Service Delivery<br />
Time to Move On from Congregated settings<br />
• Complete the move of 223 people from large institutional settings to community based models,<br />
reducing the total number of people identified in the Time to Move On from Congregated Settings<br />
Report (2009) from 4,099 to 2,518 in 2017<br />
• Enhance service improvements that will focus on compliance with regulatory standards and agreed<br />
priority sites in conjunction with HIQA<br />
• Progress implementation of the recommendations of the McCoy Review – Áras Attracta<br />
• Implement plans to meet housing requirements for those transitioning to the community with approved<br />
housing bodies, housing authorities and HSE Estates.<br />
New Directions<br />
• Provide day service supports for approximately 1,500 young people leaving school or graduating from<br />
rehabilitative training programmes<br />
• Establish New Directions implementation groups in CHOs<br />
• Develop quality improvement plans to continuously address service gaps in line with the standards and<br />
planning framework<br />
• Develop training package to support the person-centred planning framework and finalise a schedule of<br />
person-centred planning training<br />
• Develop a rehabilitation training transition programme.<br />
Progressing Disability Services for Children and Young People (0–18) Programme<br />
• Reconfigure 0–18s disability services into children’s disability network teams<br />
• Implement the National Access Policy in collaboration with primary care to ensure one clear pathway of<br />
access for all children with a disability into their local services<br />
• Evaluate the effectiveness of the national policy on access to services for children with a disability or<br />
developmental delay in collaboration with primary care<br />
• Improve Disability Act compliance for assessment of need with a particular emphasis on putting in<br />
place improvement plans for CHOs that have substantial compliance operational challenges.<br />
Neuro-Rehabilitation Strategy<br />
• Finalise and progress the implementation of the framework for neuro-rehabilitation strategy and<br />
establish an innovative pilot day service aimed at supporting people with severe acquired brain injuries.<br />
Enhance governance and management<br />
• Strengthen the overall process management regarding emergency places<br />
• Develop IT based residential bed management / tracking system for capturing residential services and<br />
emergency places across all CHOs<br />
• Improve efficiency and effectiveness of HSE and provider services with the focus on management and<br />
administration costs, transport and other non-frontline service costs including mergers and other<br />
collaborative partnerships where necessary to support sustainable models of service<br />
