• Choose the right section – in your case there<br />
is only one section i.e. 01.<br />
• Do not tick in the HW or HS box.<br />
• If you’ve made a mistake fill up the insert<br />
delete form (inside your folder or obtain it<br />
from PG office).<br />
• Submit the form to SPS before due date<br />
