SHINE, Chief Deputy Treasurer Page 36 POST BOARD ACTION AGENDA Meeting of August 9, 2001 requesting authorization for the Purchasing Agent to enter into a contract with Aston Group (Microsoft Great Plains), Chicago, Illinois, to provide Microsoft Great Plains e-Enterprise General Ledger Accounting Software. The office prepared and distributed a Request for Proposal (RFP) for General Ledger Accounting Software in May 2001. Six potential vendors received a copy of the RFP. The following four (4) responses were received: 1. Epicor Software Corporation 2. Aston Group (Navision) 3. Aston Group (Microsoft Great Plains) 4. Mitchell Humphrey The office evaluated the responses based on various criteria. The criteria included, but was not limited to: • Can the system accommodate additional detailed account information? • Does the system provide an audit trail for all transactions? • Is your company certified as a Minority-owned Business Enterprise (MBE) or Woman-owned Business Enterprise (WBE)? • Does the system provide security for posting, viewing, and reporting? • Provide the ability to drill down to transaction source? • Environmentally and strategically compatible with the Treasurer’s Office current technical environment? • Ability to interface with other financial systems? Based on the results of the office's evaluation, Aston Group (Microsoft Great Plains) ranked higher than the other firms responding. Estimated Fiscal Impact: $323,000.00. BERSKY, Chief Operating Officer, Cermak Health Services of Cook County requesting authorization for the Purchasing Agent to enter into a contract with Emmett W. Shelton, DDS, Chicago, Illinois, to provide professional general dentistry services to the detainees at the Department of Corrections. Reason: This dentist is contracted to provide dental services for detainees at the jail. Dr. Shelton will provide 750 hours of dental services. Estimated Fiscal Impact: $37,500.00. BERSKY, Chief Operating Officer, Cermak Health Services of Cook County requesting authorization for the Purchasing Agent to enter into a contract with Siemens Medical Systems, Hoffman Estates, Illinois, for a service maintenance contract for various radiology equipment owned by Cermak Health Services of Cook County. Reason: Siemens Medical Systems is the manufacturer and the only known distributor that provides service and replacement parts for this equipment. Estimated Fiscal Impact: $141,800.00 ($70,900.00 per year). Contract period: December 1, 2001 through November 30, 2003. (240-442 Account). Requisition No. 22400001. Purchasing Agent concurs. ITEM #93 APPROVED Transmitting a Communication from * * * * * LACY L. THOMAS, Chief Operating Officer, Cook County Hospital requesting authorization for the Purchasing Agent to enter into a contract with Environmental Specialties, Incorporated, Raleigh, North Carolina, for the purchase of an environmental specialties cool, dry and moist (CDM) reach-in stability chamber for the Department of Pathology, Division of Microbiology Virology. Reason: Environmental Specialties, Incorporated is the only known manufacturer and distributor of the CDM reach-in stability chamber with technology that includes a split level coil design for maintaining both temperature and humidity to allow growth of pathogens in a tight controlled temperature setting which enables more accurate test results. Estimated Fiscal Impact: $16,230.00. One time purchase. (717/897-540 Account). Requisition No. 18974547. Sufficient funds have been appropriated to cover this request. Purchasing Agent concurs.
