It contains generalinformation and serves as a claim form for preference points for Broad-Based BlackEconomic Empowerment (B-BBEE) Status Level of ContributionNB:BEFORE COMPLETING THIS FORM, BIDDERS MUST STUDY THE GENERALCONDITIONS, DEFINITIONS AND DIRECTIVES APPLICABLE IN RESPECT OF B-BBEE, AS PRESCRIBED IN THE PREFERENTIAL PROCUREMENT REGULATIONS,2011.1.
