Furthermore, gonioscopic findings maybe affected by inadvertent pressure on the gonioscopy lens and by increasedillumination (which tends to open the ACA) during the examination. Previousstudies have shown that even experienced, cross-trained examiners have onlymoderate agreement in determining angle width. 1,2Anterior segment (AS) optical coherence tomography (OCT) uses a 1093-nmdiode laser to obtain real-time images of the Anterior chamber angle andprovides a rapid non-contact method for the detection of angle closure. 3-5 Theaim of this study was to compare the performance of gonioscopy and AS OCTin detecting angle closure in a cohort of patients attending a community clinicfor non-ophthalmic care and to investigate variations in findings between thetwo methods in the different quadrants of the Anterior chamber angle.MATERIALS AND METHODSThe first random 250 eyes of 250 patients presenting to the <strong>Glaucoma</strong> clinicsuspected of having occludable angles on primary slit lamp evaluation wereincluded in the study if they fulfilled the inclusion and exclusion criteria. Aftertaking the history to obtain previous medical and ophthalmic condition, eachsubject underwent the following examinations on the same day: Visual acuity,Anterior segment imaging by AS OCT (Visante; Carl Zeiss Meditec, DublinCA), Slit-lamp biomicroscopy, Goldmann applanation tonometry, and Indirectgonioscopy. Only subjects greater than 18 years were included. Subjects wereexcluded if they had a history of intraocular surgery or penetrating trauma ineither eye, previous anterior segment laser treatment, or a history of glaucoma.279
