There Is Always Hope...<br />

What do breast cancer and osteoporosis have in common? Well, they invariably<br />

affect women more than men. As the month of <strong>October</strong> commemorates both<br />

these deadly diseases, it is time we take a closer look at them.<br />

A data provided by Asia Pacific Osteoporosis audit of Malaysia in 2013 shows<br />

that over the next four decades, the number of Malaysian population aged over<br />

70 years is expected to increase by 417%. This shows an increment in number<br />

from 939,010 in 2013 to 4.8 million in 2050. The life expectancy will also increase<br />

seven-years longer than today, rising from 74 years to 81 years. Data shows that<br />

this is also period where rapid decline of bone density with age which is the<br />

hallmark feature of osteoporosis takes place.<br />

This phenomenon is seen more markedly in women compared to men especially<br />

after the age of 50. It is estimated that osteoporosis will result in 1 in 3 women<br />

and 1 in 5 men to have fracture. If you're a woman, are you taking enough<br />

precaution and remedies to tackle the scourge of osteoporosis?<br />

While most people are aware of breast cancer, many forget to take the steps<br />

to have a plan to detect the disease in its early stages and encourage others<br />

to do the same. To motivate you to take action, we spoke to two breast cancer<br />

survivors… who somehow summed up enough courage to fight off this dreaded<br />

disease. Hopefully their story will increase awareness of the disease and inspire<br />

you to take early action.<br />

29th <strong>October</strong> <strong>2016</strong> is World Psoriasis Day. It is an annual day<br />

aimed to raise awareness, increase understanding and build<br />

unity for more than 125 million people with psoriasis and/or<br />

psoriatic arthritis around the world.<br />

Nearly three percent of the world’s population, from men,<br />

women, children or even newborn babies experience<br />

symptoms of psoriasis. Psoriasis is a common chronic,<br />

relapsing, immune-mediated and inflammatory disorder<br />

with primary involvement of the skin. The skin cell growth<br />

of psoriasis sufferer is about 3 times faster than normal<br />

and results in thick, red and silvery scales on skin surface.<br />

A substantial proportion of psoriasis patients experience<br />

inflammatory polyarthritis called psoriatic arthritis which<br />

causes physical, functional and psychosocial disability.<br />

Psoriasis cannot be cured, but there are many ways of<br />

controlling it. There are some tips to manage and avoid flare<br />

up. Keeping skin moist is important and effective yet easier<br />

way to reduce skin dryness, itchiness, redness, soreness and<br />

scaling. Other suggestions are to soak in warm water for<br />

10–15 minutes; expose under small doses of sun as ultraviolet<br />

light in sunlight can slow down growth of skin cell; avoid harsh<br />

chemicals products to inflamed skin; and avoid scratching and<br />

remember to keep fingernails cut short.<br />

Your eyes can tell so many stories about<br />

you. As you age, over time, eyelid<br />

But not to fret! Top dermatologists have<br />

offered their secret treatments and<br />

makeup tricks that can brighten and<br />

wake your skin around the eyes up.<br />

Here are some of their best kept secrets:<br />

Puffy eyes<br />

Some puffiness is genetic, and no eye<br />

cream will erase or reduce it. But if your<br />

bags come and go, fluid retention is<br />

probably the cause. Salty dinners, late<br />

nights, a cocktail or two — they can all<br />

make your body hold on to water. Since<br />

lid skin is thinner than elsewhere on your<br />

face, you'll notice swelling more.<br />

What you can do: Thanks to the pull<br />

of gravity, morning facial puffiness is<br />

usually gone by lunchtime. To speed<br />

things up, try placing a chilled eye<br />

compress, cold water-soaked tea bags,<br />

or cucumber slices over lids for five<br />

minutes. Cold constricts blood vessels<br />

and reduces swelling. The caffeine in<br />

many eye creams is tightening, too.<br />

Gently press on the bones around your<br />

eyes as you apply it. The pressure<br />

Dark circles<br />

Since skin becomes more translucent<br />

with age, blood vessels appear more<br />

prominent. When blood vessels show<br />

through skin, they can give the eye area<br />

a bluish cast. A less common cause<br />

of darkness: post-inflammatory hyper<br />

pigmentation, doctor-speak for clusters of<br />

brown pigment that collect underneath<br />

the eyes due to heredity or chronic<br />

rubbing (often because of allergies or<br />

bothersome contact lenses).<br />

What you can do: Daily use of any basic<br />

eye cream will temporarily plump skin so<br />

blood vessels are less visible. Eye creams<br />

are a better choice for this purpose than<br />

regular face creams, because they tend to<br />

have a higher concentration of emollients<br />

for plumping and a thicker texture, which<br />

makes them adhere to the ever-mobile<br />

eye area. To improve the look of brown<br />

pigment, try an eye cream with botanical<br />

skin lighteners like licorice and kojic acid.<br />

Lines and wrinkles<br />

Every time you smile, frown, yawn,<br />

or squint, the skin around your eyes<br />

crinkles up. Eventually, as the collagen<br />

levels diminish, your skin literally stays<br />

scrunched, and deeper creases take the<br />

place of fine lines.<br />

What you can do: A few ingredients are<br />

essential. The first is retinol, a vitamin A<br />

derivative that rebuilds collagen levels<br />

and helps smooth fine lines. Antioxidants<br />

like green tea and vitamin C, which help<br />

skin retain collagen, are effective wrinkle<br />

reducers, too. If your skin is especially<br />

sensitive, swap vitamin A products for<br />

peptides — proteins that also stimulate<br />

collagen growth.<br />

Sagging lids<br />

As collagen, elastin, and fat decrease<br />

with age, eyelids get droopier. Genetics<br />

determine when your skin will sag and<br />

to what extent. And like any skin ageing,<br />

sun exposure hastens the effect. As a<br />

result, some women have sagging in their<br />

late 30s.<br />

What you can do: No eye cream alone<br />

will reverse sagging once skin begins its<br />

downward slide. Only surgery gives you<br />

truly long-term improvement. But artfully<br />

applied eye makeup can make drooping<br />

lids less noticeable.<br />

Surgery to the rescue<br />

When you want faster, more dramatic<br />

results than even the best eye cream or<br />

makeup trick can offer, you'll find plenty of<br />

longer-lasting (but costlier) options at the<br />

dermatologist's or plastic surgeon's office.<br />

Best for banishing<br />

wrinkles<br />

Botox injections. The doctor makes tiny<br />

injections of botulinum toxin into the<br />

muscles that cause wrinkles, forcing them<br />

to relax. The injections take effect in a<br />

week or so, and the area stays smooth<br />

for three to six months.<br />

Chemical peels. A medium-depth peel<br />

(like trichloroacetic acid, a.k.a. TCA)<br />

stimulates collagen growth by removing<br />

the top layer of skin. The body senses an<br />

injury and cranks out new healthy skin<br />

as a way of healing itself. You'll look like<br />

you have a bad sunburn for five to seven<br />

days, but then facial skin will be smoother<br />

and less lined for six months to a year.<br />

Lasers. Many kinds of lasers are used<br />

around the eyes, usually as part of an all<br />

over facial rejuvenation treatment. Lasers<br />

help reduce wrinkles by resurfacing skin<br />

and stimulating collagen production. As<br />

with a peel, you'll look badly sunburned<br />

for a few days to a week before the<br />

smoothing shows up; it'll last for a year or<br />

more, until new wrinkles form.<br />

Best for hiding dark<br />

circles<br />

Hyaluronic acid injections. This clear gel<br />

filler is injected directly into the hollows<br />

beneath the eyes to even them so they no<br />

longer appear as dark shadows. Expect<br />

swelling, mild bruising, and tenderness<br />

for up to a week afterward (the bruising<br />

can be covered with concealer). Results<br />

will last three to six months — or more.<br />

Best for lifting sagging<br />

lids<br />

Blepharoplasty. During this surgery, the<br />

doctor makes tiny incisions around or inside<br />

the eyelids to remove loose skin and excess<br />

fat, which create a droopy, sleepy look. It<br />

takes about two weeks before the bruising<br />

goes down; you may not want to return to<br />

work until then. But the tightening effect lasts<br />

for years. This procedure could make one<br />

look 10 to 15 years younger.<br />

The skin around<br />

the eye can be 1/10th as<br />

oil glands around the eyes, this part of<br />

the face is more likely to show premature<br />

signs of ageing if it is neglected. The eye<br />

area can show everyone your lack of sleep,<br />

overindulgence of food and alcohol and<br />

just how good you’re feeling or not. It is<br />

therefore necessary to pay special<br />

attention to the eye area and<br />

treat it diligently and<br />

Laugh lines-crows feet<br />

Laugh lines commonly called crow’s<br />

feet, are caused by the breakdown of<br />

collagen in the skin, a direct result of sun<br />

exposure and extrinsic ageing, just like<br />

other wrinkles on the face. Since skin<br />

around the eyes is much thinner than<br />

that of the face, wrinkles may appear<br />

prematurely. Using an eye cream twice<br />

daily will soften the look of fine lines<br />

and deeply moisturise the delicate<br />

skin around the eyes. Adding a serum<br />

containing Collagen and Hyaluronic<br />

Acid, will keep skin firm, moist and<br />

plump. Apply a few drops under<br />

your eye cream twice daily. Avoiding<br />

excessive sun exposure and using eye<br />

creams are the keys to preventing signs<br />

of aging around the eyes.<br />

Puffiness and bags<br />

Lack of sleep, meals high in salt, high<br />

blood pressure, alcohol, and stress<br />

can all result in puffy eyes, particularly<br />

in the morning when the periorbital<br />

area retains fluid. Most often this is<br />

not a serious condition and usually<br />

improves as the day goes by mainly<br />

because being in a vertical position<br />

helps redistribute the fluid around the<br />

skin. Using and eye gel directly from<br />

the refrigerator works as a soothing,<br />

cooling morning treatment for tired,<br />

puffy eyes. In some cases the puffiness<br />

is not caused by the skin retaining<br />

fluid but by the protrusion of the fatty<br />

pad under the eye. Your dermatologist<br />

or a plastic surgeon can recommend<br />

treatment options in this case.<br />

Dark circles<br />

Dark circles are caused by blood<br />

vessels showing through the delicate<br />

skin around the eye area giving the<br />

under eye area a bluish, purple cast,<br />

often a hereditary condition. Fluid and<br />

hemoglobin from leaky capillaries can<br />

pool under the eyes giving a bruised<br />

effect and nasal congestion dilates<br />

and darkens the veins that drain from<br />

the eyes to the nose. Dark circles can<br />

also be caused by anemia. The thinner<br />

the skin and the more deep set the<br />

eyes are, the worse the condition can<br />

appear. Skin can appear more pale<br />

due to lack of sleep, hormonal changes<br />

and pregnancy, which intensify the look<br />

of the dark circles though these factors<br />

do not actually cause dark circles.<br />

How you can minimise<br />

the look of dark circles<br />

and puffiness:<br />

• Use an eye cream rich in antioxidants<br />

like Vitamins A, E and C which can<br />

help strengthen capillary walls.<br />

• Avoid over indulging in salty foods<br />

and alcohol which cause the skin to<br />

retain fluids, resulting in a shadowy<br />

look under the eyes.<br />

• Getting enough rest will minimise the<br />

look of dark circles.<br />

• Place cold cucumber slices, an ice<br />

mask or tea bags over the eyes to<br />

alleviate swelling.<br />

• Rinse your face quickly with ice cold<br />

water to reduce swelling.<br />

When it comes to skin care, most of us fall into one of two camps. There are those of us who never change<br />

our routine, using the same ill-suited face wash and moisturiser from our high school days. Then there's the<br />

other group, who embrace treatments like Botox and acid peels before there's a wrinkle in sight.<br />

As we age, our skin changes. What<br />

worked in our 20s won't do us any<br />

favour in our 30s or 40s. If the goal is<br />

to maintain a youthful, dewy glow well<br />

into our golden years, and look just as<br />

radiant every decade in between, we<br />

need to tailor our skin care regimen to<br />

the number of candles on our birthday<br />

cake. Here, how to put your best face<br />

forward at every age.<br />

If you're in your 20s<br />

At this age, your goal is to preserve and<br />

protect. In your 20s, many of the skin<br />

issues you see come from carefree living<br />

(and neglect): partying on weekends,<br />

drinking too much, pulling all-nighters<br />

and basking in the sun without<br />

sunscreen. There might be lingering<br />

acne from the teen years as well. Hard<br />

living comes at a cost, though: fine<br />

lines can start to creep up in the second<br />

half of your 20s. To stave them off and<br />

counteract your skin sins, it's important<br />

to start a consistent skin care routine that<br />

Water is essential to good health,<br />

yet needs vary by individual. These<br />

guidelines can help ensure you drink<br />

enough fluids.<br />

How much water should you drink each<br />

day? It's a simple question with no easy<br />

answers. Studies have produced varying<br />

recommendations over the years, but in<br />

truth, your water needs depend on many<br />

factors, including your health, how active<br />

you are and where you live.<br />

Although no single formula fits everyone,<br />

knowing more about your body's need<br />

for fluids will help you estimate how much<br />

water to drink each day.<br />

<strong>Health</strong> benefits of water<br />

Water is your body's principal chemical<br />

component and makes up about 60<br />

percent of your body weight. Every<br />

system in your body depends on water.<br />

For example, water flushes toxins out of<br />

vital organs, carries nutrients to your cells,<br />

and provides a moist environment for ear,<br />

nose and throat tissues.<br />

Lack of water can lead to dehydration, a<br />

condition that occurs when you don't have<br />

enough water in your body to carry out<br />

normal functions. Even mild dehydration<br />

can drain your energy and make you tired.<br />

How much water do<br />

you need?<br />

Every day you lose water through your<br />

breath, perspiration, urine and bowel<br />

movements. For your body to function<br />

properly, you must replenish its water<br />

supply by consuming beverages and<br />

foods that contain water.<br />

So how much fluid does the average,<br />

healthy adult living in a temperate<br />

climate need? The Institute of Medicine<br />

determined that an adequate intake (AI)<br />

for men is roughly about 13 cups (3<br />

litres) of total beverages a day. The AI for<br />

women is about 9 cups (2.2 litres) of total<br />

beverages a day.<br />

What about the advice to<br />

drink 8 glasses a day?<br />

Everyone has heard the advice, "Drink<br />

eight 8-ounce glasses of water a day."<br />

That's about 1.9 litres, which isn't that<br />

different from the Institute of Medicine<br />

recommendations. Although the "8 by 8"<br />

rule isn't supported by hard evidence,<br />

it remains popular because it's easy to<br />

remember. Just keep in mind that the<br />

rule should be reframed as: "Drink eight<br />

8-ounce glasses of fluid a day," because<br />

all fluids count toward the daily total.<br />

Factors that influence<br />

water needs<br />

You may need to modify your total fluid<br />

intake depending on how active you are,<br />

the climate you live in, your health status,<br />

and if you're pregnant or breast-feeding.<br />

Exercise. If you exercise or engage in any<br />

activity that makes you sweat, you need to<br />

drink extra water to compensate for the fluid<br />

loss. An extra 1.5 to 2.5 cups (400 to 600<br />

milliliters) of water should suffice for short<br />

bouts of exercise, but intense exercise lasting<br />

more than an hour (for example, running a<br />

marathon) requires more fluid intake. How<br />

much additional fluid you need depends on<br />

how much you sweat during exercise, and<br />

the duration and type of exercise.<br />

Intense exercise. During long bouts<br />

of intense exercise, it's best to use a<br />

sports drink that contains sodium, as this<br />

will help replace sodium lost in sweat<br />

and reduce the chances of developing<br />

hyponatremia, which can be lifethreatening.<br />

Also, continue to replace<br />

fluids after you're finished exercising.<br />

Environment. Hot or humid weather can<br />

make you sweat and requires additional<br />

intake of fluid. Heated indoor air also<br />

can cause your skin to lose moisture<br />

during wintertime. Further, altitudes<br />

greater than 8,200 feet (2,500 metres)<br />

may trigger increased urination and more<br />

rapid breathing, which use up more of<br />

your fluid reserves.<br />

Illnesses or health conditions. When<br />

you have fever, vomiting or diarrhoea,<br />

your body loses additional fluids. In these<br />

cases, you should drink more water. In<br />

some cases, your doctor may recommend<br />

oral rehydration solutions. You may also<br />

need increased fluid intake if you develop<br />

certain conditions, including bladder<br />

infections or urinary tract stones. On the<br />

other hand, some conditions, such as<br />

heart failure and some types of kidney,<br />

liver and adrenal diseases, may impair<br />

excretion of water and even require that<br />

you limit your fluid intake.<br />

Pregnancy or breast-feeding. Women<br />

who are pregnant or breast-feeding<br />

need additional fluids to stay hydrated.<br />

Large amounts of fluid are used<br />

especially when nursing. The Institute<br />

of Medicine recommends that pregnant<br />

women drink about 10 cups (2.3 litres)<br />

of fluids daily and women who breastfeed<br />

consume about 13 cups (3.1 litres)<br />

of fluids a day.<br />

Beyond the tap: Other sources of water<br />

You don't need to rely only on what you<br />

drink to meet your fluid needs. What you<br />

eat also provides a significant portion<br />

of your fluid needs. On average, food<br />

provides about 20 percent of total water<br />

intake. For example, many fruits and<br />

vegetables, such as watermelon and<br />

spinach, are 90 percent or more water<br />

by weight.<br />

In addition, beverages such as milk<br />

and juice are composed mostly of<br />

water. Even beer, wine and caffeinated<br />

beverages — such as coffee, tea or<br />

soda — can contribute, but these should<br />

not be a major portion of your daily total<br />

fluid intake. Water is still your best bet<br />

because it's calorie-free, inexpensive and<br />

Staying safely hydrated<br />

Generally, if you drink enough fluid<br />

so that you rarely feel thirsty and your<br />

urine is colourless or light yellow — and<br />

measures about 6.3 cups (1.5 litres) or<br />

more a day if you were to keep track —<br />

your fluid intake is probably adequate. If<br />

you're concerned about your fluid intake<br />

or have health issues, check with your<br />

doctor or a registered dietitian. He or she<br />

can help you determine the amount of<br />

water that's right for you.<br />

To ward off dehydration and make sure<br />

your body has the fluids it needs, make<br />

water your beverage of choice. It's also<br />

a good idea to:<br />

Drink a glass of water or other caloriefree<br />

or low-calorie beverage with each<br />

meal and between each meal<br />

Drink water before, during<br />

and after exercise<br />

Although uncommon, it is possible<br />

to drink too much water. When your<br />

kidneys are unable to excrete the excess<br />

water, the electrolyte (mineral) content<br />

of the blood is diluted, resulting in low<br />

sodium levels in the blood, a condition<br />

called hyponatremia. Endurance athletes,<br />

such as marathon runners who drink<br />

large amounts of water, are at higher<br />

risk of hyponatremia. In general, though,<br />

drinking too much water is rare in healthy<br />

adults who eat an average diet.<br />

their health was not as good as it was<br />

five years ago. Even more worrying is<br />

that 61% of adults under age 30 agreed<br />

with this sentiment, although this younger<br />

age group should be in the prime of<br />

their health. For the first time, the survey<br />

also uncovered obesity as a growing<br />

challenge for both adults and children.<br />

Those surveyed recognised that healthy<br />

living involved a combination of physical<br />

elements such as getting sufficient sleep<br />

and eating healthy, as well as mental<br />

aspects such as maintaining a positive<br />

mindset and knowing how to reduce<br />

stress. Unfortunately, a key component of<br />

healthy living for many in other countries<br />

– exercise – did not even feature as one<br />

of the top drivers of healthy living among<br />

Malaysian adults.<br />

adults who participated in the survey<br />

scored lower (68 points) than the<br />

regional average of 73 points when<br />

asked if they were satisfied with their<br />

health status.<br />

Unhealthy eating habits are hampering<br />

our efforts to lose weight and live healthy<br />

– 86% of those surveyed said they tend<br />

to eat when distracted with their screens,<br />

81% admitted to eating unhealthy snacks<br />

and 73% had their dinners late.<br />

Attempts to eat healthier are still restricted<br />

to the basics of drinking more water<br />

(73%) and eating more fruits and<br />

vegetables (60%).<br />

Obesity has surfaced as one of the<br />

outcomes of poor eating habits: six out<br />

of 10 Malaysians surveyed wanted<br />

to lose an average of 8.5kgs, which<br />

is considered substantial, placing<br />

Malaysian adults (69%) said they<br />

exercise regularly. They spend<br />

an average of 2.6 hours a week<br />

exercising – this is lower than the<br />

regional average of 3 hours and<br />

the minimum 3.5 hours a week<br />

recommended by most fitness experts.<br />

Interestingly, Internet and smartphone<br />

usage showed up as the barriers that<br />

are preventing Malaysians from getting<br />

enough exercise and sleep. On average,<br />

adults here spent an average of 4.1<br />

hours a day on the Internet for non-work<br />

related pursuits – one of the highest<br />

numbers of hours recorded in the region.<br />

This is a sharp contrast to the 2.6 hours a<br />

week Malaysians spend exercising.<br />

About 68% of respondents claimed that<br />

they are addicted to online activities,<br />

much higher than the regional average of<br />

57%. This addiction trend will continue<br />

to be fuelled by children growing up<br />

with the Internet and social media as<br />

integral aspects of their lives. Currently,<br />

50% of Malaysian parents claimed that<br />

their children had too much screen time<br />

– TV (44%), time spent online (31%) and<br />

playing video games (26%).<br />

It’s perhaps not surprising then that<br />

Malaysian adults are sleep deprived.<br />

While they wish to get 7.5 hours of<br />

sleep a night, in reality, they only enjoy<br />

6.3 hours – the lowest among the<br />

15countries surveyed.<br />

<strong>Health</strong> concerns rank<br />

high, but the majority<br />

don’t know the true state<br />

of their health.<br />

The majority of those surveyed said<br />

diseases related to the heart, diabetes,<br />

cancer and stroke among their top<br />

health concerns. Many were also<br />

worried about their mental well-being:<br />

76% were concerned about anxiety,<br />

74% about depression and 72% about<br />

Alzheimer’s/senility.<br />

Despite those concerns, only 46% of<br />

Malaysian adults had a medical checkup<br />

done in the last 12 months. This is<br />

lower than the regional average of 52%<br />

and a decline from previous years (52%<br />

in 2011 and 50% in 2013).<br />

There is a desire to do<br />

more, and they welcome<br />

guidance and motivation<br />

to sustain healthy<br />

behaviours.<br />

Many (91%) Malaysian adults admit that<br />

they can definitely do more to improve<br />

their health. It’s encouraging that 75%<br />

said they have taken some small steps in<br />

that direction. Only 15% confessed that<br />

they have not done anything to improve<br />

their health.<br />

What would motivate them and reinforce<br />

more healthy behaviours? Seeing<br />

tangible results: such as wanting to feel<br />

better physically (about 90%), mentally<br />

(88%) and look better (85%).<br />

Adults in Malaysia welcome guidance<br />

that would help them change one step<br />

at a time (84% said this); setting smaller<br />

and more achievable goals (79%);<br />

getting updates on their progress (76%)<br />

and earning rewards for achieving their<br />

goals (73%). For those who had not<br />

taken any steps towards healthy living,<br />

75% said not knowing what goals to<br />

work towards was the main obstacle,<br />

once again indicating the very real<br />

need for guidance.<br />

Launched in 2011, the AIA <strong>Health</strong>y Living<br />

Index is a survey conducted to understand<br />

how people feel about their health and the<br />

extent of their health habits, as well as their<br />

concerns and hopes for a healthier way of<br />

Steps to Protect Your<br />

Eyesight<br />

Have your eyes checked regularly by<br />

an eye care professional—either an<br />

ophthalmologist or optometrist. People<br />

over age 65 should have yearly dilated<br />

eye exams. During this exam, the eye<br />

care professional should put drops in<br />

your eyes that will widen (dilate) your<br />

pupils so that he or she can look at the<br />

back of each eye. This is the only way<br />

to find some common eye diseases that<br />

have no early signs or symptoms. If you<br />

wear glasses, your prescription should be<br />

checked, too.<br />

See your doctor regularly to check for<br />

diseases like diabetes and high blood<br />

pressure. These diseases can cause eye<br />

problems if not controlled or treated. See<br />

an eye care professional right away if you:<br />

• Suddenly cannot see or everything<br />

looks blurry<br />

• See flashes of light<br />

• Have eye pain<br />

• Experience double vision<br />

• Have redness or swelling of your eye<br />

or eyelid<br />

Protect your eyes from too much sunlight<br />

by wearing sunglasses that block<br />

ultraviolet (UV) radiation and a hat with a<br />

wide brim when you are outside. <strong>Health</strong>y<br />

Common Eye Problems<br />

The following common eye problems can<br />

be easily treated. But, sometimes they<br />

can be signs of more serious issues.<br />

• Presbyopia (prez-bee-OH-pee-uh) is a<br />

slow loss of ability to see close objects<br />

or small print. It is normal to have this<br />

problem as you get older. People with<br />

presbyopia often have headaches or<br />

strained, tired eyes. Reading glasses<br />

usually fix the problem.<br />

• Floaters are tiny specks or “cobwebs”<br />

that seem to float across your vision.<br />

You might see them in well-lit rooms<br />

or outdoors on a bright day. Floaters<br />

can be a normal part of ageing. But,<br />

sometimes they are a sign of a more<br />

serious eye problem such as retinal<br />

detachment. If you see many new<br />

floaters and/or flashes of light, see<br />

your eye care professional right away.<br />

• Tearing (or having too many tears)<br />

can come from being sensitive to<br />

light, wind, or temperature changes,<br />

or having a condition called dry eye.<br />

Wearing sunglasses may help. So<br />

might eye drops. Sometimes tearing is<br />

a sign of a more serious eye problem,<br />

like an infection or a blocked tear<br />

duct. Your eye care professional can<br />

treat these problems.<br />

• Eyelid problems can result from<br />

different diseases or conditions.<br />

Common eyelid problems include red<br />

and swollen eyelids, itching, tearing,<br />

and crusting of eyelashes during<br />

sleep. These problems may be caused<br />

by a condition called blepharitis<br />

(ble-fa-RI-tis) and treated with warm<br />

compresses and gentle eyelid scrubs.<br />

Eye Diseases and<br />

Disorders<br />

The following eye conditions can lead<br />

to vision loss and blindness. They may<br />

have few or no early symptoms. Regular<br />

eye exams are your best protection.<br />

If your eye care professional finds a<br />

problem early, there are often things you<br />

can do to keep your eyesight.<br />

• Cataracts are cloudy areas in the<br />

eye’s lens causing blurred or hazy<br />

vision. Some cataracts stay small<br />

and don’t change your eyesight a<br />

lot. Others become large and reduce<br />

vision. Cataract surgery can restore<br />

good vision. It is a safe and common<br />

treatment. If you have a cataract, your<br />

eye care professional will watch for<br />

changes over time to see if you would<br />

benefit from surgery.<br />

• Corneal diseases and conditions can<br />

cause redness, watery eyes, pain,<br />

problems with vision, or a halo effect<br />

of the vision (things appear to have an<br />

aura of light around them). Infection<br />

and injury are some of the things that<br />

can hurt the cornea. Some problems<br />

with the cornea are more common<br />

in older people. Treatment may be<br />

simple — for example, changing your<br />

eyeglass prescription or using eye<br />

drops. In severe cases, surgery may<br />

be needed.<br />

• Dry eye happens when tear glands<br />

don’t work well. You may feel itching,<br />

burning, or other discomfort. Dry eye<br />

is more common as people get older,<br />

especially for women. Your eye care<br />

professional may tell you to use a<br />

home humidifier, special eye drops<br />

(artificial tears), or ointments to treat<br />

dry eye.<br />

• Glaucoma often comes from too much<br />

fluid pressure inside the eye. If not<br />

treated, it can lead to vision loss and<br />

blindness. People with glaucoma often<br />

have no early symptoms or pain. You<br />

can protect yourself by having regular<br />

dilated eye exams. Glaucoma can be<br />

treated with prescription eye drops,<br />

lasers, or surgery.<br />

• Retinal disorders are a leading cause<br />

of blindness in the United States. Retinal<br />

disorders that affect aging eyes include:<br />

• Age-related macular degeneration<br />

(AMD). AMD can harm the sharp<br />

vision needed to see objects clearly<br />

and to do common things like driving<br />

and reading. During a dilated eye<br />

exam, your eye care professional<br />

will look for signs of AMD. There<br />

are treatments for AMD. If you<br />

have AMD, ask if special dietary<br />

supplements could lower your chance<br />

of it getting worse.<br />

• Diabetic retinopathy. This problem<br />

may occur if you have diabetes.<br />

Diabetic retinopathy develops slowly<br />

and often has no early warning signs.<br />

If you have diabetes, be sure to have<br />

a dilated eye exam at least once<br />

a year. Keeping your blood sugar<br />

under control can prevent diabetic<br />

retinopathy or slow its progress. Laser<br />

surgery can sometimes prevent it from<br />

getting worse.<br />

• Retinal detachment. THIS IS A<br />

MEDICAL EMERGENCY. When the<br />

retina separates from the back of the<br />

eye, it’s called retinal detachment. If<br />

you see new floaters or light flashes,<br />

or if it seems like a curtain has been<br />

pulled over your eye, go to your eye<br />

care professional right away. With<br />

surgery or laser treatment, doctors<br />

often can prevent loss of vision.<br />

Low Vision<br />

Low vision means you cannot fix your<br />

eyesight with glasses, contact lenses,<br />

medicine, or surgery. Low vision affects<br />

some people as they age. You may have<br />

low vision if you:<br />

• Can’t see well enough to do everyday<br />

tasks like reading, cooking, or sewing<br />

• Have difficulty recognizing the faces<br />

of your friends or family<br />

• Have trouble reading street signs<br />

• Find that lights don’t seem as bright<br />

If you have any of these problems,<br />

ask your eye care professional to<br />

test you for low vision. Special tools<br />

can help people with low vision to<br />

read, write, and manage daily tasks.<br />

These tools include large-print reading<br />

materials, magnifying aids, closed-circuit<br />

televisions, audio tapes, electronic<br />

reading machines, and computers with<br />

large print and a talking function.<br />

Other things that may help:<br />

• Change the type of lighting in your<br />

room.<br />

• Write with bold, black felt-tip markers.<br />

• Use paper with bold lines to help you<br />

write in a straight line.<br />

• Put colored tape on the edge of<br />

your steps to help you see them and<br />

prevent you from falling.<br />

• Install dark-coloured light switches<br />

and electrical outlets that you can see<br />

easily against light-coloured walls.<br />

• Use motion lights that turn on when<br />

you enter a room. These may help<br />

you avoid accidents caused by<br />

poor lighting.<br />

• Use telephones, clocks, and watches<br />

with large numbers; put large-print<br />

labels on the microwave and stove.<br />

Remember to ask your eye doctor if your<br />

vision is okay for safe driving.<br />

Celebrating Breast<br />

Cancer Survivors<br />

<strong>October</strong> is Breast Cancer Awareness Month, which<br />

is an annual campaign to increase awareness of<br />

the disease. While most people are aware of breast<br />

cancer, many forget to take the steps to have a<br />

plan to detect the disease in its early stages and<br />

encourage others to do the same. To motivate you<br />

to take action, we spoke to a two breast cancer<br />

survivors… and fighters. Hopefully their story will<br />

inspire you and encourage you to take action.<br />

When and how did you first find out<br />

you have breast cancer?<br />

I was first diagnosed with Stage 1 breast cancer in Nov<br />

2005 and had a lumpectomy on my right breast in Dec of the<br />

same year.<br />

What were your main concerns after<br />

being diagnosed?<br />

My main concern was whether I can afford the treatment,<br />

whether I can survive and who’s going to take care of my kids.<br />

How did you cope emotionally?<br />

When I was first diagnosed, I was shocked and shut myself out<br />

for a few days. I thought about my condition for three weeks<br />

and finally accepted my situation. Then I mustered up the<br />

courage to tell my family.<br />

How did your family react to the news?<br />

There were shocked and sad. My husband was calm as he<br />

needed to be strong for the family. My kids were too young<br />

to grasp the whole thing then. I think me being calm and<br />

accepting of my own condition helped them to understand and<br />

help reduce the stress and tension. Slowly we talked about<br />

what are the solutions we have to take as a family. Family<br />

support is really important.<br />

Is there a history of breast cancer in<br />

your family?<br />

After I was diagnosed, my uncle and sister also had cancer.<br />

Before that I was not aware of anybody in the family who<br />

had cancer.<br />

What kind of treatment did you<br />

undergo?<br />

After first diagnosed in 2005, I had six chemo sessions in<br />

I was diagnosed with breast cancer in 2011. It was diagnosed<br />

when I went for a medical check- up. We had a biopsy and<br />

fortunately it was discovered at an initial stage.<br />

being diagnosed?<br />

My main concern was, would I be a burden. I usually am the<br />

one taking care of people and I do not feel good to burden<br />

and have someone taking care of me.<br />

How did you cope emotionally?<br />

When I was first diagnosed, I was calm and trusted God.<br />

The doctor said it was an initial stage so the treatment is fairly<br />

straightforward. That helped put me at ease.<br />

How did your family react to the news?<br />

There were supportive as they were quite informed about<br />

cancer and its treatments.<br />

How Should A Breast Self-Exam Be Performed?<br />

1) In the Shower<br />

Using the pads of your fingers, move<br />

around your entire breast in a circular<br />

pattern moving from the outside to the<br />

center, checking the entire breast and<br />

armpit area. Check both breasts each<br />

month feeling for any lump, thickening,<br />

or hardened knot. Notice any changes<br />

and get lumps evaluated by your<br />

healthcare provider.<br />

2) In Front of a Mirror<br />

Visually inspect your breasts with your<br />

arms at your sides. Next, raise your arms<br />

high overhead.<br />

Look for any changes in the contour,<br />

any swelling, or dimpling of the skin, or<br />

changes in the nipples. Next, rest your<br />

palms on your hips and press firmly to<br />

flex your chest muscles. Left and right<br />

breasts will not exactly match—few<br />

women's breasts do, so look for any<br />

dimpling, puckering, or changes,<br />

particularly on one side.<br />

3) Lying Down<br />

When lying down, the breast tissue<br />

spreads out evenly along the chest wall.<br />

Place a pillow under your right shoulder<br />

and your right arm behind your head.<br />

Using your left hand, move the pads<br />

of your fingers around your right breast<br />

gently in small circular motions covering<br />

the entire breast area and armpit.<br />

Use light, medium, and firm pressure.<br />

Squeeze the nipple; check for discharge<br />

and lumps. Repeat these steps for your<br />

left breast.<br />

Zika, a generally mild disease identical<br />

to dengue, is a mosquito-borne flavivirus<br />

first identified in Uganda in 1947, with<br />

rising cases of the endemic reported<br />

in 70 countries and territories to date.<br />

The virus, which the World <strong>Health</strong><br />

Organisation calls a ‘global health<br />

emergency’, is said to be transmitted<br />

by the bite of an infected female<br />

AedesAegypti mosquito that bites in the<br />

day time 1 . While Brazil has recorded<br />

the highest case of 1 million in 2015,<br />

Singapore and Malaysia has reported<br />

41 and 3 cases of the outbreak<br />

respectively, with one case involving a<br />

four-month pregnant woman in Johor.<br />

Since Zika was first detected in Southeast<br />

Asia, online marketplace 11street has<br />

seen surge in search trends for mosquitorelated<br />

items, such as mosquito patch with<br />

a 517% increase, and mosquito bugslock<br />

at 349%. These warning signals are<br />

raising concerns about a potential rapid<br />

surge of the virus across Asian countries.<br />

As there are no vaccines and antiviral<br />

drugs to cure the disease, the Ministry<br />

of <strong>Health</strong> has urged Malaysians to take<br />

precautionary steps to reduce the risk of<br />

exposure to mosquitoes in their homes and<br />

protect themselves from Zika.<br />

Home Care Tips –<br />

Prevent Breeding of<br />

Aedes Mosquitoes<br />

Also known as a “container-breeding<br />

mosquito”, the AedesAegypti lays eggs<br />

in and around standing water. Hence,<br />

it is important that water in containers<br />

are removed and items that collect<br />

water such as old tyres, vases, pots<br />

and bottles are stowed away safely<br />

to prevent female mosquitoes from<br />

breeding. If water must be stored, ensure<br />

that containers are tightly covered to<br />

hinder mosquitoes from gaining access<br />

to water. As mosquito larvae stick around<br />

containers like glue, and can endure up<br />

to eight months in dry conditions, always<br />

empty and scrub clean containers once a<br />

week or whenever not in use.<br />

Water gardens and swimming pools<br />

are great to have, but are major<br />

mosquito producers if they are allowed<br />

to stagnate. If you have decorative<br />

ponds or a swimming pool at home,<br />

always aerate and clean the ponds<br />

and pools whenever possible to ensure<br />

that there are no surviving mosquito<br />

larvae. Other than that, always ensure<br />

that drains are properly managed,<br />

cleaned and free of trash and weeds<br />

so that they are not clogged.<br />

The most underrated piece of utility that<br />

protects you against mosquitoes and<br />

the diseases they carry are mosquito<br />

net window screen and mosquito net<br />

for when you sleep. These utilities are<br />

great preventive measures to disable the<br />

pesky insects from invading your home.<br />

If you have a baby at home, invest in a<br />

baby bed cushion that is equipped with<br />

mosquito tents for their afternoon naps or<br />

plug in an insect killer lamp. If you feel<br />

that having mosquito nets are a hassle to<br />

keep, you can always opt for air purifier<br />

with mosquito catcher.<br />

There are ample reasons to take every<br />

possible measure to avoid mosquito<br />

bites even without taking into account<br />

their terrible, stinging itchiness. What<br />

chemicals in our sweats.<br />

repellents for optimised efficacy. If the smell<br />

of the spray or patch is too strong for you,<br />

you can always opt for a more innovative<br />

It is highly advisable to postpone<br />

your travel plans to areas with Zika<br />

outbreaks. However, under unavoidable<br />

circumstances, it is imperative to consult<br />

your doctor or healthcare provider before<br />

you leave. Always equip yourself with<br />

a travel case of essential items such<br />

as mosquito repellent spray, mosquito<br />

repellent soap and mosquito bikit.<br />

Despite the hoopla on the virus, it should<br />

not deter you from enjoying the great<br />

outdoors without worrying about getting<br />

bitten. When you travel and venture<br />

outdoors for a day of sight-seeing and<br />

fun-times, always remember the two<br />

Ds – dress and defend. As mosquitoes<br />

are attracted to dark colours, opt for a<br />

more cheerful style with bright colour<br />

and thick-material clothes especially if<br />

you are doing a lot of cardio. Always<br />

remember to apply mosquito repellents<br />

and stick mosquito bikits to your<br />

clothings before stepping out of your<br />

home. From patches to sprays, bugslock<br />

to bikits, choices are aplenty for you to<br />

defend yourself against mosquitoes.<br />

Childhood Eczema vs<br />

Adulthood Eczema<br />

Eczema is a chronic inflammatory,<br />

relapsing and remitting skin disease that<br />

affects about 20% of children and 3%<br />

of adults. It is characterised by itching<br />

and scratching resulting in eczematous<br />

skin lesions. During the acute phase,<br />

the lesions are red, with blister<br />

formation and occasional oozing over<br />

a background of dry and scaly skin.<br />

However, persistent and chronic lesions<br />

tend to be associated with pigmentation<br />

and lichenification (thickened, hardened<br />

and rough skin texture).<br />

The most common type of eczema is<br />

atopic eczema, a.k.a. atopic dermatitis<br />

(AD) which usually affects children, but<br />

can also affect adults. AD affects about<br />

one in 5 children and about one in 12<br />

adults in the UK. In children, the onset<br />

often occurs between 3 and 6 months,<br />

with about 60% developing the condition<br />

before 1 year and 90% by 5 years<br />

of age. Most patients have personal<br />

or immediate family history of atopy<br />

(allergic rhinitis and asthma).<br />

AD in infants usually present with rashes<br />

over the face and extensor areas with<br />

an underlying generalised dry skin. They<br />

usually improve remarkably between the<br />

ages of 2-5 years.<br />

Childhood AD may follow infantile AD,<br />

or start between the age of 2 to 4 years.<br />

The rashes are often found in the creases<br />

of the neck, elbows, behind the knees<br />

and across the ankles. This form of AD<br />

usually improves with age. Although<br />

some children will outgrow their AD, most<br />

will continue to have dry and sensitive<br />

skin into adulthood.<br />

It is estimated that approximately 10%<br />

of adolescents continue to suffer from<br />

eczema into adulthood.<br />

Adult AD it is actually very similar to<br />

that of AD in older children. The areas<br />

of dry, itchy and reddened skin are<br />

predominantly at the neck, elbow<br />

creases, wrists, behind the knees and<br />

around the ankles.<br />

Although the terms eczema and<br />

dermatitis are generally regarded<br />

as synonymous, not all eczema can<br />

be classified as atopic dermatitis.<br />

Eczema can be broadly categorised as<br />

exogenous or endogenous.<br />

• Endogenous eczema – conditions<br />

which are not a result of exogenous<br />

or external environmental factors, i.e.<br />

is mediated by processes originating<br />

within the body, most commonly<br />

atopic dermatitis (AD).<br />

• Exogenous eczemas – conditions<br />

that are related to clearly defined<br />

external triggering factors, although<br />

inherited tendencies can also play<br />

a role. This group of disorder are<br />

broadly sub-categorised as:<br />

• Irritant contact dermatitis (ICD –<br />

occurs when chemicals or physical<br />

agents damage the surface of the<br />

skin faster than the skin is able to<br />

repair the damage. Irritants remove<br />

oils and moisture from the outer<br />

layer of the skin, allowing chemical<br />

irritants to penetrate more deeply<br />

and cause further damage by<br />

triggering inflammation.<br />

• Allergic contact dermatitis (ACD)<br />

– occurs when a material (allergen)<br />

comes in contact with the skin<br />

causing an allergic reaction. It arises<br />

some hours after contact with the<br />

allergen, and settles down over a<br />

few days, provided the allergen is<br />

removed from the skin.<br />

Signs to watch out for<br />

Pruritus is always intense, with significant<br />

effects on the activities of daily living,<br />

psychological disturbance and sleep. In<br />

adults, the disease affects different areas<br />

of the body compared to the infants. The<br />

hands, face and especially the eyelids<br />

are most often involved, in addition to<br />

skin creases and sometimes other areas.<br />

Inflammatory flare-ups can affect skin all<br />

over the body. This is called erythroderma.<br />

These widespread conditions are serious,<br />

and can become more complicated when<br />

infections and metabolic disorders develop.<br />

They may require hospitalisation with a<br />

combination of intravenous, oral, topical<br />

therapies with intensive skin nursing care.<br />

Intravenous therapy includes intravenous<br />

steroids with antibiotic. Oral therapy is<br />

mainly anti histamine for itch control,<br />

whereas topical treatment encompasses<br />

emollients, steroid and antibiotic.<br />

Why some suffer while<br />

others don't?<br />

The exact cause of AD is complicated<br />

and not very clearly defined. It arises<br />

because of a complex interaction of<br />

genetic and environmental factors.<br />

Two important hypotheses have<br />

been proposed with regards to the<br />

development of inflammation leading<br />

to AD.<br />

The first hypothesis suggests a primary<br />

immune dysfunction resulting in IgE<br />

sensitisation, while second hypothesis<br />

supports the theory of skin barrier<br />

disturbance.<br />

The primary immune dysfunction results<br />

in an imbalance in the T-cell subsets,<br />

leading to massive production of<br />

inflammatory cytokines, causing an<br />

increase in circulating IgE.<br />

The skin barrier dysfunction hypothesis<br />

suggests that AD patients develop AD as<br />

a result of skin barrier defects that allow<br />

for the entry of antigens, resulting in the<br />

production of inflammatory cytokines.<br />

Furthermore, filaggrin mutations are<br />

associated with early-onset AD. One<br />

mechanism by which filaggrin defects<br />

may influence inflammation is by the<br />

release inflammatory cytokines.<br />

How to alleviate the symptoms and<br />

remedies available:<br />

• Avoidance of triggers known to<br />

aggravate the disease severity e.g.<br />

• extremes of temperature<br />

• clothes that contain wool or other<br />

irritant fibres<br />

• harsh soaps and detergents<br />

• keeping long finger nails<br />

• contact with furry animals<br />

• Keep your skin hydrated at all times<br />

as eczema is a disease characterised<br />

by impaired skin barrier function.<br />

Therefore, there is increased transepidermal<br />

water loss resulting in<br />

constant dry skin. Emollients are<br />

the cornerstone therapy for eczema<br />

management.<br />

• As eczema is an inflammatory skin<br />

disease, topical steroids are important<br />

to reduce the inflammation and itch in<br />

addition to emollients.<br />

• Topical immunomodulatory agents,<br />

e.g. tacrolimus and pimecrolimus,<br />

are suitable alternatives to topical<br />

corticosteroids especially at creases,<br />

face and genitalia as these agents do<br />

not cause skin atrophy.<br />

• Anti-histamines are helpful for pruritus<br />

control and improve sleep at night.<br />

Treat acute flares of eczema rapidly:<br />

• Intermediate or high potency topical<br />

steroids for short periods of time to<br />

treat exacerbations.<br />

• Oral or intravenous antibiotics as skin<br />

infections with Staphylococcus aureus<br />

may be a recurrent problem.<br />

• Oral anti-viral therapy may be<br />

required if there is secondary viral skin<br />

infection e.g. herpes simplex virus.<br />

When to seek medical<br />

help?<br />

Patients diagnosed with eczema will<br />

be advised to be under the follow up<br />

with their dermatologists or general<br />

practitioner. This will improve long term<br />

care and monitor acute flares early<br />

during the course of disease.<br />

Medical help is needed at different<br />

stages of disease:<br />

Initial stage:<br />

• Patients and their family members<br />

should be educated about the<br />

chronic nature of eczema, and most<br />

importantly the non-contagious nature<br />

of this condition.<br />

• They should be aware of aggravating<br />

factors, and appropriate management<br />

to achieve effective control of their<br />

condition. This ensures cooperation<br />

and improved compliance which<br />

translates to better outcomes.<br />

Patients should be exposed to<br />

organisations and local support groups<br />

to receive up-to-date information about<br />

their condition.<br />

Acute flares:<br />

• Patients should be aware of the signs<br />

and symptoms of an impending flare<br />

of their condition.<br />

• They may accelerate their therapy<br />

during the initial stage, but they must<br />

not hesitate to get professional support<br />

when they are unable to control the<br />

progression of disease.<br />

• Different patients have different<br />

severity of disease and hence, therapy<br />

is catered for each individual patient<br />

Clinical scenario<br />

Madam Kim is a 75 years old lady<br />

who presented to a public hospital with<br />

a painful right hip after a trivial fall at<br />

home. She was diagnosed to have<br />

a fracture of the neck of right femur.<br />

She was then advised for a surgical<br />

procedure to fix the fracture bone which<br />

she agreed upon. However due to the<br />

long waiting list of at least two weeks in<br />

the public hospital before the surgery can<br />

be performed, Madam Kim opted for<br />

treatment in a private hospital instead.<br />

A partial hip replacement surgery was<br />

carried out and Madam Kim recovered<br />

well from the surgery soon after. She was<br />

prescribed with a medication to reduce<br />

the risk of recurrent fracture during her<br />

follow up appointment in which she was<br />

compliant for a brief period of time. She<br />

did not return for subsequent follow up<br />

and it is not known if she continues taking<br />

the medication prescribed.<br />

Scope of the problem<br />

Ageing population<br />

This is a common clinical scenario in<br />

our community as far as osteoporosis<br />

care and prevention is concerned.<br />

The word osteoporosis literally means<br />

“porous bones”. In simple term it means<br />

that the bone becomes brittle and<br />

weak, making it susceptible to fracture<br />

when trivial trauma or minimal force<br />

applied. Common osteoporotic fractures<br />

include neck of femur fracture, vertebral<br />

compression fracture and distal radius<br />

fracture. These fractures are commonly<br />

seen in our healthcare system as the<br />

society ages and the life expectancy of<br />

the population increases.<br />

A data provided by Asia Pacific<br />

Osteoporosis audit of Malaysia in 2013<br />

shows that over the next four decades,<br />

the number of Malaysian population<br />

aged over 70 years is expected to<br />

increase by 417%. This shows an<br />

increment in number from 939,010 in<br />

2013 to 4.8 million in 2050. The life<br />

expectancy will also increase seven-years<br />

longer than today, rising from 74 years<br />

to 81 years. The graph shown below<br />

(Figure 3) explains the rapid decline<br />

of bone density with age which is the<br />

hallmark feature of osteoporosis.<br />

This phenomenon is seen more markedly<br />

in women compared to men especially<br />

after the age of 50. The increased<br />

number of Malaysia’s greying population<br />

would mean osteoporosis and its<br />

complications would be a significant<br />

burden in our healthcare system. It is<br />

estimated that osteoporosis will result in<br />

1 in 3 women and 1 in 5 men to have<br />

fracture. In Malaysia the cost of fracture<br />

treatment is increasing. Some Malaysian<br />

data have shown that a cost of operative<br />

surgery for a hip fracture is around USD<br />

6000 in private healthcare. It is much<br />

cheaper in public hospitals but the<br />

waiting list may take around two weeks<br />

or more.<br />

Who else are at risk?<br />

Apart from increasing age there are also<br />

several other factors that contributes to<br />

the development of osteoporosis. These<br />

factors are further divided into two broad<br />

categories namely non-modifiable and<br />

modifiable factors.<br />

Non-modifiable factors<br />

1. Advancing age<br />

2. Ethnic group (Oriental & Caucasian)<br />

3. Female gender<br />

4. Premature menopause (< 45 years)<br />

5. Slender build<br />

6. Family history of osteoporosis in first<br />

degree relative<br />

Modifiable factors<br />

1. Low calcium intake<br />

2. Sedentary lifestyle<br />

3. Cigarette smoking<br />

4. Excessive alcohol intake<br />

5. Excessive caffeine intake<br />

What can be done?<br />

<strong>Health</strong> education<br />

As in any other significant health<br />

problems the key for successful<br />

intervention is to increase the awareness<br />

level of our community through health<br />

educations. <strong>Health</strong>care professionals<br />

too need to be aware of this as they<br />

are one of the key players in educating<br />

patients regarding osteoporosis. <strong>Health</strong><br />

campaigns at every level need to be<br />

done regularly by health authorities as<br />

well as non-governmental organisations.<br />

Public needs to be educated about<br />

osteoporosis in general, its risk factors,<br />

screening test available and prevention<br />

as well as treatment.<br />

Screening tests<br />

Bone mineral density (BMD) is a<br />

screening tool for osteoporosis. The<br />

hallmark of osteoporosis is when there<br />

is a significant reduction in BMD in<br />

certain areas of our body. This test can<br />

be easily done with various instruments<br />

and it is affordable as some centres<br />

provide this test for free. In general there<br />

are two main screening modalities used<br />

in Malaysia for BMD namely DEXA or<br />

dual energy X-ray absorptiometry and<br />

quantitative ultrasound. The former test is<br />

the gold standard and widely available<br />

in many hospitals and in some health<br />

screening centres in Malaysia. The<br />

ultrasound test is becoming popular<br />

as it is portable and can be used for<br />

screening in health promotion activities at<br />

shopping malls and work places.<br />

DEXA scan is used to screen those who<br />

are at risks. If the test results show that<br />

an individual has a significant low<br />

bone density, then he or she need to<br />

be started on treatment. DEXA scan is<br />

also used to monitor those individuals<br />

that are already on treatment; every<br />

1-2 years after treatment starts. QUS<br />

on the other hand does have a role in<br />

mass public screening but any abnormal<br />

test needs to be verified by a health<br />

professional most of the time together<br />

with a DEXA scan result.<br />

There are other tests such as biochemical<br />

blood test which measure the formation<br />

and resorption of bone but they are<br />

mainly for treatment monitoring and not<br />

for making a diagnosis of osteoporosis.<br />

Prevention of osteoporosis<br />

Many studies have shown that<br />

osteoporosis may be prevented and in<br />

those who have established osteoporosis,<br />

its complications such as fracture can be<br />

prevented by taking several steps.<br />

Calcium intake<br />

In osteoporosis there is a net loss of<br />

calcium from our body and evidence<br />

has shown that an adequate intake of<br />

calcium is positively correlated with good<br />

bone mass. What is the adequate level<br />

of calcium intake? It varies according to<br />

age, gender and physiological status as<br />

shown in the table nex page.<br />

osteoporotic patients is related to a fall<br />

as shown in the clinical case described<br />

above. There are various factors that<br />

can contribute to falling, such as poor<br />

vision, poor balance, weak or painful<br />

limbs and joints which may be difficult<br />

to control and need advices from health<br />

professionals for them to overcome.<br />

There are also factors which are can be<br />

easily managed and this is frequently<br />

missed by patients and their caregivers.<br />

Household hazardous environment such<br />

as slippery floor, inadequate lighting,<br />

steps, damaged carpets with curled<br />

edges or loose and unsecured floor rugs<br />

is associated with slipping and tripping.<br />

It is important that this preventable cause<br />

of fall is made aware to patients and<br />

their caregivers to reduce the risks of<br />

falling and possibly fractures.<br />

It is recommended that daily calcium<br />

intake is taken in the food form, however<br />

calcium supplement is strongly advised<br />

if daily calcium intake is inadequate.<br />

Calcium supplements come in various<br />

concentration therefore consultation with<br />

qualified professional is recommended.<br />

Vitamin D intake<br />

Vitamin D is an important element in the<br />

absorption of calcium from our gut. Those<br />

who are exposed to adequate sunlight<br />

(more than 15 minutes a day) should<br />

have adequate level of Vitamin D in their<br />

body but in individuals who are house<br />

bound, sedentary and so forth Vitamin D<br />

supplement is needed.<br />

Exercise<br />

Regular physical exercise is beneficial<br />

not only for cardiovascular health<br />

but in the prevention of osteoporosis<br />

too. Weight bearing exercise such<br />

as walking, hiking, jogging, climbing<br />

stairs and dancing is the best exercise<br />

for healthy bone. Needless to say<br />

any physical activity to be carried out<br />

need to discussed with relevant health<br />

professionals to avoid problems.<br />

Pharmacological treatment<br />

Other than calcium and vitamin<br />

D supplements there are specific<br />

medications that are used by doctors<br />

to prevent osteoporosis and reduce the<br />

risks of fractures in those patients with<br />

established osteoporosis. There are<br />

various types of medications can be<br />

used such as hormone replacement,<br />

selective oestrogen receptor modulators,<br />

biphosponates, strontium and parathyroid<br />

hormone. Studies have shown that<br />

medical treatment is cost-effective and<br />

recommended to those who are at risks<br />

of osteoporosis and its complications.<br />

TRINITY METHODIST CHURCH<br />

A Community Service Jointly Organised<br />

by <strong>Natural</strong> <strong>Health</strong><br />

It was an exciting Tuesday morning where a round of talks<br />

and presentation on age-related illnesses was organised by<br />

<strong>Natural</strong> <strong>Health</strong> at the Trinity Methodist Church, Petaling Jaya<br />

on 30th August. Speakers from Anion International and Life<br />

Factor Sdn Bhd presented ground breaking thoughts on health<br />

to the benefit of the participants. Among the key attractions<br />

of the health talk was the diet assessments conducted by the<br />

experts. The church goers, especially the senior ones, took<br />

the chance to get their health checked and consult the experts<br />

who were at hand to provide them with professional advice.<br />

Some insightful topics touched on at the event include:<br />

Immunotheraphy – presented by Vincent Low, a Director of<br />

Anion International which lasted for almost 45 minutes.<br />

Another interesting topic titled The Solution to Stress &<br />

Situational Anxiety by Life Factor Sdn Bhd was presented<br />

by Melvin Chung, a Nutritionist who has almost 10 years<br />

of experience in the nutrition and healthcare industry. He<br />

presented a brief introduction on stress and how it may affect<br />

our health.<br />

During the event a diet analysis, blood pressure test, weight<br />

management advise, BMI test and mental state examination<br />

were also provided for the participants.<br />

The participants also gave admirable feedback during the<br />

Q & A sessions and received free goodies bag from <strong>Natural</strong><br />

<strong>Health</strong> in return. Around 80 participants joined the talk and<br />

later enjoyed refreshments prepared.<br />

their wellbeing. Understanding that the solution starts<br />

and management of bad air quality available to the<br />

Fonterra Launches Its All New<br />

Anchor Food Professionals In<br />

Malaysia<br />

19 Aug – With the massive growth worldwide and<br />

busier lifestyles, more and more people are grabbing<br />

food on the go or dining out. Globally the Foodservices<br />

market is growing at a rate of 6% per year. Capitalising<br />

on the growth of the foodservices sector, dairy giant<br />

Fonterra from New Zealand, will be rolling out its new<br />

global foodservices brand, Anchor Food Professionals.<br />

The Anchor Food Professionals reflects the way the<br />

foodservice team are now doing business, working<br />

alongside customers to bring business and product<br />

solutions that answer questions such as improving<br />

profitability and reducing waste.<br />

In Malaysia the Anchor brand is already a leader in the<br />

dairy segment. When you dine in a restaurant, a hotel,<br />

or a quick service restaurant (QSR), chances are you<br />

Students Do Good For Children<br />

July 30 – Make U Sense recently collaborated with Malaysia Lysosomal Dis-eases Association<br />

(MLDA) and raised a grand total of RM1,075 to contribute their help to the rare disease,<br />

Lysosomal Storage Diseases (LSD).<br />

The final year Diploma students of IACT College created Make U Sense to do booth sales and<br />

mini carnival to raise funds and at the same time raise awareness towards the life threatening<br />

LSD which mostly affects children who often die at a young age, many within a few months<br />

or years of birth. Conducting the campaign as part of their final project for their Event<br />

Management and Public Relations Campaign Strategy class, the team of eight joined hands to<br />

help MLDA. According to Ir Lee Yee Seng, the President of MLDA, every LSD patient needs at<br />

least RM1,000,000.00 annually for their Enzyme Replacement Therapy (ERT). In order to help<br />

LSD pa-tients for their cost of ERT, MLDA come out with “RM1 Every Life Counts” campaign<br />

aiming to have 33,333 people to be a donor and donate with at least RM1 per day.<br />

20 Sept – This year, the VITAGEN Good Gut <strong>Health</strong><br />

Awareness campaign for schools will emphasise the<br />

importance of identifying the 5 signs of good gut health<br />

and the 5 simple steps to achieve it through interactive<br />

workshops at selected primary schools, nationwide. The<br />

campaign at Sekolah Kebangsaan Taman Tun Dr. Ismail<br />

(1) was a fun-filled session comprising an informative<br />

talk on the signs of good gut health, followed by the 5<br />

steps to achieve good gut health. Quizzes and games<br />

to reinforce what the children had learned earlier were<br />

next. A follow-along fitness session on abdominal<br />

exercises to strengthen the core proved to be a fun<br />

challenge for the kids.<br />

The session came to an end with the free distribution of<br />

the delight of the children. Children and teachers were<br />

educated on the 5 signs of good gut health which are<br />
