This grant agreement funds three (3) part-time social workers who facilitate monthly support groups for survivors of homicide victims and who facilitate support group sessions in conjunction with the Annual Homicide Memorial Service. This grant funds a portion of the salary and fringe benefits of one (1) sexual assault/domestic violence specialist who provides services to adult and child victims of sexual assault/abuse and domestic violence within the Victim Witness Assistance Program. This is a renewal grant that provides continued partial funding for this position. Estimated Fiscal Impact: $24,123.00. Grant Award: $23,447.00. Funding period: July 1, 2001 through June 30, 2002. (250-110 Account). ITEM #23 APPROVED GRANT AWARD AMENDMENT Transmitting a Communication, dated July 16, 2001 from KAREN L. SCOTT, M.D., M.P.H., Chief Operating Officer, Department of Public Health requesting authorization to accept an amended grant award in the amount of $52,672.00 from the Illinois Department of Public Health. The purpose of this grant amendment is to conduct syphilis elimination activities in suburban Cook County through the establishment of a coalition between the Cook County Department of Public Health and community-based organizations. Page 13 POST BOARD ACTION AGENDA Meeting of August 9, 2001 COOPERATIVE EDUCATIONAL MASTER AGREEMENT PROGRAM ADDENDA ITEM #24 APPROVED Transmitting a Communication, dated June 25, 2001 from RUTH M. ROTHSTEIN, Chief, Bureau of Health Services requesting authorization from the Cook County Board of Commissioners to extend by twelve (12) months and amend the scope of the agreement with the Board of Trustees of Community College District No. 508, Chicago, Illinois, to provide monies for scholarship and book funds. This allocation will provide relief for deserving Malcolm X Physician Assistant Students who as adult learners are not eligible for standard student aid. For over ten years Cook County Hospital and Malcolm X College have jointly sponsored the Physician Assistant Student Program whose mission is to prepare individuals to become certified physician assistants and to practice in medically underserved communities. Since the program requires two full years of intensive study and significant clinical experience at Cook County Hospital, these funds were requested to enable needy students to complete the course of study. Eligibility for the award of funds will be based on financial need and service commitment. These funds will be disbursed to students selected by a committee with Hospital representation. Estimated Fiscal Impact: $100,000.00. Contract period: August 1, 2001 through July 31, 2002. (899- 298 Account). Requisition No. 18990944. ITEM #25 APPROVED * * * * * Transmitting a Communication, dated July 26, 2001 from CYNTHIA HENDERSON, M.D., M.P.H., Chief Operating Officer, Oak Forest Hospital of Cook County In accordance with the Cooperative Educational Master Agreement between Oak Forest Hospital of Cook County and South Suburban College, we are requesting approval of the Licensed Practical Nurse (LPN) Clinical Rotation Program Addendum for the rotation of students. The objective of the addendum is to provide educational instruction for nursing students in a hospital environment. Estimated Fiscal Impact: None. Contract period: October 1, 2001 through September 30, 2004. Requisition No. 18980857. ITEM #26 APPROVED COOPERATIVE EDUCATIONAL MASTER AGREEMENT RENEWAL Transmitting a Communication, dated June 27, 2001 from LACY L. THOMAS, Chief Operating Officer, Cook County Hospital requesting authorization for the Purchasing Agent to renew the Program Addendum to the Cooperative Educational Master Agreement with the University of Illinois College of Medicine at Chicago, for the Affiliated Residency Program in Orthopaedics, Otolaryngology and Plastic Surgery. The goal of the training program is to provide balanced, comprehensive training for residents from the University of Illinois College of Medicine at Chicago, and to maintain quality patient care at Cook County Hospital.
