Mayra Martinez conducted an awareness-level Tool Safety Training at the <strong>ATD</strong> Foreman’s Meeting held on<br />

11/20/19.<br />

• Power Tool Safety<br />

• Extension Cord Safety<br />

• Guards<br />

• Hazard and Hazard Prevention.<br />

CONCLUSIONS All Foremen were present for training. Foreman are now to impart and train their crews on the information above.<br />

10.5 Foreman’s Meeting<br />

A Foreman’s Meeting was scheduled for November 20, 2019.<br />

DISCUSSION<br />

During the foreman’s meeting consisted of the following:<br />

• Tool Safety Training (See 10.4)<br />

• Pre-Construction Hazard Analysis re-training<br />

• Honoring the two foremen with the overall highest scores for the 2 nd and 3 rd quarter of 2019<br />

• Truck Inspection Policy<br />

• Assigning Rudy, the Yard Foreman, to spotter duty during traffic hours in the yard.<br />

CONCLUSIONS<br />

All foremen were present for the meeting. <strong>ATD</strong> Management and Mayra Martinez spoke with Rudy and assigned him<br />

to become the spotter for all <strong>ATD</strong> Company Trucks and other vehicles in operation.<br />

360 Degree Evaluation<br />

10.5 Barrier Identification<br />

On 11/25/2019, Mayra Martinez utilized the method of Barrier Identification to identify the root the deficiencies<br />

discovered during an evaluation with Foreman Sal Flores.<br />

Sal Flores failed to obtain all paperwork and required documentation due to not having his backpack on site.<br />

DISCUSSION<br />

Sal Flores stated that a previous emergency arose while on site, causing him to leave behind his backpack with all<br />

<strong>ATD</strong> Safety Plans, Incident/Emergency Procedures, Pre-Construction Hazard Analysis, Toolbox, and OSHA<br />

required Posters.<br />

According to <strong>ATD</strong> Management Safety Program Policy, all paperwork and documentation is to be kept behind or<br />

beneath the Company Truck.<br />

Foreman Sal Flores did not follow the standard, therefore, all required documentation was lost.<br />

CONCLUSIONS <strong>ATD</strong> Management must require and enforce all Foremen and <strong>ATD</strong> Employees to meet company standards.<br />

ANTHONY TORRES SAFETY SYNOPSIS<br />

November 2019<br />

Last quarter we implemented 360-degree evaluations in an effort to gauge the overall safety culture and to identify any barriers to<br />

carrying out <strong>ATD</strong>’s safety guidelines.<br />

As a result, we were able to identify barriers such as literacy, lack of training or knowledge, as well as time management and<br />

engagement with upper management. Below are our suggestions for mitigation strategies that <strong>ATD</strong> can implement during the final<br />

quarter of 2019. We have concluded our complimentary 360-degree evaluations and will now continue our safety evaluations, providing<br />

additional awareness level trainings and helping to bring the <strong>ATD</strong> team into compliance.<br />

SUMMARY OF MEANS<br />

The methods of mitigation strategies include:<br />

§ 360 Virtual Assessments<br />

§ Awareness Level Training<br />

These measures will help us ascertain the leading<br />

and lagging indicators, which will help us to<br />

implement strategies to eliminate or decrease<br />

incidents and injuries.<br />

Demolition Policy.<br />

Worker's Statement:<br />

Sal Flores stated, “I had an emergency a couple weeks ago, so I had to leave the site. I forgot to grab my backpack<br />

with all my forms and tablet in it. I don’t know where it is, it still hasn’t been turned in. I asked for another packet, but I<br />

haven’t gotten it yet.”<br />

What Corrective Measures were taken/or suggested: Mayra Martinez discussed the importance and requirement to<br />

have all documentation on site with Foreman Sal Flores. She coached him on maintaining the standard to protect<br />

self, workers, and the company. It is suggested to enforce <strong>ATD</strong> policies across the board with all foremen.<br />

Week 1<br />

Anyone working in a cold environment may be at risk for<br />

cold stress. This could include an indoor workplace like cold<br />

storage or an outdoor job in construction or agriculture.<br />

Prolonged exposure to cold and/or freezing temperatures<br />

while on the job may cause serious health problems such as<br />

trench foot, frostbite and hypothermia. In extreme cases,<br />

exposure to cold temperatures can lead to death.<br />

Danger signs include uncontrolled shivering,slurred<br />

HEALTH HAZARDS || Cold Stress<br />

COMMON TYPES OF COLD STRESS:<br />

Hypothermia is when the normal body temperature (98.6⁰F) drops to less than<br />

95⁰F. Hypothermia can occur temperatures above 40⁰F if one is chilled from rain,<br />

sweat or cold water.<br />

Symptoms: uncontrollable shivering, loss of coordination, confusion, and slurred<br />

speech.<br />

First Aid: Move the worker to a warm, dry area, remove wet clothing and<br />

replace with dry clothing,. Wrap the entire body in layers of blankets and<br />

seek medical attention immediately.<br />

Frostbite occurs when body tissues freeze and can occur at temperatures<br />

above freezing due to<br />

wind and<br />

may result in permanent damage or amputation.<br />

Frostbite typically occurs on fingers, toes,nose and ears.<br />

Symptoms: numbness, red skin that may develop<br />

gray/white patches, skin feels hard, sometimes blisters.<br />

First Aid: Protect area by wrapping loosely in dry cloth.<br />

Do not rub the affected area, do not apply snow or water,<br />

do not break blisters and do not attempt to re-warm the<br />

area. Seek medical attention immediately.<br />

IMPORTANT TIPS TO PREVENT COLD STRESS:<br />

‣ Recognize environmental and workplace conditions<br />

that may be dangerous.<br />

‣ Wear proper clothing for cold, wet and windy<br />

conditions.<br />

‣ Take frequent short breaks in warm dry shelters.<br />

‣ Schedule work for the warmest part of the day.<br />

‣ Avoid exhaustion or fatigue.<br />

‣ Keep extra clothing handy in case clothes get wet.<br />

‣ Drink warm, sweet beverages and avoid drinks with caffeine or<br />

alcohol.<br />

‣ Eat warm, high-calorie foods.<br />

‣ Learn the signs and symptoms of cold-induced illnesses and<br />

injuries.<br />

‣ Use the buddy system – work in pairs.<br />

‣ Stay dry in the cold because moisture or dampness, even from<br />

sweating, can increase the rate ofheat loss from the body.<br />
