The <strong>Post</strong> <strong>Polio</strong> <strong>Syndrome</strong> 5<br />

this theory it is assumed that many motor neurons were destroyed by<br />

the orig<strong>in</strong>al <strong>in</strong>fection, leav<strong>in</strong>g small numbers of overworked neurons to<br />

<strong>in</strong>nervate many (orphaned) muscle fibres. With time and overuse, these<br />

few work<strong>in</strong>g neurons simply wear out leav<strong>in</strong>g muscles denervated. 7<br />

Proposed aetiologies <strong>in</strong>clude:<br />

● Motor unit dysfunction-degenerative change with<strong>in</strong> motor<br />

units.<br />

● Muscle overuse.<br />

● Muscle underused.<br />

● Loss of motor units due to age.<br />

● Growth hormone or other hormonal effects.<br />

● The comb<strong>in</strong>ed effects of disuse, overuse, pa<strong>in</strong>, weight ga<strong>in</strong> or<br />

other illness.<br />

<strong>Management</strong> of <strong>Post</strong> <strong>Polio</strong> <strong>Syndrome</strong><br />

There is no medical or pharmacological treatment of choice for PPS<br />

but there are many ways of manag<strong>in</strong>g polio-related difficulties, which<br />

should be symptom specific and <strong>in</strong>volve a multidiscipl<strong>in</strong>ary team. 8 A<br />

well <strong>in</strong>formed general practitioner may be at the hub of this team which<br />

may <strong>in</strong>clude the follow<strong>in</strong>g discipl<strong>in</strong>es:<br />

● Neurology.<br />

● Physiotherapy.<br />

● Occupational Therapy.<br />

● Speech and Language Therapy.<br />

● Respiratory medic<strong>in</strong>e.<br />

● Chiropody/Podiatry.<br />

● Counsellors/psychologists.<br />

● Dietetics.<br />

● Pa<strong>in</strong> specialists.<br />

● Social workers.<br />

● Nurse specialists.<br />

● Orthotists.<br />
