans are NOT acceptable and do NOT fulfillthe University requirement for adequate medical/health insurance:(1) travel insurance plans of any kind, (2) any plans purchased outsidethe U.S. and/or not issued by a U.S. company, (3) reimbursementarrangements or vouchers, including those from homegovernments and consulates in the U.S.<strong>UCLA</strong> offers students the Medical Insurance Plan (MIP) which fulfillsthe University requirement for adequate medical insurance. Forall graduate and international students, the MIP fee is includedeach term in the amount due on the <strong>UCLA</strong> Billing Statement (BAR).This is the only method by which MIP can be purchased.Students who decide to waive out of MIP because they have adequateprivate medical insurance must complete the Medical InsuranceWaiver Request included each term with the <strong>UCLA</strong> BillingStatement (BAR) and submit the form when they pay their registrationfees. However, students with private insurance who register afterthe second week of classes are not eligible to waive out of MIP.Students who do not purchase the <strong>UCLA</strong> Medical Insurance Planmust have an adequate private medical insurance plan that providesall of the following minimum benefits:(1) A minimum of $100,000 in lifetime benefits.(2) Coverage of at least 75 percent of medical expenses, with a deductibleof $500 or less and a copayment of 20 percent or less.(3) A policy issued in the U.S. by a U.S. carrier.(4) Inclusion of Medical Evacuation and Repatriation benefits forthose on J-1 or J-2 visas.If the private medical insurance plan does not meet all of the aboverequirements, students must purchase MIP.For all other undergraduate students, the MIP fee appears as avoluntary option on the <strong>UCLA</strong> Billing Statement (BAR) and is in additionto the amount due each term. To request MIP, students must selectit by marking that item on the remittance portion of the <strong>UCLA</strong>Billing Statement. The remittance slip must be returned to the <strong>UCLA</strong>Main Cashier by the published registration deadline each term. Thisis the only method by which MIP can be purchased.This serves as notice of the mandatory medical insurance requirement;it is each student’s responsibility to obtain the necessary informationbefore they register. Changes and exceptions requested afterstudents have registered cannot be honored. MIP cancellation requestsmust be made to the Student Health Service (SHS) InsuranceOffice within 10 days from the MIP effective date. For further informationon MIP or adequate medical insurance requirements, call theSHS Insurance Office at (310) 825-1856.HoldsStudents may have “holds” placed on their student record, whichcould affect registration, enrollment, financial aid, and/or release ofacademic transcripts for the following reasons:1. Failure to comply with admission provisions2. Failure to settle financial obligations with the University3. Failure to respond to official noticesEach student who becomes subject to a hold action is given advancenotice and ample time to respond. In most cases, the hold must bereleased from the initiating office before a student may enroll inclasses or receive various campus services. You can obtain holds informationon URSA OnLine at http://www.ursa.ucla.edu/ or a readingof your holds on URSA Telephone at (310) 208-0425.Enrollment in ClassesThe “Class Listings” section of this schedule has class times, meetingrooms, instructors, and other information necessary for enrollment.For the most up-to-date listings, see the online Schedule ofClasses. Using the class listings and academic counseling from yourschool or college advisers (see “Counseling”), you can assemble aprogram of courses.You should plan two or three alternate programs in case your firstchoice of courses is not available (use the “Enrollment Worksheet” onthe inside back cover of this schedule to list alternate courses). Donot choose two courses in the same final examination group (see thefinal examination chart in “Class Listings”) or select classes that conflictin meeting times. If conflicts are unavoidable, consult with the instructorof each course at the first class meeting.URSAUniversity Records System Access (URSA) enables all <strong>UCLA</strong> studentsto acquire information from their University academic recordsstored on the student system computer databases. Below is an overviewof URSA features; see the “URSA” section for full details.URSA Telephone EnrollmentURSA Telephone (310-208-0425) allows you to process your classenrollment, to obtain course confirmation (i.e., Study List, includingday/time, location, examination code, instructor name), your exactURSA Telephone enrollment appointment time, <strong>UCLA</strong> grades for anycompleted term, GPA, completed units, and outstanding holds, toconfirm registration fee payment, to update or review selected studentinformation (“degree expected term,” telephone number, residencehall address, privacy release, ethnic-based mailing option, andethnic background), and to change the security code used to accessURSA.Through URSA Telephone’s Student Records Menu, you can enroll inclasses, add, drop, or exchange classes/sections, put yourself on thewait list for a class, add a class using a PTE Authorization Number,change the grading basis for a class (i.e., Passed/Not Passed), obtaina reading of your Study List, check your wait-list position, and obtaininstructor names for all courses. You enroll during your assignedappointment periods, which you also obtain either by calling URSATelephone or using URSA OnLine.
