Growth and development are an
essential part of a child’s life. Every
parent or caregiver would love to
have their child living up to their
full potential of their growth and
development. Proper intake of nutrients
plays a significant role in moulding the
growth and development of a child.
Ideally, children get a major portion
of their nutrition from a balanced and
healthy meal.
The prevalence of malnutrition is
staggering globally. The World Health
Organization’s (WHO) database
indicates that 2.3 billion people suffer
from malnutrition. Out of that, 149
million children under five years of
age are categorised as too short for
their age (stunted). Another 45 million
children less than five years of age do
not weigh enough in proportion to
their height (wasted).
The Essential Vitamins
and Minerals for
Children
The essential vitamins required would
be Vitamins A, B (especially B12), C,
D, E, and K. Minerals like calcium,
magnesium, phosphate, zinc, iron,
fluoride, and iodine are also crucial. The
most common deficiencies for children
with no chronic illnesses are Vitamin D
and Iron.
Vitamin D is a fatsoluble
vitamin
that promotes
bone and tooth
formation.
Deficiency in
infants and
children usually
presents with
Rickets (bowing of legs). This vitamin
is commonly deficient in premature
babies and it is usually supplemented
in the oral form (daily minimum
requirement about 400 to 600 IU).
Good food sources of Vitamin D would
be milk and fatty fish like salmon and
mackerel. The best source of Vitamin
D is definitely sunlight, especially the
morning sun.

Every child needs vitamins for healthy growth and
development. Fortunately, most kids have no issues getting
all the nutrients they need when they eat a well-balanced
diet. In this article by Dr Sharmini Pillay Datuk Sivaraman,

discusses the essential vitamins for kids and how to ensure
they get the vitamins they need.

The mineral that goes
hand in hand with
Vitamin D is calcium.
Vitamin D helps with
Calcium absorption.
99 per cent of
calcium distribution
in the body is in the
bones and teeth. Good
food sources for calcium would
be milk, cheese, yoghurt, tofu and
calcium fortified orange juice. The
Recommended Dietary Allowance of
milk in a child is about 3 to 4 cups
daily; for infants, 5 to 6 ounces about 4
times a day. In severe deficiencies, the
paediatrician would supplement with
oral calcium supplements.
Vitamin A is also
a fat-soluble
vitamin that is
very important
in preventing
poor vision,
dry eyes
(xerophthalmia)
and blindness. This
vitamin also promotes
hair and skin growth; most importantly,
it is vital for good immune response
against infections like diarrhoea,
respiratory diseases and infectious
diseases like measles. Dietary sources
are usually sufficient. These include
milk, cheese, eggs, and yellow to
orange vegetables like carrots, yams
and squash (pumpkin). A paediatrician
would prescribe Vitamin A supplements
if it is severely deficient in a child’s diet.
Vitamin B comprises B-1 (Thiamine),
B-2 (Riboflavin), B-3 (Niacin), B-5,
B-6 (pyridoxine), B-7, B-9 (Folic Acid)
and B-12 (Cobalamin). These group
of vitamins are very important to aid
body metabolism, energy production,
healthy circulatory and nervous system.
A deficiency especially in vitamin
B-12 component results in reduction
of cognitive growth and causes
developmental delay in children. Food
rich in B-12 are meat, chicken, fish,
nuts and in a lesser amount, eggs, milk,
cheese, beans and soy-beans. Children
on vegetarian diets would need B12
fortified food like cereals or bread
spreads. Supplementation might be
needed in rare circumstances.
Vitamin C plays an
important role
in children’s
growth,
development
and good
immunity. It is
an important
antioxidant that
helps to combat
illnesses and toxins. This vitamin also
helps to improve iron absorption
from the intestines. There are a
variety of colourful fruits high in
Vitamin C that would attract kids. For
example, oranges, kiwi, grapefruit,
strawberries, raspberries and
vegetables like tomatoes, cabbage,
cauliflower and Brussels-sprout. If a
child has difficulty taking the above,
supplements may be needed.
Zinc is an important mineral that
works hand in hand with Vitamin
C and is responsible for good brain
development and maintaining good
immunity. Studies show that adequate
zinc with adequate vitamin C reduced
the incidence of diarrhoea and
respiratory infections in children under
five. Food that contains zinc would
be legume, nuts, whole grain bread,
meat, eggs and seafood. There are also
supplements in the market specially
formulated in syrup form that combine
Vitamin C and Zinc. Please consult your
paediatrician before getting them over
the counter.
Iron is a mineral that
is essential for
cell metabolism
and to produce
healthy red
blood cells. Iron
deficiency (iron
deficiency anaemia)
occurs in infants who
are formula and breast fed, especially
after they start weaning to solid food
at six months of age. This is primarily
due to solely relying on milk as the
staple diet instead of a balanced meal.
Premature babies are liable to iron
deficiencies as well. Iron-rich foods
are beef, red meat, chicken liver,
spinach, beans, beetroot and prunes.
Supplementation with Syrup Ferrous
would be given at the discretion of
your paediatrician if needed. Vitamin C
rich food or supplement is needed for
good intestinal iron absorption.
Vitamin K, also a
fat-soluble vitamin
is a very important
vitamin given at
birth to prevent
bleeding disorders.
Subsequently, after
the weaning period
food rich in vitamin
K is important, for
example, green
leafy vegetables like spinach, kale,
lettuce, brussels sprout, broccoli and
fruits like prunes, kiwi, avocado, berries
and pomegranate.

As for vitamin E, it is an equally
important antioxidant for good
immunity and skin growth. Food rich in
vitamin E would be almonds, peanuts,
spinach and avocado.
Iodine is needed for formation of
thyroid hormones and is the most
preventable cause of low IQ and
slow development in children. WHO
estimates 31.5% of children globally
have iodine deficiency. Food rich in
iodine are mainly animal protein (beef
liver and chicken), fish and other
seafood, table salts (iodised ones),
dairy, eggs and seaweed. Besides that,
iodine is also found in fortified bread,
cereals and milk.
As for fluoride, a good toothpaste
with adequate fluoride is important
to maintain good dental hygiene
in children and prevent cavities,
while phosphate is mainly found in
dairy products, chicken and turkey.
Magnesium-rich food such as soy
products, dairy milk, legumes, nuts,
whole-grain breads and cereals are
also recommended.
When Do Kids Need
Supplements?
One common question asked is
which group of children need vitamin
supplements and at what age to start
them. In an ideal situation, a wellbalanced
meal is all that is necessary
for most children born full-term with
no existing medical conditions. Having
said that, in this time and age where
parents and caregivers are occupied
with work, time constraints often get in
the way of preparing a balanced meal
for their children. In such a situation,
the introduction of multivitamins may
be necessary.
Usually, taking vitamin supplements
would be recommended to the
following group:
• Picky eaters whose growth
parameters are not going according
to the desired growth potential
• Premature babies
• Children with chronic illnesses, for
example, respiratory problems (like
asthma, frequent lung infections),
immunodeficiency, malabsorption
disorders and, amongst others, food
or milk intolerance and allergy
• Children on a vegetarian diet
for cultural or religious reasons
(would need iron and vitamin B
supplementation specifically)
The recommended age for starting
vitamins has been advised for children
after four years of age, but for the
above-stated reasons, paediatricians
may recommend the starting of
multivitamins at an earlier age.
Too much of anything is bad, even
with food, multivitamins and minerals
– for instance, overconsuming vitamin
supplements and minerals can also
cause toxicity. Thus, it is important
to strike a balance so that every child
receives appropriate amounts of
minerals and vitamins in their daily
diet. Proper nutrition still wins over
supplements, so consult your child’s
nutritionist and paediatrician for the
best outcome for your child.

Understanding Breastfeeding and Its Benefits
According to Dr Balkees, breastfeeding is a normal physiological human activity with countless benefits for the
mother, child, family, society, and the environment. In the Q&A below, she offers comprehensive insights into how
breastfeeding and breast milk can benefit both the mom and baby.
How does breastfeeding impact a
child’s health?
Dr Balkees: Breast milk is uniquely
suited to the infant’s nutritional needs
and is a life substance with unparalleled
immunological and anti-inflammatory
properties that protect against various
illnesses and diseases. Colostrum, or
the “first golden milk”, gives newborns
an immunity boost while their immune
system is developing and supports
lifelong health. Most babies will get all
the nutrition they need through their
mother’s colostrum during the first few
days of life.
The benefits of breastfeeding have
been found to continue long beyond
infancy, even into adulthood. Children
who have been breastfed have a lower
chance of becoming obese, overweight
and developing type 2 diabetes
as adults, besides having a higher
intelligence quotient. In addition,
breastfed infants also experience lower
mean blood pressure later in life, which
reduces the risk of heart disease.
What are the advantages of nursing
for mothers?
Dr Balkees: Breastfeeding also has
protective effects for the mother,
including decreased risk of postpartum
haemorrhage, postpartum depression,
heart disease and breast and ovarian
cancer. It is also associated with a lower
maternal risk of hospitalisation and
mortality due to cardiovascular disease
in middle-aged and older women.
It may offer long-term benefits for
maternal cardiovascular health.
Can you talk about how
breastfeeding can help a mother
and child bond?
Dr Balkees: Breastfeeding is not
only excellent for your child’s physical
development as it also nurtures the
mother and baby emotionally and
strengthens the bonding between
them due to the increased skin-toskin
contact while nursing. Nursing a
child leads to the release of two very
important hormones in the mother’s
brain: prolactin and oxytocin. Prolactin
(the mothering hormone) facilitates
breastmilk production and helps
moms relax and feel more affectionate
toward their babies. Oxytocin (the
love hormone) is responsible for milk
ejection, the euphoric feelings in the
mother, and lifting her mood.
What are the key stages of
breastfeeding?
Dr Balkees: For the first six months,
breast milk is the only food and
drink an infant needs. After six
months, a baby will require other
types of food in addition to breast
milk to ensure healthy growth and
development. The transition from
exclusive breastfeeding to family
food is referred to as complementary
feeding and typically covers the period
from six to 18 or 24 months of the
babies’ age. Breastfeeding should be
continued until two years of age and
beyond. This is because the first two
years of life provide a critical window
of opportunity to ensure a child’s
appropriate growth and development
through optimal feeding.

Essentials for Breastfeeding
In this section, let’s explore what a nursing mom would need during different stages of breastfeeding.
Nursing accessories for
breastfeeding newborns
The first few days with your baby are
usually quite hectic, so preparation is
vital. Whether you’re still in the hospital
or you’ve returned home, these items
will provide you with adequate support:
• Nursing bras, night bras, and
nursing vest tops
• Nursing nightdresses or pyjamas
• A breastfeeding pillow
• Disposable or washable nursing pads
• Muslin cloths
• Nipple cream for sore nipples
• Nipple formers if you have flat or
inverted nipples
In addition, you can also opt for these
items if the need arises.
• Nipple shields if your baby
struggles to suckle or if you’re
suffering from sore nipples. In
general, nipple shields are a shortterm
solution. If pain persists,
consult your lactation consultant or
breastfeeding specialist.
• A breast pump to relieve
engorgement or help establish your
milk supply.
• Hydrogel pads can provide soothing
effects for some mothers.
If you still encounter problems or
things don’t quite go to plan, contact
a lactation consultant or breastfeeding
specialist for further advice.
Nursing equipment in the weeks
that follow
As the first couple of weeks progress,
you and your child should be getting
more accustomed to one another.
The number of feeds can be frequent
and time-consuming. Here are a few
things to make life easier and more
comfortable as you progress:
• Nursing chair so that you and your
child can feel more relaxed
• Use a breastfeeding app to keep
track of meals
• Have a steady supply of disposable
or washable nursing pads
Baby feeding accessories for
producing milk
There will come a time when you may
want to supply or store more milk
for your baby, which means you will
require a breast pump. The type of
pump you’ll need depends on your
situation and how much milk you want
to produce.
Apart from the pump, you may also
need:
• A steam or cold-water sanitiser
• Microwave sanitising bags and
cooler bags
• Breast milk storage bags
• A hands-free pumping bra

Dr Balkees recommends these storage
containers to store breast milk properly:
• Hard-sided containers: Usually made
from hard plastic or glass, these are
the preferred containers for longterm
human milk storage. Choose
one with an airtight seal.
• Plastic bags (specifically designed
for breast milk storage: Only use
this option for short-term use as
they may spill, leak, or become
contaminated more easily than hardsided
containers.
“Wash your hands before expressing
milk. Store in small portions (60-120
ml) to minimise waste.
Consider storing smaller portions (30-
60 ml) each for unexpected situations.
A small amount of milk can keep a
baby happy until the mom comes to
nurse the baby. You may combine
breast milk expressed throughout the
day to get the desired volume in a
container. Chill the newly expressed
milk for at least 1 hour in the main
body of the refrigerator or in a cooler
with ice or ice packs, and then add it to
previously chilled milk expressed on the
same day. Do not add warm breast milk
to frozen milk because it will partially
thaw the frozen milk. Also, do not mix
milk from other days. Leave some room
at the top of a container because breast
milk expands as it freezes. Also, label
containers clearly with the date using
waterproof labels and ink, if possible,”
Dr Balkees advises.
Take Note!
Milk will “separate” during
storage. The top of the milk
(the cream) will look thicker
and whiter. Gently swirl the
container to mix the separated
milk but avoid vigorously
shaking the container.

Can you tell us what you have been
busy with recently?
I’ve been pretty busy with my restaurant (Restoran Rebung
Chef Ismail) and my role as a fellow researcher and lecturer
at UKM. As the Founder of Afyaa, I have also been sharing
health-related insights with the public. Work aside, my kids
are my priority. Thus, my wife and I try to spend as much time
as possible with them. We enjoy watching them grow up and
helping to bring out the best in them. Both of us are doctors,
so it’s been quite hard to juggle our time. However, we do
our best as we do not want to miss out on crucial parts of the
parenting journey.
Your professional journey has been
incredibly inspiring. Can you share
what led you to transition from one
career path to another?
I always believe in trying new things. Once you’ve succeeded
in something, you should try a different field. For me, once
I’ve achieved my goal of being a doctor, I decided to pursue
my dream of becoming an astronaut. Now, I’m transitioning
into parenthood and parenting. Always pushing yourself and
trying new things are what truly inspire me to keep going. I
do not live in my comfort zone. My next target is to become a
pilot in two years and work in Africa.
Let’s talk about your passion as a
parent. Tell us more about how you
raise your kids. For example, what
do you do to build a stronger bond
with them?
I encourage my kids to try different activities such as
swimming, taekwondo, music, and so on. Guiding them
to do their very best is important. You also need to be
knowledgeable about parenting, and you need to have
plans. From short-term goals to long-term goals, these will
help the parenting process. I think if your relationship with
them is built on a solid foundation, you can ensure a strong
bond with them even when they grow older. Even though
I’m very strict when it comes to their education, I still
encourage open communication.

In hindsight, my kids are doing very well. The eldest is Bella,
who’s 10, and the second is Tasha, who’s 9. We have two
pairs of twins: 5-year-olds and 1-year-olds. They all have
different personalities – this can be challenging, but we
constantly try to be there and support them physically,
mentally, and psychologically.
In what manner would you say that
your children have changed you?
They’ve undoubtedly changed my outlook on life. They are
my everything now, and my life revolves around what they
do. For example, I’ll make sure I am home when they come
back from school. My schedules are often planned around
them. My children and my family always come first.
Coming back to your success in the
professional field, can you share
more about your role as Afyaa’s
FOUNDER?
Afyaa is a functional food product that anyone can use daily.
It is doing pretty well in the market, and I am proud of the
positive impact it has left on our target audience. “Sihat
Nikmat Terindah” has always been our tagline, and this is
ultimately what we want to convey to the public. Health is
the most important thing in our life, and I want everyone to
know how this product can help us lead a healthier lifestyle.
I would love to expand Afyaa. We are now in four different
countries and we have launched a new product called
Blacktide in Indonesia. Currently, the three products we
have in Malaysia are Ryndu, Hayyiba and Sahiyya. Moving
forward, we are looking to produce three more products.
We are also planning to come out with products for kids in
the near future.
Now that we’re heading towards
the “new normal” way of life, what is
your advice for parents?
Don’t take things for granted. We may think that the virus
is not as dangerous as it used to be, but it is still spreading.
Even though we are in the endemic phase, we still need to be
cautious. What’s best is to get vaccinated, not just against the
COVID-19 virus but also to prevent other diseases. A healthy
diet is also essential instead of just relying on medication
when you’re sick. Simply put, prevention is better than cure.
What is your favourite quote/motto?
“What the mind believes, the body achieves.”

Physiological changes during pregnancy can
affect posture. As a pregnant woman’s body
weight increases by 15-25%, this will cause a
significant burden on the tendons, ligaments, and

womb and increased breast volume will also
shift the body’s centre of gravity to the front.
Furthermore, the pelvis is tilted simultaneously,
and along with increased lumbar lordosis, her
posture can be affected.

to keep the spine in proper alignment.
This will improve blood flow and nerve
function as well as support the muscles,
ligaments, and tendons.
Good vs Bad Posture
Women with a habit of using correct
posture are less likely to experience
related back and neck pain when she is
pregnant. When we talk about posture,
it refers to the position in which we
hold our body while standing, sitting,
or lying down.
Good posture means ensuring these
positions would place the least
strain on our muscles and ligaments,
especially while moving or performing
weight-bearing activities. The key
to good posture is making sure the
position of the spine is in its normal
and natural curvature.
On the flip side, a bad posture is
when our spine is in an unnatural
position, in which the curves are
overemphasised, resulting in pressure
or strain to muscles, joints and
ligaments. Prolonged bad posture may
result in build-up stress on these tissues
and cause long-term complications.

Pregnant women should
always maintain a pelvic

forward. Contract the
abdominal muscles and
buttock to act as a natural

with the shoulders.
2. Sitting
An incorrect sitting position in pregnant
women can lead to backaches.
Maintain a pelvic tilt and avoid
slouching when sitting. Sit up with your
back straight and your shoulders back.
Buttocks should touch the back of the
chair to distribute your body weight
evenly on both hips. Ideally, you should
sit back on a chair with the length of
the thigh supported by the seat. The
knees should be at the same level or
slightly lower than the hips, while the
feet should touch the floor. Do not
cross your legs while sitting, as this can
impact blood circulation.
3. Lying/sleeping down
on the side
Lying or sleeping on the side is an
excellent position to take the stress off
the spine. This position facilitates better
blood flow to the placenta, giving
adequate circulation to the baby in
the uterus. To maintain a good spinal
posture when lying down on the side,
place a pillow between the legs to
support the weight of the top leg and
decrease lower back strain. You can
also put a pillow under the abdomen to
support the weight of the womb.
4. Lying/sleeping down
on the back
Avoid lying or sleeping on your back
as the enlarged uterus will compress
the large blood vessels and reduce
blood flow and oxygen to the placenta
and baby. This position has been
associated with the risk of stillbirth
in late pregnancy. It can also increase
unpleasant symptoms such as dizziness
and heartburn. However, if you need to
lie on the back, elevate the head and
place a pillow under your knees. Keep
the knees bent to help tilt the pelvis
and flatten the back.
5. Lifting or bending
things
Pregnancy hormones often soften
ligaments and joints, making pregnant
women prone to back strain. Thus,
bending down can potentially injure
your muscles. To pick up an object that
is lower than the level of your waist,
keep the back straight and bend at your
knees and hips. Do not bend forward
at the waist with your knees straight.
Stand with a wide stance close to the
object you are trying to pick up and
keep your feet firm on the ground. To
lower the object, tighten your stomach
muscles and bend your hips and knees.
Remember not to lean forward.
Loosened ligaments, increased weight
and changes in a pregnant body can
lead to discomfort in the back and
other parts of the body. If you don’t
practise proper muscular support, it
can even cause prolonged pain and
other ailments in the long run. Always
pay attention to how your body feels
and adjust how you move or position
yourself. If you are unsure what to do
or have been feeling any pain in your
body, consult your ob-gyn.

Establish a diaper
system
Some parents simply split their
diapering patrol between night and
day, while some fathers take on the
task anytime they’re home when it
happens. Find a system that works
for your and your spouse. A standard
diapering routine generally comprises
these steps:
1. Unsnap onesie.
2. Undo diaper.
3. Wipe excess away using a diaper.
4. Clean with a wet wipe.
5. Wrap everything to dispose of in the
diaper and discard it.
6. Apply cream.
7. Put on a new diaper and dress
your baby.

There are variations in a diapering
routine based on skin care regimen and
diaper type. Nevertheless, it is important
to develop a consistent routine and do it
the same way every time.
Choose the right
diaper size
Disposable nappies are preferred as
they offer various diaper sizes to better
fit a child as they grow. Some kids
might need a larger diaper earlier, or
they might stay in the same size longer
than expected. Parents will need to
experiment a bit until they find the
right brand and size.
As a rule of thumb, diapers should
accommodate a kid’s waist with plenty
for the tabs to grip on to. When
fastened correctly, parents should be
able to fit a finger between the waist
and the baby without too much effort.
Ensure the diapers are snug around
the legs.
Leg ruffles are
essential
Even if they look cute on your little one,
the ruffles around diaper leg holes are
not a fashion element – they are there
to keep poop and pee inside the diaper,
preventing them from leaking down
the legs. Ensure the leg ruffles are
visible on the outside and not tucked
inside the diaper around the legs.
Keep your supplies
ready
The key to changing a kid without fuss,
whether inside the home or outside the
home, is having all the supplies on hand
and easily accessible. Nothing makes
baby-changing more awful than having
to stop in the middle to search for wipes.
Be cautious when using
a changing table
While changing tables ensure that an
adult doesn’t have to kneel and stoop
to change a kid, there’s a huge risk
to it: elevating the baby to a height
could potentially lead to a disastrous
fall. One hand should always be kept
on the kid if a belt isn’t available. If
the belt is available, use it, but remain
super vigilant.
Clean private areas

For a baby girl, clean her vulva by
wiping front to back. The idea is to
remove excrement from the vulva,
which will help prevent urinary tract
infections. For little boys, you can just
wipe their penises with disposable wet
wipes or cotton balls. Do not try to pull
the foreskin if they are uncircumcised.
On the other hand, great care should
also be taken when wiping a freshly
circumcised penis.
Here are some essential items for
thorough cleaning
• Cotton balls or soft disposable
towels with warm water are the
preferred cleansing method
• A pH-neutral cleanser may be used if
stools are dry and difficult to remove
• Wipes should be avoided (if
required, they should be free from
alcohol and fragrance)
• Barrier creams should be used on all
neonates at risk of perineal dermatitis
at every nappy change at the first
sign of erythema or skin breakdown.
Dealing with diaper rash
Diaper rash can turn nappy change into
a nightmare. To reduce the discomfort
of your baby’s irritated skin, dab rather
than wipe the skin. You can also
minimise further contact by cleaning
the area with water via a spray bottle
filled with warm water. Make sure that
the baby’s skin is thoroughly dabbed
dry after cleaning.
Diaper rash is very much about
controlling moisture. Fortunately, plenty
of creams and lotions can help manage
the discomfort. Apply an appropriate
amount of barrier cream and leave it on.
Removing the cream between nappy
changes is not necessary. You can just
add another layer of cream to the one
before. Avoid barrier creams containing
plant extracts or fragrances as they can
exacerbate the rash. If the rash persists
or worsens even after using creams,
seek a paediatrician’s opinion.

Bayer Collaborates with DoctorOnCall and MyEndosis to Launch
Telehealth Platform for Endometriosis
Bayer Co. (Malaysia) Sdn Bhd recently launched a telehealth platform dedicated to
endometriosis, in collaboration with Endometriosis Association of Malaysia (MyEndosis) and
DoctorOnCall, a digital health platform by Health Digital Technologies Sdn. Bhd. The launch
also kickstarts a 3-month campaign that will run from June, which enables women to seek
advice for their endometriosis condition through free teleconsultations with obstetriciangynaecologists
(OB-GYN) on the DoctorOnCall platform.
Through the dedicated telehealth platform, first-time site visitors or women who have no prior
knowledge or consultations with OB-GYNs on endometriosis are encouraged to undertake an
endometriosis risk analysis by answering 25 simple quiz questions on the platform. During the 3-month campaign period,
the first teleconsultation with the OB-GYN will be free of charge. If necessary, the consulting OB-GYN can subsequently
propose a physical check-up to clinically diagnose that patient as having endometriosis. There are 10 OB-GYNs from various
hospitals on the DoctorOnCall’s panel who can provide teleconsultation on endometriosis.
