Additionally, the plan will require a place <strong>of</strong> employment that is free<strong>of</strong> unsanitary or hazardous conditions that would harm an employee’s health orsafety.8. <strong>Safety</strong> Inspections: Detailed information concerning how daily safetyinspections will be conducted, and their documentation.9. <strong>Safety</strong> Personnel: State the name <strong>of</strong> the Contractor’s <strong>Safety</strong> Representative(s),their experience and qualifications. Training in the OSHA 500, 10-hour <strong>Safety</strong>and Health Course. Indicate their authority to take the appropriate measures toeliminate hazards or stop work until hazardous conditions are corrected.10. <strong>Safety</strong> Requirements, Electrical: Testing, inspection and repair <strong>of</strong> electricalequipment, GFCI Program, lockout/tagout procedures, how existing circuitswill be located and the installation <strong>of</strong> electrical circuits in accordance with theNational Electric Code or <strong>Port</strong> Mandated Requirements.11. <strong>Safety</strong> Requirements, Equipment: Operation, inspection, and maintenance fortrucks and heavy equipment such as backhoes, dozers, motor graders, elevatingwork platforms, powered industrial trucks, and hand and power tools.12. <strong>Safety</strong> Requirements, Ladders: Types <strong>of</strong> ladders for specific uses and theirtraining requirements.13. Site Layout: A layout drawing <strong>of</strong> the site indicating access roads, fire andambulance lanes, location <strong>of</strong> first aid stations, location <strong>of</strong> required alarmsystems, location <strong>of</strong> <strong>of</strong>fices, parking for private vehicles and equipment, andstorage <strong>of</strong> all flammable and/or combustible liquids, gases, or other hazardousmaterials.14. Storage: Requirements for storage <strong>of</strong> flammable and combustible liquids orgases.15. Field Sanitation: Provisions for toilet and hand washing facilities, includingthe frequency at which they will be cleaned and maintained.B. SPECIAL PROVISIONSDepending on the type <strong>of</strong> construction, additional items must be incorporated into theContractor’s <strong>Safety</strong> Plan.1. Confined Space Entry: Procedures for confined space entry and workoperations in and around confined spaces (including elevator shafts) as well asemergency measures. When entry is to be made into a Permit RequiredConfined Space the <strong>Port</strong> <strong>of</strong> <strong>Seattle</strong> Fire Department shall be contacted. Theseprocedures must meet or exceed the <strong>Port</strong> Of <strong>Seattle</strong> requirements found in theCapital Improvement Program’s <strong>Safety</strong> & Health Manual.2. Cranes: Use <strong>of</strong> cranes or derricks and the testing and inspection there<strong>of</strong>,including hooks, latches, wire rope, operator certification, boom stops, loadcharts, wind speed warning devices, fire extinguishers, crane operation signals,suspended work platforms and pre-lift planning.4. Excavations: Excavation plans must indicate sloping, documented dailyinspections, shoring, barricading, excavation access, fall protection, andexcavated material storage.5. Fall Protection: How 100% protection will be maintained, identify the use <strong>of</strong>personal fall arrest equipment, fall protection systems, and fall protection workplans for heights 4 feet. NOTE: The Monitor System is prohibited!6. Formwork: Submittal <strong>of</strong> formwork and false work drawings for review andapproval to the TCI.7. Hazard Communication Program: including MSDS, their location, MasterList <strong>of</strong> Chemicals, Personal Protective Equipment, Training, Labeling, andMSDS review and special procedures for use <strong>of</strong> sealers and specialty paints.8. Interruption <strong>of</strong> Fire/Security Systems: Plans shall include measures and/orprocedures to provide interim fire and security protection to facilities or areasaffected by interruptions. These include automatic detection devices andalarms, automatic sprinkler systems, fire pumps, fire hydrants, applicable watersupplies and reservoirs.9. Lockout/Tagout: Procedures for lockout/tagout <strong>of</strong> energy sources during workoperations.10. Scaffolding: “Use” tag system, planking, guardrails, toe boards, anchor points,fall protection, access points, and inspections <strong>of</strong>.11. Fire Protection Plan: To include, Hot Work Permits, welding, shields, fireextinguishers, ventilation, PPE, fire watches and cylinder storage.12. Work Adjacent To Occupied Spaces: Procedures for ensuring occupants <strong>of</strong>spaces adjoining, above and below construction areas will be protected fromhazards created by construction, including but not limited to, falling debris,equipment noise, and penetration <strong>of</strong> partitions, ceilings, and floors.13. Competent Persons: Where regulatory requirements (WISHA) specify the use<strong>of</strong> Competent Persons, the Contractor shall submit in writing the names <strong>of</strong> thosepersons, their area <strong>of</strong> competency and applicable experience/trainingdocumentation.14. Energized Electrical Work Plan: Submit detailed procedures for working onand guarding <strong>of</strong> energized equipment, baggage handling systems, or conductingsystem outages.15. Health Considerations: The Contractor shall submit a plan that addressessafety & health procedures for working in contact with contaminated soils.This plan shall be revised and resubmitted in the event that conditions00860-23 Tenant
