Stigma, said Botticelli, even
manifests itself in clinical care,
in the language used to describe
those with substance abuse disorder,
and in media coverage.
In medical training, he continued,
the subject of addiction has
often been optional, (although he
has worked to ensure that every
doctor now has at least minimal
treatment in SAD), and both
public and private insurance
have not offered treatment benefits.
In Massachusetts, said Botticelli,
“Only 10-14% of (those
with addiction) get care and
treatment. 85% of people with
Diabetes get care. Why? We usually
wait until people reach the
most acute level before we refer
them. It’s not the same with hypertension.”
Botticelli noted that only 8%
of those referred to treatment
come from health care centers,
while the plurality (36%) are
coming from the criminal justice
system.”
In addition, Botticelli pointed
to what’s going on in the community
as playing a role in the
epidemic. The declining life expectancy
among men, for example,
he said, was related to drug
overdose, suicide, and diseases relating
to alcoholism -- also known
as diseases of despair. Are community
members lacking in education,
are they socially isolated,
engaged in school or in faith, he
asked. Community engagement
INVITE US FIRST.
was important in stemming
harmful behaviors.
“The good news is, we have
solutions,” said Botticelli, who
added he wanted to support
those who are tackling the problem
on a local level. Preventing
early use, for example is important
(and more cost effective than
treatment) as is employing highly
effective and under-utilized pharmaceutical
treatments.
“People do better when
they’re on medicine and they
get the support they need,” said
Botticelli, “but only 5% of those
who have had a nonfatal overdose
get on methadone.” The
expert praised law enforcement
for its role in pushing for treatment
options, but lamented that
those who have suffered an overdose
are 120 times more likely to
suffer a fatal overdose than the
general public. Parent education
and support is crucial, he said, as
well as is employer support.
The fundamental solution,
said Botticelli, will be what happens
when everyone works together
to attack and address this
problem. In early November,
The night wrapped up with a
question and answer session, as
well as a S.A.F.E. Coalition impact
statement by S.A.F.E. Vice
President Jim Derick, and one
guest’s personal story of her own
struggle with addiction.
