SAFETY REPRESENTATION COMMITTEE<br />

15 <strong>May</strong> <strong>2004</strong><br />

<strong>Present</strong>: Rohendra Atapattu, Laurie Dale, Todd Szarlan for Susan Brotman, Nadine Manset for Thierry<br />

Forveille, DeeDee Warren, Lisa Wells, Billy Mahoney for Kanoa Withington, and Roger Wood. Absent:<br />

Coleen Hickman, Sharon Potter, and Yasu Uchima.<br />

1. Call to Order<br />

A. Rohendra Atapattu called the meeting to order at 13:30<br />

2. Approval of 14 March <strong>2004</strong> Minutes<br />

3. Minutes were approved as written.<br />

4. Review of New Accidents & Incidents<br />

B. One injury was reported. An employee suffered back injury while assisting with raising the WFC<br />

to reach the crane. Suggestion to avoid this hazard were to (1) add 3’ legs to WFC to that it could<br />

reach crane or (2) put longer straps on crane to reach WFC.<br />

C. One incident was reported. An OA discovered a small glycol spill on the 5 th floor. The daycrew<br />

cleaned up the spill. It was suggested that a detection device could be installed that would warn of<br />

the leak and shut down systems.<br />

D. Rohendra reported on the most recent MKSS Ranger speed report. Two CFHT vehicles were<br />

noted driving above 35mph (36mph and 38mph) at approximately 16:45 on the survey dates.<br />

5. Concerns, Accidents & Incidents<br />

E. Previously reported items:<br />

(1) It was noted that the EEV does not have a rope or an AED. Rohendra sent email advising<br />

MKSS and will bring up these concerns at the MKSS Safety meeting<br />

(2) Compressed gas cylinders at the summit have been fitted with proper protective guards.<br />

Protective guards need to be ordered for cylinders at headquarters. (Action item for<br />

Rohendra.)<br />

(3) Immediately following the meeting, Roger and Rohendra rearranged cables in large conference<br />

room to reduce trip hazard. (item closed)<br />

(4) Laurie Dale reported that the Admin Groups would like a procedure to be prepared that<br />

addresses situations when immediate help is needed in the front office due to a direct or<br />

perceived threat. The committee suggests that the procedure also address a situation when a<br />

“silent” emergency evacuation of the building is warranted. The admin group met with a local<br />

police officer to discuss possible solutions. (Rohendra and DeeDee will develop an<br />

Emergency Response Plan to be reviewed and implemented. – still pending.)
