Employee and spouse completing a health risk assessment. (If only theemployee or only the spouse completes assessment, the premium reductionshall be $200 for a family plan in each year.)2. Completing one of the following will reduce the premium share by $200 for afamily plan and $100 for a single plan. If both are completed, the premium sharewill be reduced by $200 for a family plan and $100 for a single plan:Employee participating in a minimum of three health education seminars.3.
