Step 5 ­ Establish Data Collection And Report<strong>in</strong>g System<br />

n The <strong>in</strong>formation and data required for all SPIs should be collected on an agreed regular basis;<br />

n It is advisable to appo<strong>in</strong>t one person to ensure data is collected, compile reports to <strong>in</strong>form<br />

management of data and highlight any deviations from tolerance levels to management;<br />

n Presentation of data should be summarised <strong>in</strong> a s<strong>in</strong>gle sheet and kept as clear and simple as<br />

possible. Any deviations from tolerance levels should be highlighted. A traffic light system (green – ok,<br />

amber – slight deviations, red – large deviations) is a useful way to present data;<br />

n If a traffic light system is used, it is important to be m<strong>in</strong>dful that aggregate scores can result <strong>in</strong> an “all<br />

green” or “all red” outcome on an <strong>in</strong>dicator. It may be useful to adopt a weight<strong>in</strong>g system so that <strong>the</strong><br />

overall results reflect a true picture of safety <strong>performance</strong>.<br />

Step 6 ­ Review<br />

Senior managers should review <strong>the</strong> entire SPI programme rout<strong>in</strong>ely. This review should address <strong>the</strong> follow<strong>in</strong>g:<br />

n Are <strong>the</strong> chosen <strong><strong>in</strong>dicators</strong> mean<strong>in</strong>gful and useful? i.e. is useful <strong>in</strong>formation be<strong>in</strong>g collected that can be<br />

acted upon.<br />

n Are <strong>the</strong> tolerances levels appropriate? For example, if a traffic light system was chosen to rate<br />

housekeep<strong>in</strong>g tours (>90% is green, 80­90% is amber and
