Though those funds have expired, it’s likely the collaborative relationships will remain and new value-based models will emerge. Ultimately, Hughes said, there’s a universal recognition that no one health system is strong enough to do everything on its own. Hospitals need ambulatory-surgery centers and primary care needs specialty services. “You will start to see a realization from everyone that we are stronger together,” he said. The ongoing integration of smaller practices into larger, multi-group practices and hospital-employment arrangements will continue. Without those, many smaller practices almost certainly would not have survived shutdowns during the pandemic. Those relationships allowed many Optimum practice groups to receive tech support for telehealth, attain access to personal-protective equipment in group-purchasing arrangements and demand advance payments from insurers to ensure a steady cash flow. “If you were truly an independent physician, you would find yourself very exposed at the moment,” Hughes said. “Whether it’s through emerging deals or tight partnerships, you’re going to see more a year from now.” Electronic scheduling Electronic scheduling for appointments and tests is almost certain to remain in place. Though some labs and physician offices have offered that through patient portals and electronic health systems, the practice has become more widespread in recent weeks. “Historically, health care is one of the most backward industries in the country, so we’re very slow adapters,” Mills said. “This may radically shake up some of that and how we think about ourselves and maybe, fast-forward health care into a more competitive and maybe more nimble industry that doesn’t drag its feet as much as it has.” Options for patients, providers Catholic Health is banking on telehealth continuing as a bigger part of its future. The Covid-19 pandemic led to the development of a new on-demand care platform for its 10,000 associates in tandem with Independent Health, allowing free virtual visits. And now the system is deploying 2,000 digital units to its home-care patients to stay connected with caregivers. “We’ve worked for years using telemed or telemonitoring, and now they’ve been bringing in additional telemonitoring equipment that has a Covid-19 protocol in it,” said Joyce Markiewicz, executive vice president and chief business development officer. The program was added to the computers of all home-care nurses, allowing them to complete virtual visits with patients who want to minimize potential exposure to the virus. “We want to see the patients face to face, but there are some saying, ‘I need the care but I don’t want people coming into my home,’” she said. “The beauty of it is, it gives them an alternative to still see those patients.” New destinations for care The Centers for Medicare & Medicaid Services will now pay for more than 80 additional services furnished via telehealth. These include emergency-department visits, nursing-facility and discharge visits and home visits provided by a clinician. The changes allow for video and audio calls.
