The Medina County Senior Services<br />

Network (<strong>MCSSN</strong>) was established<br />

in 1994 by a small group<br />

of dedicated individuals who set<br />

out to develop, coordinate and act<br />

as advocates for a community of<br />

care network in Medina County.<br />

Over the years, <strong>MCSSN</strong> has<br />

evolved into a group with over 100<br />

members and organizations with<br />

one common mission: “To develop<br />

and coordinate a network of care<br />

and resources for older adults in<br />

Medina County.” For twenty years,<br />

we have done this through networking,<br />

sharing ideas, and publication<br />

of this guide.<br />

<strong>MCSSN</strong>’s first resource guide<br />

was produced in 1996. This publication<br />

was in response to a<br />

growing need for a comprehensive<br />

guide of services for senior citizens<br />

in Medina County. The need has<br />

now grown and become greater<br />

and, much like <strong>MCSSN</strong>, our guide<br />

has evolved into what it is today.<br />

It is our hope that <strong>MCSSN</strong><br />

Senior Solutions, formerly The<br />

Source, provides you with a for-<br />

mat to easily navigate and find the<br />

products and services you need.<br />

Medina County Office for Older Adults<br />

The Medina County Office for<br />

Older Adults is dedicated to helping<br />

those over age sixty and their<br />

families by providing education<br />

regarding benefits and services<br />

available in Medina County and<br />

beyond. Our services are extensive<br />

and range from simply providing<br />

referrals to helping seniors enroll<br />

in eligible programs.<br />

Transportation Services<br />

Transportation can be an issue<br />

for seniors and those with disabilities.<br />

We work with Medina Transit<br />

and visit people in their homes to<br />

register them for in-county transit<br />

trips. Subsidized trips include<br />

those for medical appointments,<br />

groceries or social service agencies.<br />

Registration with MCOOA entitles<br />

each participant to free loop<br />

rides in the cities of Brunswick,<br />

Medina and Wadsworth.<br />

Home Delivered Meals<br />

For those who are homebound,<br />

outreach workers make visits to<br />

determine eligibility for home delivered<br />

meals. Six routes throughout<br />

the county serve more than<br />

160 hot, nutritious, lunchtime<br />

meals at no charge. The Medina<br />

County Commissioners make<br />

these services possible with help<br />

from Title III funding provided by<br />

the Western Reserve Area Agency<br />

on Aging through the Ohio Department<br />

on Aging.<br />

Senior Centers and Services<br />

Medina County Office for Older<br />

Adults has one central senior center<br />

for programming throughout<br />

the county, located in Medina, and<br />

also coordinates senior services in<br />

Brunswick.<br />

Meal Programs,<br />

Entertainment & Activities<br />

We have meal programs in<br />

Medina, as well as a voucher program<br />

in Wadsworth and a congregate<br />

program in Brunswick. Our<br />

center is a fun place for seniors to<br />

gather with friends, new and old,<br />

and enjoy a tasty meal with good<br />

conversation. Guests are offered<br />

fun activities such as Senior Day<br />

in May and Picnics on the Patio<br />

throughout the summer.<br />

Other entertaining and informative<br />

activities in our senior center<br />

include: Lunch and Learn programs,<br />

day trips, weekly exercise<br />

classes, card games, yoga classes,<br />

bingo and special celebrations for<br />

holidays.<br />

Seasonal Programs<br />

Seasonal programs offered<br />

include Farmer’s Market, HEAP<br />

and free tax preparation by AARP<br />

volunteers. Our programs are<br />

featured in our newsletters, available<br />

in Brunswick and Medina, or<br />

online at www.mcooa.org<br />

Older Adults (MCOOA) has<br />

partnered with the City of Brunswick<br />

to offer seniors service coordination<br />

at the Brunswick Recreation<br />

Center. Five days a week, an<br />

MCOOA employee is at the Rec<br />

Center to offer service assistance,<br />

referrals to programs and other<br />

support to seniors in the area.<br />

Case Management in Medina County for Seniors<br />

Case management is a service<br />

provided in the home to seniors,<br />

their caregivers, and their families.<br />

It often helps families plan<br />

for life’s next steps or get them<br />

through a crisis by defining the<br />

immediate issues, setting achievable<br />

goals and close support to<br />

help meet those goals. While there<br />

are several different types of case<br />

management service providers,<br />

including insurances, we offer<br />

some specialty services in Medina<br />

County.<br />

Medina County Office for Older<br />

Adults / Options Counseling<br />

Program - MCOOA offers short<br />

term case management to individuals<br />

or families who are ready to<br />

look at alternatives to keep their<br />

aging loved ones in the home<br />

or to transition into alternative<br />

living arrangements. Many times,<br />

as people grow older, they need<br />

assistance to keep aging in place.<br />

A case manager helps seniors and<br />

their caregivers assess their current<br />

living situation and see what<br />

resources are available to help<br />

them, whether it is financial help,<br />

linking to other services, nutritional<br />

help or other supportive services.<br />

County JFS – Sometimes,<br />

older adults in the community<br />

fall victim to physical abuse and<br />

neglect, self-neglect or financial<br />

exploitation. Medina County<br />

Adult Protective Services (APS)<br />

investigates all allegations of the<br />

abuse or neglect of adults age<br />

60 and over. Investigations can<br />

involve case management to help<br />

alleviate the causes of maltreatment.<br />

This could include securing<br />

service providers in the home,<br />

obtaining medical treatment for<br />

victims of abuse or neglect, police<br />

involvement or court-ordered<br />

services as well as other supportive<br />

action. All APS investigations and<br />

any case management services that<br />

result from these investigations are<br />

as people under age 60 who are<br />

card automatically, may apply for<br />

a card at most public libraries and<br />

How to Obtain a Disabled Parking Permit in Ohio<br />

Anyone who has a disability that limits mobility by<br />

impeding the ability to walk can apply for an Ohio<br />

Disability parking placard or disability plates. The<br />

disability must be certified by the person’s physician<br />

or chiropractor. Disabilities that qualify include<br />

visual problems, cardiac, arthritic, respiratory and<br />

orthopedic conditions.<br />

Determine that you meet the definition of disability according<br />

to State of Ohio regulations and that the medical condition<br />

can be certified as a qualified disability by a physician<br />

Vehicles (BMV) website. Download “For, BMV 4826” for disability<br />

plates. Complete the pertinent parts of the applicable<br />

form.<br />

Step 3<br />

Request a prescription from your physician or chiropractor<br />

which details the nature of the disability and how long the<br />

disability is expected to last. The application will be rejected<br />

if the expected ending date of the disability is not included.<br />

Indicate if the disability is permanent. Placards expire<br />

on the date specified by the physician or chiropractor but<br />

cannot exceed five years.<br />

Take the application form along with the prescription from<br />

the physician or chiropractor to any local Deputy Registrar’s<br />

State ID Card: Who Needs One; Where and How to Get One<br />

These days, you need an identification card even to purchase cold<br />

medicine!<br />

Senior citizens who’ve given up the wheel, as well as folks who aren’t<br />

U.S. citizens or don’t drive for any reason (such as age), still have to prove<br />

their identities. That’s where identification cards come in handy. No matter<br />

what the reason you need an ID, the local BMV is the one to administer<br />

a state identification card.<br />

What You Need to Apply<br />

Generally, only a few items are necessary when you apply for this card<br />

(but just like with driver’s licenses, it’s important to have just the right<br />

documents).<br />

Depending on which state you reside in, you will need or will need to<br />

provide:<br />

• A specific identification card application form.<br />

• Your Social Security number.<br />

• An official Social Security card.<br />

• Social Security number identification printout from<br />

the SSA.<br />

• Proof of your birthday and your legal name.<br />

• Certificate of birth (with a seal, must display gender).<br />

• Court order (certified copy) that includes full name and<br />

date of birth.<br />

• Driver license (current or expired less than 6 months).<br />

• Proof of legal presence.<br />

Plan Ahead<br />

card will be mailed to you. The<br />

Medical Verification<br />

After filling out the mailed<br />

packet, a customer can request a<br />

visit by a member of the BMV<br />

field staff, who will come to the<br />

The information about how to get<br />

medically verified is in the packet,<br />

but it is essentially a letter on doctor’s<br />

official letterhead stating the<br />

person is medically homebound or<br />

confined to their home, and which<br />
