Work began Aug. 22 on a 3,400 square-foot clinic
addition that will house 12 new exam rooms as well as
three additional provider offices. Construction is
expected to be complete in early 2012.
“This expansion is vital for us to continue providing the
our medical staff and specialty services.”
The $1 million project will be funded by CentraCare Health System – Long
Better care, better treatment, better future
Long Prairie ranked third in state for decreasing readmissions for depression
Improving care for patients with depression is a priority
at CentraCare Health System – Long Prairie. In a recent
Minnesota Community Measurement report, Long
Prairie ranked third in the state for decreasing
readmissions to its clinic for depression.
“It’s an honor to be identified among the top three
clinics in the state,” said Toni Tebben, clinic coordinator.
“Our staff and providers are helping patients with
depression get to the point where they have no
symptoms, or almost no symptoms, six or 12 months
after starting treatment.”
The report measures how well patients are feeling
based on feedback from a patient questionnaire. It is
the only measure that reports outcomes for all patients
with diagnosed depression without regard to care
setting, treatment approach or provider’s ability to
maintain contact with the patient.
“This recognition confirms that we are on the right
track,” Tebben said. “We will continue to work hard to
find ways to make a meaningful difference in improving
the health of our community.”
DIAMOND Program
At CentraCare Health System – Long Prairie, patients
who are diagnosed with depression can choose to
enroll in the DIAMOND (Depression Improvement
Across Minnesota, Offering a New Direction) program.
They are assigned to a care manager who monitors the
patient’s progress through regular follow-up care.
“It’s a team-based approach to treatment,” said
psychiatrist Chris Moellentine, MD, of the St. Cloud
Hospital Behavioral Health Clinic and an integral part of
the DIAMOND program. “Care managers assist patients
with goal-setting and address any barriers to care.”
