The Medina County Senior Services 

Network (MCSSN) was established 

in 1994 by a small group 

of dedicated individuals who set 

out to develop, coordinate and act 

as advocates for a community of 

care network in Medina County. 

Over the years, MCSSN has 

evolved into a group with over 100 

members and organizations with 

one common mission: “To develop 

and coordinate a network of care 

and resources for older adults in 

Medina County.” For twenty years, 

we have done this through networking, 

sharing ideas, and publication 

of this guide. 

MCSSN’s first resource guide 

was produced in 1996. This publication 

was in response to a 

growing need for a comprehensive 

guide of services for senior citizens 

in Medina County. The need has 

now grown and become greater 

and, much like MCSSN, our guide 

has evolved into what it is today. 

It is our hope that MCSSN 

Senior Solutions, formerly The 

Source, provides you with a for- 

mat to easily navigate and find the 

products and services you need.

Medina County Office for Older Adults 

The Medina County Office for 

Older Adults is dedicated to helping 

those over age sixty and their 

families by providing education 

regarding benefits and services 

available in Medina County and 

beyond. Our services are extensive 

and range from simply providing 

referrals to helping seniors enroll 

in eligible programs. 

Transportation Services 

Transportation can be an issue 

for seniors and those with disabilities. 

We work with Medina Transit 

and visit people in their homes to 

register them for in-county transit 

trips. Subsidized trips include 

those for medical appointments, 

groceries or social service agencies. 

Registration with MCOOA entitles 

each participant to free loop 

rides in the cities of Brunswick, 

Medina and Wadsworth. 

Home Delivered Meals 

For those who are homebound, 

outreach workers make visits to 

determine eligibility for home delivered 

meals. Six routes throughout 

the county serve more than 

160 hot, nutritious, lunchtime 

meals at no charge. The Medina 

County Commissioners make 

these services possible with help 

from Title III funding provided by 

the Western Reserve Area Agency 

on Aging through the Ohio Department 

on Aging. 

Senior Centers and Services 

Medina County Office for Older 

Adults has one central senior center 

for programming throughout 

the county, located in Medina, and 

also coordinates senior services in 

Brunswick. 

Meal Programs, 

Entertainment & Activities 

We have meal programs in 

Medina, as well as a voucher program 

in Wadsworth and a congregate 

program in Brunswick. Our 

center is a fun place for seniors to 

gather with friends, new and old, 

and enjoy a tasty meal with good 

conversation. Guests are offered 

fun activities such as Senior Day 

in May and Picnics on the Patio 

throughout the summer. 

Other entertaining and informative 

activities in our senior center 

include: Lunch and Learn programs, 

day trips, weekly exercise 

classes, card games, yoga classes, 

bingo and special celebrations for 

holidays. 

Seasonal Programs 

Seasonal programs offered 

include Farmer’s Market, HEAP 

and free tax preparation by AARP 

volunteers. Our programs are 

featured in our newsletters, available 

in Brunswick and Medina, or 

online at www.mcooa.org

The Center is the premier gathering 

place for seniors filled with

on the vibrant multigenerational 

Wadsworth High 

School & Community Center 

campus, the Center serves the 55+ 

population of Wadsworth and surrounding 

communities. In addition 

to ample and accessible space for 

senior programming, the Center 

also features a full-service Café that 

is open to the public, offering fresh 

and tasty food at reasonable prices. 

Unique Gathering Place 

The Center takes great pride in 

offering traditional senior programming 

paired with a wide array 

of new and innovative opportunities. 

Programs and classes focus on 

the social, recreational, educational, 

and health/wellness preferences 

of our members. 

The Café is operated by Windfall 

Industries and is staffed by the 

Medina County Board of Developmental 

Disabilities. This innovative 

partnership also includes the 

City of Wadsworth and Medina 

County Office for Older Adults. 

These entities work collaboratively 

to provide a dining hot spot and 

unique gathering place, as well as 

a training site for individuals with 

developmental disabilities.

couldn’t be easier. Through the 

support of the City of Wadsworth, 

the Wadsworth Older Adults 

Foundation and over 80 Professional 

Friends, Center membership 

is free for the 55+ crowd. 

Membership offers a number 

of perks such as the quarterly 

newsletter (mailed to Wadsworth 

residents), designated times on the 

YMCA indoor walking track and 

a 10% discount at the Café.

Medina County Senior Centers 

Senior Service 

Coordination 

The Medina County Office for 

Older Adults (MCOOA) has 

partnered with the City of Brunswick 

to offer seniors service coordination 

at the Brunswick Recreation 

Center. Five days a week, an 

MCOOA employee is at the Rec 

Center to offer service assistance, 

referrals to programs and other 

support to seniors in the area. 

Nutrition Programs & 

Free Screenings 

The MCOOA provides a nutrition 

program at the Rec Center 

twice weekly for seniors who 

register. Any adult age 60 or over 

is qualified for this program. Various 

free health screenings are also 

available on-site from local professionals 

and agencies. 

Indoor/Outdoor 

Senior Activities 

There is an amazing variety of 

activities offered to Brunswick area 

seniors through the Rec Center, 

including: Weekly free lunch-andlearns, 

bingo, Mah Jongg, Single 

Men’s Club and Single Ladies 

Club as well as Couples Night 

Out, Hiking and Fishing Clubs, 

Line Dancing, Baking and Gardening 

Clubs, knitting, art classes, 

and much more. 

Silver Sneakers, 

Concerts & Travel 

The Brunswick Recreation 

Center is proud to offer Silver 

Sneakers classes to all who qualify. 

And don’t forget to check out the 

Brunswick Recreation Center’s 

summer concert series for your 

listening and dancing enjoyment. 

Trips are coordinated through 

both the Medina and Brunswick 

locations offering monthly casino 

trips as well as many non-gambling 

excursions several times 

throughout the year.

keeps everything on the calendar

takes place every January. Over 60 

vendors that provide services for 

seniors are on-hand to meet and

Case Management in Medina County for Seniors 

Case management is a service 

provided in the home to seniors, 

their caregivers, and their families. 

It often helps families plan 

for life’s next steps or get them 

through a crisis by defining the 

immediate issues, setting achievable 

goals and close support to 

help meet those goals. While there 

are several different types of case 

management service providers, 

including insurances, we offer 

some specialty services in Medina 

County. 


Adults / Options Counseling 

Program - MCOOA offers short 

term case management to individuals 

or families who are ready to 

look at alternatives to keep their 

aging loved ones in the home 

or to transition into alternative 

living arrangements. Many times, 

as people grow older, they need 

assistance to keep aging in place. 

A case manager helps seniors and 

their caregivers assess their current 

living situation and see what 

resources are available to help 

them, whether it is financial help, 

linking to other services, nutritional 

help or other supportive services. 

MCOOA case management 

is voluntary and free of charge.

Emotional Wellness and Catholic

– Catholic Charities Senior Case 

Management program is a more 

intensive case management service. 

It offers both in home counseling 

and case management for seniors

their ability to succeed in the community. 

Catholic Charities Senior 

Case management can assist with 

adjusting to different stages of life. 

Services may be available at no 

cost and insurances can be billed 

for services. It is available to any

Adult Protective Services, Medina 

County JFS – Sometimes, 

older adults in the community 

fall victim to physical abuse and 

neglect, self-neglect or financial 

exploitation. Medina County 

Adult Protective Services (APS) 

investigates all allegations of the 

abuse or neglect of adults age 

60 and over. Investigations can 

involve case management to help 

alleviate the causes of maltreatment. 

This could include securing 

service providers in the home, 

obtaining medical treatment for 

victims of abuse or neglect, police 

involvement or court-ordered 

services as well as other supportive 

action. All APS investigations and 

any case management services that 

result from these investigations are 

provided at no cost. Referrals for 

suspected abuse or neglect of an 

older adult can be made by calling

How to Get a Card 

Ohio residents who hold current 

Ohio Driver Licenses or State 

ID cards automatically receive 

their Golden Buckeye cards in 

the month of their 60th birthdays. 

Individuals who do not have a 

current license or State ID, as well 

as people under age 60 who are 

eligible due to disability, or who 

otherwise do not receive their 

card automatically, may apply for 

a card at most public libraries and 

senior centers. Due to ID, age and 

disability (when appropriate) verification 

requirements, you cannot 

apply for a Golden Buckeye card 

online or by phone. 

How to Get a 

Replacement Card 

If your Golden Buckeye card 

has been lost, stolen or damaged, 

call 1-866-301-6446 to request a 

replacement. 

Where to Use Your Card 

Since its inception, the Golden 

Buckeye program has been a small 

business program. Thus, you are 

most likely to find locally owned 

small businesses who honor the 

card. Merchants of all types (e.g., 

restaurants, retail, auto care, medical 

and more) voluntarily offer 

special savings or deals for Golden 

Buckeye cardholders. [The savings 

percentage off a total purchase or 

special deals on certain products or 

services.] Limitations may apply. 

Ask anywhere you spend money 

about a Golden Buckeye or senior 

discount. Some participating 

merchants display the Golden 

Buckeye logo on their door, in 

their window or at the checkout, 

but many do not. Unfortunately,

WRAAA helps older adults and persons with disabilities 

meet their needs and achieve the highest possible 

quality of life. We are the state-designated funding, 

planning and coordinating authority for home and

How to Obtain a Disabled Parking Permit in Ohio 

Anyone who has a disability that limits mobility by 

impeding the ability to walk can apply for an Ohio 

Disability parking placard or disability plates. The 

disability must be certified by the person’s physician 

or chiropractor. Disabilities that qualify include 

visual problems, cardiac, arthritic, respiratory and 

orthopedic conditions. 

Instructions Difficulty: Moderately Easy 

Step 1 

Determine that you meet the definition of disability according 

to State of Ohio regulations and that the medical condition 

can be certified as a qualified disability by a physician 

or chiropractor. 

Step 2 

Obtain the application forms from the Ohio Bureau of Motor 

Vehicles (BMV) website. Download “For, BMV 4826” for disability 

plates. Complete the pertinent parts of the applicable 

form. 

Step 3 

Request a prescription from your physician or chiropractor 

which details the nature of the disability and how long the 

disability is expected to last. The application will be rejected 

if the expected ending date of the disability is not included. 

Indicate if the disability is permanent. Placards expire 

on the date specified by the physician or chiropractor but 

cannot exceed five years. 

Step 4 

Take the application form along with the prescription from 

the physician or chiropractor to any local Deputy Registrar’s 

office, or mail it to Ohio BMV at the address on the form. 

Enclose the proper fee amounts as shown on the form.

State ID Card: Who Needs One; Where and How to Get One 

These days, you need an identification card even to purchase cold 

medicine! 

Senior citizens who’ve given up the wheel, as well as folks who aren’t 

U.S. citizens or don’t drive for any reason (such as age), still have to prove 

their identities. That’s where identification cards come in handy. No matter 

what the reason you need an ID, the local BMV is the one to administer 

a state identification card. 

What You Need to Apply 

Generally, only a few items are necessary when you apply for this card 

(but just like with driver’s licenses, it’s important to have just the right 

documents). 

Depending on which state you reside in, you will need or will need to 

provide: 

• A specific identification card application form. 

• Your Social Security number. 

• An official Social Security card. 

• Social Security number identification printout from 

the SSA. 

• Proof of your birthday and your legal name. 

• Certificate of birth (with a seal, must display gender). 

• Court order (certified copy) that includes full name and 

date of birth. 

• Driver license (current or expired less than 6 months). 

• Proof of legal presence. 

• Certificate of birth (certified with a seal, must display gender). 

• Valid U.S. passport or U.S. passport card. 

• Naturalization document issued by the U.S. 

• Proof of Ohio residency. 

• Current insurance policy (automobile, life or premises liability). 

• Ohio certificate of title. 

• Utility bill (electric, phone, cable, etc.). 

• Have your photo and thumbprint taken by an official 

staff member. 

• Pay a nominal fee of $8.50. 

Plan Ahead 

The BMV does NOT take appointments for Ohio 

ID card applications. The time it takes to receive your 

identification card also varies so if you need your card 

in a hurry, make sure to plan 

ahead. Once you have completed 

the process, your new Ohio ID 

card will be mailed to you. The 

OH BMV will issue you a temporary 

document for the meantime.

Medical Verification 

After filling out the mailed 

packet, a customer can request a 

visit by a member of the BMV 

field staff, who will come to the 

customer to verify the paperwork 

and take the photo. This service is 

only available for state IDs and the 

person must be medically verified. 

The information about how to get 

medically verified is in the packet, 

but it is essentially a letter on doctor’s 

official letterhead stating the 

person is medically homebound or 

confined to their home, and which 

is signed and dated by the doctor.
