72Standard 6: Graduation from the ProgramStandard 6 indicates that EPI is designed to be a time limited service. Most clients will stay forup to 3 years and when they graduate, they should be linked to the least intensive and leastintrusive level of care required to meet their ongoing needs.Summary of Results:o Implementation of this Standard was lower, with only 56% of sites implementing ‘most ofthe time’. This element was a challenge across all sites.o About a third of programs reported that over 25% of current clients had been in the programfor more than 3 years, and the portion was greater for community than for hospital sponsoredsites (47% versus 21%).o Upon program graduation, clients are mainly referred to primary care, outpatient psychiatryservices and - especially among community sponsored programs - case management.o Clients may be referred to services within the same organization.o The main challenges to transferring care at the end of 3 years included the lack of appropriatealternatives and clients not being ready to leave.o Allowing more flexibility in the length of time clients can remain in the program was raised. Also suggested was providing a step-down/transition option, and exploring shared careoptions with PC physicians.Chart 15: Graduation from the program: Implementation* relative to other programs (Q 130)Public educationEarly referralWellness plansGraduat ionOutreach andFamily educationPhysical healthFamily supportWorking with IP unitsCrisis managementFamily role in assessmtClient psycho-educationAccess to psy assessmtResponse timesAntipsychotic meds useRelapse preventionstandard elements0 20 40 60 80 100% of sites reporting implementation 'most of the time'* % programs reporting implementation ‘most of the time’ versus ‘some of the time’ or ‘not too often’.
