1. OBSERVING REPORTS<br />

1.1 Instrument: Mega Prime + QSO<br />

Dates: <strong>May</strong> 19 – 26, <strong>2004</strong> (8 nights)<br />

Success: very good. The first night and the first half of the second night were lost to<br />

high humidity, the rest of the run had excellent observing conditions, mostly photometric<br />

wit excellent seeing period. This gave us a good opportunity to better balance the<br />

agency shares and provide first class data to the users.<br />

Problems: The only major problems to report is a leak in the cryogenic vessel that<br />

demanded to pump every 5-6 hours. 1h per night was lost to this problem throughout the<br />

run. Other problems included some minor system glitches. No more than 20 min where<br />

lost to them over the run.

SAFETY REPRESENTATION COMMITTEE<br />

15 <strong>May</strong> <strong>2004</strong><br />

<strong>Present</strong>: Rohendra Atapattu, Laurie Dale, Todd Szarlan for Susan Brotman, Nadine Manset for Thierry<br />

Forveille, DeeDee Warren, Lisa Wells, Billy Mahoney for Kanoa Withington, and Roger Wood. Absent:<br />

Coleen Hickman, Sharon Potter, and Yasu Uchima.<br />

1. Call to Order<br />

A. Rohendra Atapattu called the meeting to order at 13:30<br />

2. Approval of 14 March <strong>2004</strong> Minutes<br />

3. Minutes were approved as written.<br />

4. Review of New Accidents & Incidents<br />

B. One injury was reported. An employee suffered back injury while assisting with raising the WFC<br />

to reach the crane. Suggestion to avoid this hazard were to (1) add 3’ legs to WFC to that it could<br />

reach crane or (2) put longer straps on crane to reach WFC.<br />

C. One incident was reported. An OA discovered a small glycol spill on the 5 th floor. The daycrew<br />

cleaned up the spill. It was suggested that a detection device could be installed that would warn of<br />

the leak and shut down systems.<br />

D. Rohendra reported on the most recent MKSS Ranger speed report. Two CFHT vehicles were<br />

noted driving above 35mph (36mph and 38mph) at approximately 16:45 on the survey dates.<br />

5. Concerns, Accidents & Incidents<br />

E. Previously reported items:<br />

(1) It was noted that the EEV does not have a rope or an AED. Rohendra sent email advising<br />

MKSS and will bring up these concerns at the MKSS Safety meeting<br />

(2) Compressed gas cylinders at the summit have been fitted with proper protective guards.<br />

Protective guards need to be ordered for cylinders at headquarters. (Action item for<br />

Rohendra.)<br />

(3) Immediately following the meeting, Roger and Rohendra rearranged cables in large conference<br />

room to reduce trip hazard. (item closed)<br />

(4) Laurie Dale reported that the Admin Groups would like a procedure to be prepared that<br />

addresses situations when immediate help is needed in the front office due to a direct or<br />

perceived threat. The committee suggests that the procedure also address a situation when a<br />

“silent” emergency evacuation of the building is warranted. The admin group met with a local<br />

police officer to discuss possible solutions. (Rohendra and DeeDee will develop an<br />

Emergency Response Plan to be reviewed and implemented. – still pending.)


(5) The dome mezzanine has points where there is a potential for someone to be trapped during<br />

rotation. Railings at these corners should be clearly marked to warn of the potential danger.<br />

(Signage has been posted and a chain was installed at the top of the stairs by the Summit<br />

Crew ). OAs requested that it would be better to have the chain installed at the bottom of the<br />

stairs. It was additionally emphasized that the placement of the chain at the top of stairs is a<br />

hazard as the chain cannot be seen by someone holding equipment in front of them nor at<br />

night.<br />

(6) When the dome is placed into automatic rotation mode during nighttime observing, it was<br />

suggested that a buzzer sound as a warning for Dome movement Rohendra and Grant<br />

Matsushige determined that a buzzer alarm was not practical as both the Telescope and<br />

Dome move automatically and pose a danger. An alternate suggestion was to post signage in<br />

the elevator and on the 5 th floor stairway door warning that movement of the dome and<br />

telescope occurs without warning. Also that the OAs should inform visitors that they must<br />

notify the OA that they intend to access the 5 th floor before doing so. (Action item for<br />

Rohendra to order signage.)<br />

(7) The OAs have expressed concern that there are inadequate procedures to address the<br />

conditions of nighttime work. Rohendra will set up a meeting with Pierre Martin and the OAs<br />

to discuss their concerns and begin a process of preparing Job Hazard Analyses to identify<br />

needed procedures. (Lisa Wells and David Woodworth have begun preparing job lists for Job<br />

Hazard Analysis.)<br />

(8) Brighter nightlights have been installed in the hallways at the Waimea Headquarters. Also,<br />

lighting is still needed in the hallway near the lounge, near the clean room, in the loading bay<br />

and near the shop area. (Electrical contractor has hired and parts are on order for the job.)<br />

(9) Machine shop has been reorganized to reduce clutter. (Electrical contractor was hired and<br />

parts are on order for the job.)<br />

(<strong>10</strong>) The revised Housekeeping procedure was issued to supervisors. Staff training on new<br />

procedure is required by each group. Next walkthrough is scheduled for the last week of <strong>May</strong>.<br />

(11) The Executive reviewed the Two-Person rule and agreed that the rule should be enforced<br />

without exception. The Astronomy group discussed procedures and will provide their<br />

suggestions to management.<br />

(12) The center-back door continually needs adjustments to close properly.<br />

(13) An employee caught his hand in the elevator on the 5th floor do to dark conditions. (Sharon<br />

Potter will order glowing tape to put around the doorframe and elevator call button to<br />

illuminate these items in the dark.)<br />

(14) The Instrumentation Group expressed concern regarding lack of proper fall protection A<br />

retractable tethering device for attaching the fall harness to the Seurrier trusses was provided<br />

by Rohendra. The Instrumentation group needs to install the devices in the appropriate areas.<br />

(15) After flooding in Astronomer’s wing, new electrical outlets were installed with safety breakers.<br />

F. New items:<br />

(1) An OA and SO accidentally locked themselves out of the observatory without keys for the<br />

Suburban. Since then, spare summit door keys have been placed in the glove compartment<br />

of each Suburban. It was also suggested that spare keys be provided to other observatories<br />

for emergency purposes.<br />

(2) Lisa Wells noted that during power outages at the summit, the generator is turned on and the<br />

exterior door(s) are opened to ventilate the room. She expressed concern that someone<br />

could wander into the building with the doors open and suggested that signage and/or a<br />

barrier could be installed to prevent unwanted entry. Rohendra will review the suggestion.<br />

(3) Courtyard lights are not coming on until it becomes very dark. Roger will adjust sensor so that<br />

the lights come on earlier in the evening.


6. Governor’s Safety Conference<br />

7. Rohendra, Ralph, DeeDee and Sharon attended the conference in April. There were many useful<br />

safety sessions including fall protection, environmental management, confined space, etc.<br />

8. A confined space vendor/expert subsequently visited our observatory and will be contracted to review<br />

our policies, install equipment and signage and train personnel.<br />

9. Next meeting<br />

G. The next meeting was scheduled for 15 July, 13:30 – 15:30.<br />

H. Rohendra will prepare and distribute an agenda prior to the meeting date.<br />
