When exactly does learning really

start for a newborn? Does it start

after the baby is born, or does it start

much earlier than that? According to

numerous research on learning and

brain sciences across the globe, babies

begin learning when they’re still

little foetuses in their mother’s belly.

The anticipation and delirious

excitement of a new addition to the

family is indescribable. The sheer

thought of having a mini-you waddling

around the house with that infectious

baby giggle and heart-melting babble

fills your heart with overwhelming

joy – enough to have you reaching

out to your unborn way before he

or she is birthed into the world.

Think headphones (playing timeless

compositions of Beethoven, no less)

strapped to a growing belly … or

an overzealous daddy whispering

words of eternal love while swapping

“state secrets” at momma’s bump.

Now, can babies in utero really hear

the sweet, caressing notes of Für Elise?

Well, studies have shown that they

are able to make out sounds from

within mom’s body (her heartbeat, for

example) at around 4 months, and

by the time they’re between 6 and

7 months, an unborn baby can hear

external sounds, such as voices, songs

and music.

As they mature in their mother’s womb,

foetuses experience various sensations

as their senses begin to develop.

Here’s a breakdown of the sensory

development of your precious little one,

through the trimesters/months.

First Trimester (Months

1 to 3 / Weeks 4 to 12)

A foetus is fully formed by the end of

the third month, and starts exploring

a little bit, eg moving its mouth and

fist. The organs and limbs are present,

including fingernails and toenails, as

well as the external ears. These will

continue to develop in order to become

functional.

Month 1

Second Trimester (Months 4 to 6 / Weeks 16 to 24)

By around 18 weeks, foetuses become sensitive to sounds. They can hear the

various sounds occuring in the mother’s body; they can feel the air going in and out

of the lungs, the gurgles in the stomach. At this point, it’s all internal sounds.

As they continue to develop, they become conscious of external sounds. The

foetus is able to hear the mother’s voice. However, they cannot hear the sound of

the father clearly. When the mother speaks, the sound moves through her entire

body, making it easier for the foetus to hear it, and becomes comfortable with that

particular voice.

However, when the father speaks, the external sound has to travel through the

abdominal tissue of the mother, pass through the amniotic fluid and then reach the

foetus. This is why sounds from the external world are highly muffled for the foetus.

The first little wiggles and squirms occur around 16 to 22 weeks. The mother

might not feel anything. At around 24 weeks, babies in the womb begin to

explore the lower part of their faces with their tiny fingers, and learn to touch

their lips and cheeks.

While taste buds begin to form around 8 weeks, by around 14 to 15 weeks they

develop enough to enable the foetus to identify and differentiate flavours.

So, if the mother has garlic on a particular day and there are traces of that flavour

in the amniotic fluid, the baby swallows it. The next day, if it is honey and maple

syrup, the baby takes an extra gulp of the amniotic fluid if they like it!

The findings of a study by Julie Mennella et al, published in PubMed in June 2001,

suggest that a mom's diet during pregnancy can influence a baby's food preferences

later. Therefore, expectant mothers are encouraged to watch what they eat.

Third Trimester (Months 7 to 9 / Weeks 25 to 36)

By the time a foetus has spent seven months in the womb, it acquires a sense of taste and smell. Whatever the mother

consumes also reaches the foetus in some mild form. Since the foetus is surrounded by amniotic fluid, flavours tend to find their

way into the fluid, which is commonly swallowed by the foetus.

In the last month, the baby practices motor skills and kicks. When the mother laughs, eats sweets or drinks an iced tea, the baby

responds by bouncing up and down.

By the time they are 28 weeks, they start with their strong little kicks, telling mommy that they are alive and well and learning

how to use their bodies.

A study by scientists at the University of Dundee, published in PLOS ONE in June 2015, showed that foetuses between the 21st

and 23rd week of gestation moved around and touched themselves in response to their mothers stroking the bellies.

According to an article titled ‘Foetuses may express emotion in the womb’ by James Meikle, published in Fairfax Digital in

September 2003, “images published for the first time seem to suggest that unborn babies can smile, blink and cry weeks before

they leave the womb.”

Yes, that's quite true. And today, we've come even further, thanks to 4D technology. Ultrasound scans have captured varied

emotions on a foetus' face – crying, squirming, smiling, wrinkling the nose, raising a brow and so on. Often, these are reactions

to what the mother may feel or do. If she is angry, upset or under stress, it can certainly reflect on the little baby.

How to prepare and

train babies in the womb

There are no hard and fast rules but

a general guide would help you feel

more in control and help you bond with

your baby.

Enjoy the sunshine

Go outside and soak in the sun. Add

foods rich in Vitamin D to your diet.

Autism and weak bones have been

linked to Vitamin D deficiency. A mere 20

minutes in the sun helps your little one

get strong bones and a healthy heart.

Start reading out loud to your baby

Researchers found that regular

reading triggered a preference for

hearing stories out loud which was

demonstrated by babies increasing their

sucking speed once they were born.

Exercise

Exercise helps blood circulation in your

body and your womb. It increases

neuron cell growth in your baby’s

hippocampus. This triggers learning and

memory development by 40 percent.

Touch your baby bump

Your baby will be able to feel the

sensations of touch and caress.

Stroking your bump gently sends

calming signals to your baby’s nervous

system, and this is a good time to take

advantage of that and make your baby

feel loved. It is said that babies can

distinguish between a mother’s and

father’s touch!

Talk to your baby

From story time sessions to just

talking to the baby about your daily

experiences helps stimulate your

baby. Your little one pays attention to

every word you send to your womb,

especially from week 27 when their

ear-brain connections come in place.

This is the perfect time to chat with

your baby!

Parents' Responsibility for the Baby in the Womb

Both the mother and father have to assume responsibility for the wellbeing

of the child growing in the womb of the mother.

1. As soon as the mother conceives, both parents should decide on the

designated gynaecologist and go for prenatal check-ups without

fail. It is encouraged that the father accompanies the mother; when

the father has a good attitude towards the pregnancy, it impacts

the mother in a healthy way. Caring for the baby and seeing it in

ultrasounds help parents build a connection and bond easily with the

baby growing within.

2. Both parents should pick up good habits – start a healthy diet, take

vitamins and start simple exercises like walking to keep fit and happy.

Quit smoking and alcohol intake.

3. Go for prenatal classes and get as much information as you can about

the physical and mental changes that occur during pregnancy. Men

also require psychological support as this is a time of uncertainty and

natural fear.

4. Prepare for the arrival of the baby. Together you can decorate the

baby’s room, buy baby clothes and most of all, determine the best

hospital for you and the baby.

Remember, just like motherhood, fatherhood also begins at conception.

Eat right

The best way to enhance your baby’s

growth and development is to nurture

it from inside the womb. Don’t force

yourself or be too rigid. Take one baby

step at a time and enjoy the process.

An Insight Into

Hand, Foot and

Mouth Disease (HFMD)

The Hand, Foot and Mouth Disease (HFMD) is considered an endemic disease in Malaysia as

it tends to manifest sporadically in mass outbreaks, predominantly affecting young children.

This year alone, between January 1 and June 18, a total of 106,477 cases have been reported,

marking a 39-fold increase compared with the same period in 2021 which

recorded only 2,710 cases.

In this segment, we slide HFMD under the microscope as Dr Melanie Majaham,

Consultant Paediatrician – Sunway Medical Centre Velocity sheds light on the disease,

its symptoms, treatment and prevention methods.

Who are susceptible?

Young children below age 5 are usually

more prone to contracting HFMD.

However, adults and older children can

also be affected.

Cleanliness is next to godliness, goes

the age-old adage. And indeed so. If

there’s anything we have learnt these

past few years since the menace that

is Covid-19 turned world economies

and lives topsy-turvy, it’s that it all

boils down to prevention – prioritizing

cleanliness is the key to keeping

Covid-19 steadily at bay.

And that rings true for a string of

diseases endemic in Malaysia, including

the Hand, Foot and Mouth Disease

(HFMD), rearing its ugly head in an

alarming way of late with Selangor

having documented 29,880 cases (at

press time), followed by Kuala Lumpur

and Putrajaya with a collective 11,687

cases, Perak 10,938 cases, Johor 7,052

cases, and Kelantan 6,532 cases.

Eighty-nine percent of these cases

involved children aged six and below,

with 56% reported having occurred

in nurseries, kindergartens and preschools,

followed closely by private

residential units (40%) and child care

centres (3%).

What is HFMD, and how

does it present itself?

Hand, Foot and Mouth Disease (HFMD)

or ‘enteroviral vesicular stomatitis with

exanthem’ is a common viral infection

that affects, as the name clearly

suggests, the hand, foot and mouth.

It leads to rashes on the hands and

feet that may be red spots or blisters,

which can sometimes be confused

with chicken pox. One of the hallmarks

of the condition is extensive multiple

ulcers that can be seen on the roof of

the mouth, tongue and lips.

Causes and early

symptoms

HFMD is commonly caused by the

Enterovirus Coxsackie A-16 and EV-71.

Coxsackie A16 tends to lead to a more

benign course, and thankfully is the

type most often seen here in Malaysia.

Severe HFMD that leads to brain and

heart inflammation and mortality is

usually associated with E-71.

HFMD – common or

seasonal?

The disease is generally endemic in

Malaysia, previously seen to peak

during the Southwest Monsoon

season particularly between January

and March, usually coinciding with

the opening of the new school term.

However, following the Covid-19

pandemic, we have seen a rather

exaggerated increase of cases this year,

which is likely due to the opening of

borders and the sudden movement of

people across states, the reopening

of physical schools and daycare,

which create a domino effect leading

to a marked increase of all common

childhood infections, including HFMD.

Is it contagious?

HFMD spreads easily via contact with

respiratory droplets, saliva, fluids from

the blisters and infected feces. It is

usually most contagious during the

first week of symptoms. However,

less commonly, a child can remain

infectious for weeks after recovery.

The stages of HFMD

HFMD usually starts with vague nonspecific

symptoms such as fever, lack

of appetite, sore throat and tiredness.

At this stage, it mimics any viral

infection. Subsequently, 1 to 2 days

later, the child will develop spots or

blisters mainly on the hands and feet,

as well as multiple painful oral ulcers.

In severe cases of HFMD, the child may

develop encephalitis (inflammation

of the brain) and myocarditis

(inflammation of the heart).

Quick cure?

There is no specific cure for HFMD. For

mild HFMD, treatment is prescribed

mainly to relieve the symptoms of the

condition until self-recovery. Children

who have the red flags (as described

below) will need more specific medical

care depending on the severity.

Recovery period

Most children will recover within a

week. They should remain isolated from

the school/daycare for 10 days from

the beginning of the symptoms. The

child should be fully recovered from all

symptoms prior to returning to school.

Where are children

most exposed to HFMD?

HFMD tends to cause outbreaks in

daycare centres or kindergartens

as these are places where young

children spend long hours together in

close proximity.

Could your child have

HFMD? What is your next

course of action?

First of all, ensure correct diagnosis

by bringing your kid to a health

professional. If the child is otherwise

active and drinking/eating well, he

or she can be treated at home with

fever medication and oral sprays to

soothe the ulcers. There are some oral

sprays for pain relief that are available

to reduce mouth pain and enable the

child to eat. Although the rash tends

to be harmless, occasionally creams

containing zinc oxide is used to provide

a “cooling” effect on the skin for the

child’s comfort.

Should the child develop red flag

symptoms such as fatigue, drowsiness,

refusal to drink, persistent high fever

beyond 48 hours, reduced urine, fast

breathing and fits, the child needs to

be brought immediately to a hospital

for further treatment. These children

may require intravenous (IV) fluids and

further investigation. Although most

children only have mild symptoms,

there is a severe version of HFMD

that can lead to brain and heart

inflammation; although uncommon,

these children need to be diagnosed

early and get intensive care.

CAN PREGNANT WOMEN

AND THEIR UNBORN BABIES

CONTRACT HFMD?

Pregnant women who are close to

their delivery due dates may be at risk

of passing the infection to their baby.

HFMD in a newborn can be more

severe and may even be fatal. It is best,

where possible, for pregnant women to

avoid all contact with HFMD patients.

Prevention

A diligent practice of hand hygiene

coupled with surface sanitization is the

best way to avoid HFMD. Hand hygiene

is especially important after contact

with feces (eg when changing diapers)

and after sneezing or coughing. The

use of face masks do play a role in

preventing the spread of respiratory

droplets. Also, it is best to avoid

crowded areas as much as possible.

No parent wants to suspect their child of having a

medical condition or health disorder, but catching

a condition early could mean the difference

between optimum treatment and outcome, and

not being able to do anything effective to help the

child. This is especially so when it comes to short

stature or stunting.

According to the 2019 National Health and

Morbidity Survey, over one in five (21.8%)

Malaysian children below the age of five years is

considered stunted, making this a fairly common

condition.

As height is a measurement that varies naturally

within a population, it may be difficult for parents

to tell whether their child is naturally short or is

not actually growing well.

In view of this, there are three things parents can

keep in mind when considering their child’s growth.

Expected Height Based On Sex & Age

First, is the child’s expected height based on

their sex and age? This can be measured against

standardized growth charts, which come in the

form of percentiles or standard deviations. If

your child’s height falls below the third percentile

for age or is more than two standard deviations

below the norm on the appropriate growth

chart, then they can be considered short.

Such growth charts are available online from

reputable organizations such as the World

Health Organization (WHO) and the US Centres

for Disease Control and Prevention (CDC),

as well as from your child’s paediatrician and

in your child’s school report once they start

attending primary school.

Genetic Height Potential

The second factor is the child’s genetic height potential. How

tall your child can grow is dependent on the associated genes

he or she inherits from both you and your partner.

Therefore, if both of you are naturally short, it shouldn’t be

a surprise if your child is also short.

However, if you and your partner are both tall and your child

is short, then it might be a sign that something is preventing

your child from achieving his or her full height potential.

Causes and Types of Short Stature

There are many potential causes of short stature, but they can

generally be grouped into three categories.

Normal Variant Short Stature

The first – and most common – is normal variant short stature.

Those within this category are generally short by nature. It

could be due to either their genetic potential, ie their parents

are naturally short, or some unknown reason (idiopathic).

Children in this group are usually otherwise healthy – just

shorter than the norm.

Those who are “late bloomers” or “late developers” also fall

within this normal variant short stature. Known medically as

having a constitutional delay in growth and puberty, such

children typically grow steadily, but slowly (usually below the

third percentile, according to the growth charts). They achieve

puberty much later than their peers; however, they will

continue to grow even after their peers have completed their

growth, eventually catching up in height.

Growth Velocity

The third is the child’s growth velocity. How fast is he or she

growing? If your child is growing at a normal rate, then starts

to slow down, it is definitely a cause for concern. This can be

observed by tracking their growth on the appropriate sex- and

age-based growth charts.

Your child’s growth should be fairly steady, ie more or less

on the same percentile or standard deviation, throughout

their childhood and adolescent years. Growth faltering to a

lower percentile is a sign of slowing down in growth rate and

should be investigated.

Intrinsic or Primary Short Stature

The second category comprises intrinsic or primary short

stature, ie being born with the cause of your short stature.

A common reason for this is syndromic disorders, which are

typically a result of genetic or chromosomal abnormalities.

In such cases, short stature is usually one out of a number of

symptoms resulting from the main disorder.

Examples of such disorders include Turner syndrome, Down

syndrome, Prader-Willi syndrome, Noonan syndrome and

achondroplasia, among others.

An intrauterine infection – where the mother catches an

infection during pregnancy that also affects the foetus – can

also cause the baby to be small. Such infections can usually

be recognized and diagnosed at birth, allowing the baby to

receive appropriate treatment.

Also in this category are babies who are considered small for

their gestational age. This can be due to factors concerning

the mother (eg having chronic conditions such as high blood

pressure, chronic kidney disease), the baby (eg being a twin

or having a birth defect) or the placenta (eg decreased blood

flow to the placenta, placenta previa or placenta abruption).

While most children in this group tend to catch up in growth

after birth, about 15% don’t and will remain small throughout

their lives if not diagnosed and treated early enough.

Secondary Causes of Short Stature

There are multiple causes within this category, but one

of the most common in Malaysia is that of nutritional

deficiency including protein calorie malnutrition, iron and

vitamin D deficiency.

Vitamin D deficiency is common among children as nowadays

kids tend to spend most of their time indoors, resulting in a

lack of exposure to sunlight. The sun’s ultraviolet B (UVB) ray

is a critical component in the production of vitamin D in our

body. Vitamin D is, in turn, vital for the absorption of calcium

to build and maintain our bones. In vitamin D deficiency, the

bones are soft and bowed especially in the weight-bearing

lower limbs resulting in short stature.

Other secondary causes of short stature include systemic

disorders such as gut problems, heart disease or a weak

immune system, as well as hormone deficiencies (eg growth

hormone deficiency).

Act fast

It is important that parents bring their child to see the

appropriate specialist – a paediatric endocrinologist, or at

least, a general paediatrician – if they start having doubts

about their child’s growth.

Do not be complacent and hope your child will catch up in

height later. Delaying diagnosis shortens the window period

for remedy as the bones continue to mature with time. Once

fully mature, nothing more can be done to help them grow

any taller.

Besides this, if your child has an underlying medical condition

that is hindering his or her growth, delaying diagnosis and

treatment may lead to prolongation of a poor quality life and

emergence of complications.

Hence, if you notice that your child does not seem to be

growing as he or she should, seek help from a paediatric

endocrinologist. Do not delay. You may make a significant

difference to your child’s future.

As regular measurement of your child’s growth is important,

parents can play an active role in monitoring their child’s

height. Convenient digital tools such as the Growth

as getting serial measurements to ensure growth is on track.

The second you hear your first-born’s cry for the first

time, all the pain you endured during labour magically

disappears, and your heart is filled with immense joy at the

sight of your baby after carrying the little one in your belly

for 9 months. Now that the baby’s finally here, reality sinks

in … and fear creeps in. What do I do now? How am I going

to feed this bundle of perfection? Where do I begin?

Calm down and start enjoying your baby with a few simple

tips and go with your maternal instincts. You will generally

know what to do but the following guidelines will certainly

help you through moments of uncertainty.

Birth to 4 Months

Remember, babies are born with

natural instincts. They can smell and

hear their mothers from being in

the womb for at least 9 months. As

you bring the baby to your breast,

gently stroke the corner of the baby’s

mouth. This makes them turn their

head and open their mouth to follow

and root in the direction of the

stroking. This is called the rooting

reflex which lasts about 4 months.

This guides the baby to find the

breast to start latching and feeding.

What to feed

Babies who are breastfed exclusively

for the first 4 months and partially after

that have a reduced risk of respiratory

and gastrointestinal infections.

If for some reason, the mother is

unable to produce milk, the baby can

be fed formula until they are ready to

take other forms of nutrition.

How much to feed?

Most newborns sleep through their first

day. After that, they nurse every two

to three hours up to eight to 12 times

a day.

How much breast milk a baby needs

differs from baby to baby and from

month to month. A new born takes

less than an ounce of breast milk per

feeding and by the time they are a

month old they can drink about 3 to 4

ounces per feed. They can feed up to

eight times a day. This is about 24 to

28 ounces daily.

How to know if the baby is getting

enough milk?

Trust your child. If the baby cries for

more feeds, then give them extra. If

your baby stops sucking or falls asleep

at the breast, it is a sign that they are

full.

Some newborns finish in five minutes

while others may take up to 20 minutes

or more on one or both breasts while

older babies may take five to 10

minutes on each breast.

But, check your baby constantly. Is

there weight loss? Is the urine dark

coloured? Are there fewer than six wet

diapers daily?

This would indicate that your baby isn’t

getting enough breast milk or formula.

You should start increasing either the

amount of milk per feed or increase the

number of feeds per day.

But, don’t worry. Often, there is a

growth spurt around 2 to 3 weeks,

6 weeks and 3 months where you may

need to feed your baby more often.

This is called cluster feeding and it tells

your body to produce more milk for

your growing baby.

Feeding tip

Till about six months it is advisable to

stick to breast milk or formula. Your

baby’s digestive tract is still developing,

so it is safer to stick to liquids and stay

away from any form of solid food.

4 to 6 months

Most babies are ready to start on solid

food between the ages of 4 and 6

months as a complement to breast

feeding or formula feeding. But how

would you know if your child is ready?

You don’t want to rush into this phase.

Watch out for some simple signs of

readiness.

• There is a significant weight gain

(generally double the birth weight).

On average, Asian babies weigh

about 3.26 kilograms (7.2 lb)

• Can stay in a sitting position

• Can hold their head steady

• Can coordinate their eyes, hands

and mouth so they can look at the

food, pick it up and put it in their

mouths

• Swallow food rather than spit it

back

• Can close their mouth around

a spoon

• Can move food from the front

to the back of their mouth

Daily Coffee Lowers Risk of

Early Death

A few cups of coffee a day keep early death away! According to

a recent study published in Annals of Internal Medicine, people

who drink moderate amounts of coffee (about 1.5 to 3.5 cups a

day) are less likely to wind up early on their deathbed. This goes

for both black coffee and coffee sweetened with sugar, said the

May-published study which involved 172,000 adults without any

history of heart disease or cancer for an average of seven years;

all participants were from the UK Biobank with extensive medical

details including diet and lifestyle.

“People who regularly drank black coffee were 16 to 21 percent

less likely to die during the study period. Meanwhile, coffee

drinkers who added about a teaspoon of sugar had 29 to 31

percent lower odds of premature death,” quoted the study. Many previous studies have indeed found that coffee lowers the risk

of early death, however, they did not distinguish between black coffee and coffee with added sugar. Though the study was not

designed to show how coffee might directly help prevent disease or help people live longer, it concluded that coffee in moderation

is safe.

Coffee has been linked with many benefits such as improved memory and mood, increased fat burning, and a lower risk of

diabetes and certain neurogenerative diseases. At the same time, it has also been confined to not more than four cups a day;

anything above the average intake is tied to health problems, including difficulty sleeping, headaches, rapid heartbeats and muscle

tremors.

Researchers concluded that most coffee drinkers don’t need to eliminate the beverage from their diet but to be mindful about

high-calorie specialty coffees.

Workouts Boost Immune Response

to Flu Shots

A new study conducted across Australia and the United

Kingdom shows that workouts before and after a flu vaccination

help to boost a person’s immune response, and these benefits

are particularly noticeable in older adults. However, experts warn

against physical activity when one is feeling sick.

The study reported that workouts increase a person’s immune

response after they get a flu shot but it seems to be more

pronounced among those who already regularly exercise or

exercise “acutely”.

Researchers measured the antibody response before vaccination,

immediately after vaccination, and at 4 to 6 weeks after

vaccination, the time for peak antibody growth. The data

examined from 550 participants clearly showed higher

antibodies in the acute-exercised participants compared to rested controls and physically active compared to inactive.

The periods of workouts were between 15 and 50 minutes and most participants were monitored immediately before

vaccination and immediately after. Three potential moderators were included in the model: sex, body mass index (BMI) and age.

“Improved response with [physical activity] is more consistently identified among older populations. This suggests the young

have the ability to respond well to vaccination regardless of sufficient (physical activity),” said the study authors.

They concluded that they found some benefit from either acute workouts or physical activity on antibodies but there was no

added benefit of acute exercise to inactive participants.
