2. PROGRESS REPORTS 2.1 SAFETY OFFICER (R. Atapattu) Minutes of the Meeting CANADA-FRANCE-HAWAII TELESCOPE CORP. SAFETY REPRESENTATION COMMITTEE 15 May 2004 Present: Rohendra Atapattu, Laurie Dale, Todd Szarlan for Susan Brotman, Nadine Manset for Thierry Forveille, DeeDee Warren, Lisa Wells, Billy Mahoney for Kanoa Withington, and Roger Wood. Absent: Coleen Hickman, Sharon Potter, and Yasu Uchima. 1. Call to Order A. Rohendra Atapattu called the meeting to order at 13:30 2. Approval of 14 March 2004 Minutes 3. Minutes were approved as written. 4. Review of New Accidents & Incidents B. One injury was reported. An employee suffered back injury while assisting with raising the WFC to reach the crane. Suggestion to avoid this hazard were to (1) add 3’ legs to WFC to that it could reach crane or (2) put longer straps on crane to reach WFC. C. One incident was reported. An OA discovered a small glycol spill on the 5 th floor. The daycrew cleaned up the spill. It was suggested that a detection device could be installed that would warn of the leak and shut down systems. D. Rohendra reported on the most recent MKSS Ranger speed report. Two CFHT vehicles were noted driving above 35mph (36mph and 38mph) at approximately 16:45 on the survey dates. 5. Concerns, Accidents & Incidents E. Previously reported items: (1) It was noted that the EEV does not have a rope or an AED. Rohendra sent email advising MKSS and will bring up these concerns at the MKSS Safety meeting (2) Compressed gas cylinders at the summit have been fitted with proper protective guards. Protective guards need to be ordered for cylinders at headquarters. (Action item for Rohendra.) (3) Immediately following the meeting, Roger and Rohendra rearranged cables in large conference room to reduce trip hazard. (item closed) (4) Laurie Dale reported that the Admin Groups would like a procedure to be prepared that addresses situations when immediate help is needed in the front office due to a direct or perceived threat. The committee suggests that the procedure also address a situation when a “silent” emergency evacuation of the building is warranted. The admin group met with a local police officer to discuss possible solutions. (Rohendra and DeeDee will develop an Emergency Response Plan to be reviewed and implemented. – still pending.)
(5) The dome mezzanine has points where there is a potential for someone to be trapped during rotation. Railings at these corners should be clearly marked to warn of the potential danger. (Signage has been posted and a chain was installed at the top of the stairs by the Summit Crew ). OAs requested that it would be better to have the chain installed at the bottom of the stairs. It was additionally emphasized that the placement of the chain at the top of stairs is a hazard as the chain cannot be seen by someone holding equipment in front of them nor at night. (6) When the dome is placed into automatic rotation mode during nighttime observing, it was suggested that a buzzer sound as a warning for Dome movement Rohendra and Grant Matsushige determined that a buzzer alarm was not practical as both the Telescope and Dome move automatically and pose a danger. An alternate suggestion was to post signage in the elevator and on the 5 th floor stairway door warning that movement of the dome and telescope occurs without warning. Also that the OAs should inform visitors that they must notify the OA that they intend to access the 5 th floor before doing so. (Action item for Rohendra to order signage.) (7) The OAs have expressed concern that there are inadequate procedures to address the conditions of nighttime work. Rohendra will set up a meeting with Pierre Martin and the OAs to discuss their concerns and begin a process of preparing Job Hazard Analyses to identify needed procedures. (Lisa Wells and David Woodworth have begun preparing job lists for Job Hazard Analysis.) (8) Brighter nightlights have been installed in the hallways at the Waimea Headquarters. Also, lighting is still needed in the hallway near the lounge, near the clean room, in the loading bay and near the shop area. (Electrical contractor has hired and parts are on order for the job.) (9) Machine shop has been reorganized to reduce clutter. (Electrical contractor was hired and parts are on order for the job.) (10) The revised Housekeeping procedure was issued to supervisors. Staff training on new procedure is required by each group. Next walkthrough is scheduled for the last week of May. (11) The Executive reviewed the Two-Person rule and agreed that the rule should be enforced without exception. The Astronomy group discussed procedures and will provide their suggestions to management. (12) The center-back door continually needs adjustments to close properly. (13) An employee caught his hand in the elevator on the 5th floor do to dark conditions. (Sharon Potter will order glowing tape to put around the doorframe and elevator call button to illuminate these items in the dark.) (14) The Instrumentation Group expressed concern regarding lack of proper fall protection A retractable tethering device for attaching the fall harness to the Seurrier trusses was provided by Rohendra. The Instrumentation group needs to install the devices in the appropriate areas. (15) After flooding in Astronomer’s wing, new electrical outlets were installed with safety breakers. F. New items: (1) An OA and SO accidentally locked themselves out of the observatory without keys for the Suburban. Since then, spare summit door keys have been placed in the glove compartment of each Suburban. It was also suggested that spare keys be provided to other observatories for emergency purposes. (2) Lisa Wells noted that during power outages at the summit, the generator is turned on and the exterior door(s) are opened to ventilate the room. She expressed concern that someone could wander into the building with the doors open and suggested that signage and/or a barrier could be installed to prevent unwanted entry. Rohendra will review the suggestion. (3) Courtyard lights are not coming on until it becomes very dark. Roger will adjust sensor so that the lights come on earlier in the evening.
